In tro d u ctio n : T he carcinoem bryonic antigen, C E A , is the tum or m arker m ost used in colorectal patients, principally during follow up after radical surgery. H igh serum C E A levei before surgery is often associated w ith w orse prognosis, in som e studies. O b je ctive : T he purpose of this study w as to evaluate the preoperative carcinoem bryonic antigen leveis (C E A ) and the frequency of recurrence. M a te ria l a n d M e th o d s: E ighty-three patients w ith colorectal cancer at D ukes stages A , B or C w ere evaluated retrospectively. T he patients'follow up w as at least tw o years or to death. C E A w as detem ined in serum by enzym e im m unoassay (S orin B iom edica), norm al value 0-5ng/m l. R e su lts: D isease recurrence w as observed in 32 patients (38.5% ), 13 D ukes B and 19 D ukes C . S eventy five per cent of the patients w ith C E A higher than 1 O ng/m l relapsed and 80% of the patients w ithout recurrence had norm al C E A . D isease recurrence in patients w ith preoperative elevated C E A occurred during the first year of follow up in 56% of the patients. C o n clu sio n : A lthough the tum or stage is today the m ost valuable prognostic variable in colorectal cancer, the preoperative C E A value can provide som e additional inform ation in the prognosis of the patient.

Introdução: O antígeno carcinoem brionário, C E A é o m arcador tum oral m ais usado em pacientes com câncer colorretal, principalm ente no acom panham ento clinico após ressecção cirúrgica do tum or. N íveis .séricos elevados de. C E A no pré-operatório são frequentem ente associados, em alguns estudos, a pior prognóstico. O bjetivo: C om parar os níveis séricos de C E A no pré-operatório e a frequência da recorrência no acom panham ento clinico. M aterial e M étodos: O itenta e três pacientes com câncer colorretal estadiados em D ukes A , B , e C foram avaliados retrospectivam ente. O s pacientesforam acom panhados, por no m ínim o dois anos ou até o óbito. O C E A sé rico foi determ inado por E lisa (S orine B iom édicas, valor norm al 0-5 ng/m l). R esultados: A recorrência foi observada em 32 pacientes (38.5% ), sendo 13 pacientes D ukes B e 19 D ukes C . S etenta e cinco por cento dos pacientes com C E A m aior que 1O ng/m l apresentaram recorrência e oitenta por cento dos pacientes sem recorrência tinham níveis sé ricos de C E A dentro do valor de norm alidade. N os pacientes .com C E A elevado no pré-operatório e que desenvolveram recorrência, esta ocorreu no prim eiro ano de acom panham ento em 56% dos pacientes. D ezesseis pacientes foram a óbito durante o acom panham ento, 11 destes tinham C E A sérico aum entado no pré-operatório. C onclusão: E m bora, o estadiam ento clinico seja a variável m ais usada na avaliação do prognóstico, os valores de C E A no pré-operatório podem fornecer algum as inform ações adicionais sobre o prognóstico do doente.
