SATISFACTION/MOTIVATION OF NURSES: INTEGRATIVE REVIEW

SATISFAÇÃO/MOTIVAÇÃO DO ENFERMEIRO: REVISÃO INTEGRATIVA

SATISFACCIÓN/MOTIVACIÓN DE LAS ENFERMERAS: REVISIÓN INTEGRADORA

ABSTRACT

Objective: analyzing the scientific production about satisfaction/motivation of nursing work. Method: an

integrative review, held in LILACS and Bdenf databases, and SciELO Library between January and February 2013. The search was guided by the following question << What was produced in Brazil about satisfaction/motivation in nursing work in online journals between 1980 and 2012?>>The classification sample regarding the methodological quality consisted of 4 levels of evidence. Results: 30 articles characterized byyear of publication, journal, methodological design and thematic trend. The beginning of publications in 1980, 17% qualitative approach, 43% quantitative and 40% quantitative and qualitative; 77% of articles in specific journals in nursing, 23% in general periodics. Job satisfaction of nurses worked more thematic trend, followed by motivation and motivation/satisfaction. Conclusion: there are gaps in the literature about the satisfaction/motivation of nurses, especially surveys involving specific populations and extra hospital.

Objetivo: analisar as produções científicas sobre satisfação/motivação no trabalho do enfermeiro. Método:revisão integrativa, realizada nas bases de dados LILACS e Bdenf, e na biblioteca SciELO, entre

janeiro e fevereiro de 2013. A busca foi pautada pela seguinte questão << O que foi produzido no Brasil sobre satisfação/motivação no trabalho do enfermeiro em periódicos on-line entre 1980 e 2012? >> A classificação da amostra com relação à qualidade metodológico consistiu no nível 4 de evidência. Resultados: 30 artigos, caracterizados quanto ano de publicação, periódico, delineamento metodológico e tendência temática. Início das publicações no ano de 1980, 17% abordagem qualitativa, 43% quantitativa e 40% quanti-qualitativa; 77% dos artigos em revistas específicas da enfermagem, 23% em periódicos gerais. Satisfação no trabalho do enfermeiro com tendência temática mais trabalhada, seguido por motivação e motivação/satisfação.

Conclusão: existem lacunas nas publicações sobre a satisfação/motivação do enfermeiro, sobretudo,

levantamentos envolvendo populações específicas e extra-hospitalares. Descritores: Saúde do Trabalhador; Motivação; Satisfação no Emprego; Enfermagem.

Objetivo: analizar la producción científica sobre la satisfacción/motivación en el trabajo de enfermería. Método:una revisión integradora, que se celebró en las bases de datos, Bdenf, LILACS y Biblioteca SciELO,

entre enero y febrero de 2013. La búsqueda se basó en la siguiente pregunta << ¿Lo qué fue producido en Brasil acerca de la satisfacción/motivación en el trabajo de enfermería en revistas on line, entre 1980 y 2012? >>La clasificación de la muestra con respecto a la calidad metodológica consistió en el nivel 4 de evidencia.

Resultados: 30 artículos, que se caracterizaron por el año de publicación, periódico, tendencia metodológica

y temática de diseño. Con el inicio de publicaciones en 1980, el 17% de enfoque cualitativo, 43%cuantitativo y el 40% cuantitativo y cualitativo; 77% de los artículos en magazines específicos de enfermería, el 23% en las publicaciones periódicas de carácter general. La satisfacción laboral de las enfermeras trabajó en tendencia más temática, seguida de la motivación y la motivación/satisfacción. Conclusión: existen lagunas en la literatura sobre la satisfacción/motivación del personal de enfermería, en especial las encuestas con poblaciones específicas y extra hospitalaria. Dissatisfaction at work by employees seems to prevail in relation to the satisfaction of various institutions whether healthcare or not, public or private. This dissatisfaction leads to harmful effects for both the institution and for the worker, reflecting on health issues for the employee and the company, low productivity, extra spending on recruitment, selection, training, and training of new professionals, among others. While there seems to be this general dissatisfaction among workers regarding companies what we see is a

constant search to give employees

increasingly motivated.1

To begin thinking about this scenario, it is necessary to understand the concepts of satisfaction and motivation. Thus, for satisfaction is understood as "the fulfillment of a need or disposal"1:24or "indirect or

extrinsic energy, linked to aspects such as salary, benefits, recognition, leadership, colleagues and several other conditions that must be met ".1:98However, motivation is

understood as "an inclination for action coming from a motive (need)"1:24 or“direct or

intrinsic energy, linked to the meaning and nature of work itself done”.1:98It is noteworthy

that reason is defined as "everything that drives a person to acting in a certain way or at least that gives rise to a propensity to a specific behavior".2:76

From these concepts, it appeared that motivation is generated from a worker's own need, ie, something individual; secondly satisfaction stems from the completion / attendance a need. Therefore, institutions are only able to offer elements to employees those assist to meeting their own needs, not being able to motivate employees, because what will satisfy / meet the need of the worker being not the necessity (reason) itself.1,2

About nursing work there are some features like act in the promotion and protection as the rehabilitation and recovery of the users, individually or collectively, performing various activities in diverse locations, involving assistance actions, administrative, teaching and research.3Moreover it is characterized by

a hierarchy of work (among nursing workers themselves, within multidisciplinary teams).3

It is a predominantly female profession since its founding, with the female figure / maternal feeding, care and teaching, reflecting today, where there is a predominance of nurses in institutions with work overload in the workplace itself, added to the domestic work and responsibility / duty

to educate her children, which leads to accumulation of functions, overloading this woman.4

Another aspect of this work is related to hours of workthat many nurses do double and triple shifts.5Added to this the fact that the

work of nursing as a whole is targeted both as generator of suffering, as the pleasure in their professional.6 Thus, especially by the

characteristics of their work, dissatisfaction among nurses is also present, beginning with the work environment, especially the hospital, that the very nature of this site and the activities performed in this environment often harmful to health professional eventually make it unsuitable for professional practice.7In addition, there are low wages,

lack of social recognition, depreciation, the conflicting relationships, the overhead of activities, highlighted as sources of dissatisfaction by these workers, impacting negatively on the professional, personal life and the quality of care given to the patient and the institution itself.1,2,7,8

Facing these considerations, the interest comes to identifying the Brazilian scientific production, the thematic satisfaction and motivation of nurses in their various contexts. Such research is justified for knowledge and understanding of the Brazilian production on the theme, in order to contribute to the development of future research in this area.

● Analyzing the scientific productions on satisfaction/motivation at work of nurses.

The study consists in an integrative literature review, obeying systematically the methodology, through which the reader can identify the main features of the publications,

providing knowledge synthesis and

incorporation of the applicability of study results.9

The guiding question was proposed << What was produced in Brazil about satisfaction / motivation in nursing work in online journals between 1980 and 2012? >>There search was conducted on the databases of Latin American and Caribbean Health Literature on Health Sciences (Lilacs) and Database of Nursing (Bdenf) and the Electronic Library SciELO Brazil. For that we made use of the Health Sciences Descriptors Headings (MeSH) available on the Virtual Health Library (VHL). The descriptors used in the library and the databases were: Motivation, Job Satisfaction, Nursing, Nurses and Nurse Women. The

combination of descriptors was performed using the Boolean operator "and".

The data collection was conducted between January and February 2013, and there were selected 69 publications those have examined the theme, and the sample was composed of 30. The classification of the sample with respect to methodological quality consisted of the level 4 of evidence.

During the search of the articles, considering the proposed study objectives, the works those were not made in Brazil were excluded; articles those, despite having cited the descriptors, did not specifically address the proposed theme; articles related to motivation / satisfaction involving the nursing staff (nursing auxiliary and technical) and / or multidisciplinary teams and / or graduate students in nursing. Theses and dissertations were also excluded by selecting only the items because they were widely circulated in journals.

The inclusion criteria defined for selecting studies that comprised the final sample consisted of the following items: articles published in national journals; journals indexed in SciELO Brazil library and Lilacs and Bdenf databases; articles those work the thematic satisfaction and / or motivation involving the nurse in several areas.

The investigation period covered the years 1980 to 2012, due to the fact of the first Brazilian production about the subject appear in this period. The first step was to search in reading the summaries whose titles demonstrate equivalence to the topic searched,followed by the step of reading in full of articles in order to characterize them

as the year of publication, which were published in journals, methodological design andthematic trend.

As for the characterization of the methodological design it was used the benchmark of Polit, Beck and Hungler, that

includes the following categories:

Quantitative studies (experimental,

quasi-experimental and non-experimental),

Qualitative (ethnography, phenomenology and grounded theory) and Quanti-qualitative. For insertion and tabulation of data it was used Microsoft Excel, analyzing them later quantitative / qualitatively.

The number of articles found was 840 publications, 115 in SciELO Brazil, 412 in the Lilacs and 313 Bdenf. Initially articles were selected based on reading the title and abstract of each article that had reference to the subject studied. Thus, seven articles were selected in SciELO Brazil, 38 in the Lilacs and 24 Bdenf in the database, making a total of 69 articles, which were read in their entirety. After reading the repeated items and items those did not meet the inclusion criteria or met any of the exclusion criteria, especially by involving the nursing staff as a whole, multidisciplinary teams, patient satisfaction, among others, were excluded. Thus, the final sample consisted of 30 items related to the thematic motivation and satisfaction of nurses,according to explicit characterization in Figure 1.

It is evident, based on Figure 1, that the selected articles specifically address the issue of satisfaction and / or motivation of the nurse and have been published in different

journals and years, with varying

methodologies, since they consist of qualitative studies, quantitative and

qualitative and quantitative and approaching diverse themes.

In Table 1 the data are presented in relation to the year of publication, as well as the percentage of papers published per year. We notice that the work begin to emerge in the 80s with only three publications. However, from the year 2003 there is an

increase in the production of knowledge generated in the area of professional satisfaction / motivation, showing a linear

trend in scientific production starting this year, concentrating 53% of publications in a period of nine years.

Table 2 shows the distribution of articles regarding the methodological design used, according to Polit, Beck and Hungler. Facing the complexity of variables involved and

determinants in satisfaction and/or motivation of nurses found the use of different methodological guidelines for the investigation of the phenomenon under study.

When assessing the most frequent methodological designs in our sample, we found that five (17%) used a qualitative approach 13 (43%) developed studies with quantitative methods and 12 (40%) conducted studies on quantitative and qualitative approach. From these results, it is understood that the knowledge produced by nursing on motivation / satisfaction of nurses, was largely focused on the description of situations at/of work translated into

motivation / professional satisfaction, employing for this purpose, outlining the diversified research.

Table 3 shows the journals that published articles involving the topic under study. It was found that the journals that published the largest number of related motivation / satisfaction of nurses articles were "Journal of the School of Nursing" and "Gaucho Journal of Nursing" each with four (13%) publications.

Another aspect to be noted in Table 3 is the presence of 23 publications, ie, 77% of articles published in specific journals of nursing, while seven (23%) were published in general journals.

Table 4 presents the data on the thematic trends of the studies are presented. With the intention of organizing the data, thematic trends were grouped into three broad

categories, namely: satisfaction, motivation and motivation / job satisfaction, regardless of the items work satisfaction and / or motivation themselves or dissatisfaction and / or demotivation of nurses.

It is evident in Table 4 that the job satisfaction of nurses was the most searched category, obtaining 23 (77%) articles found, followed by motivation with four (13%) and motivation / satisfaction with three (10%). Analyzing satisfaction category, it is noted that six (20%) articles have focused on the various sectors, being involved in group work addressing several units of hospital and / or different hospitals in the same geographic area and / or various departments within a hospital and / or hospitals and other health care facilities where the nurse works.

The nurses’ profession and its

characteristics have been analyzed by the International Labor Organization (ILO) in

association with the World Health

Organization (WHO) .10During the 70’s, during

the 61st Annual Conference of the ILO, in poor

conditions this work was professional discussion agenda, associated with journeys and unsuitable working shifts, lack of rest periods, and especially the non-participation in planning and decision making directly related to professional practice.10

among researchers, professionals and institutions to evaluate satisfaction of nurses. This fact is evident in our literature search with the emergence of publications focusing study this topic of research since the 80’s, as shown in Table 1.

The appearance of this work in the 80’s can also be justified in part by the advent of postgraduate programs which begin to train qualified professionals in occupational health.11Thus, evidence that teaching the

graduate is a contributing for the development of Brazilian nursing and building their knowledge based on research.11,12

Allied to teaching post, add policies that invest in the areas of scientific development, coupled with the growing number of nursing journals and strengthening of groups of nursing research, dedicated to studying the work of nursing and health workers as relevant publications related to increasing the health of the nursing work.13

In 2001 is established by the Ministry of

Health, the National Program for

Humanization of Hospital Care (PNHAH), which aims to transform the models of care offered to people in public hospitals in Brazil aiming to quality, efficiency and humanization of services.14In 2003, this program is

reviewed, and then place the seat PNHAH National humanization Policy (NHP), increasing humanization to all devices and health activities of the Unified Health System (SUS).14,15Thus, the search for professional

enhancement arises , with better / at work for those involved, emerging as a factor in improving health professional.14,16

From the year 2003 it can be seen in Table 1 based on an expansion in the production of knowledge generated in the area of job satisfaction, with at least one published work per year.

This concentration of works from the year 2003 may be associated with PNH and PNHAH,

aimed at the enhancement of the

professional, the pursuit of autonomy of these workers and their motivation and professional development, which are directly related to job satisfaction.14,16 Moreover, the PNH itself

proposes a strategy of conducting research related to job satisfaction.16

Methodological design is the way the investigator will conduct research in order to answer the research question.17In nursing

research it demonstrates the use of qualitative methodological design highlighted with.18 However, in this study, it is observed

the prevalence of articles with a quantitative approach.

Looking at Table 2, it is possible to identify a significant number (40%) of studies using quantitative and qualitative methodologies, this combination of quantitative and qualitative methods in the same research has been increasingly observed in studies in nursing and has advantages as the completion and the increment between quantitative and qualitative data, further validation of the results and the emergence of new research hypotheses based on previous research.9,17

When referring to Table 3, it appears that the "Journal of the School of Nursing" and "Gaucho Journal of Nursing" constituted the main vehicle for the dissemination of the theme, which can be explained because they are related to periodic higher education institutions and research reference in nursing.

The "Journal of the School of Nursing" presented highlighted in other research related health healthcare worker, thus corroborating the finding in our study.19

Another noticeable aspect is that 23 publications, ie, 77% of the articles were published in specific journals of nursing, while 23% were published in general journals. This result is opposed to that found in literature review related to oncology nursing, in which most of the results obtained was in general periodicals published.20However, in

bibliographic addressing prostate cancer and the production of nursing are is similar to our study, in which 72,7% of the articles were published in nursing journals results, confirming the result.21

In examining Table 3, it appears the lack of publications in journals specific to healthcare worker, which can be translated into trouble for discussion and update of the theme.

Taking a specific focus on the thematic trends, as shown in Table 4, it is interesting to note the absence of studies focused on specific areas of nursing knowledge such as pediatrics, geriatrics, oncology, neonatology, among others, as well as the absence of studies in specific health devices such as basic health units, clinics, beds - day. This lack of work in specific areas of knowledge, as well as the low number of publications related to job satisfaction of nurses as a whole, may be linked in part to the very genesis of the profession, grounded in love and devotion, so aspire to satisfaction of these workers, or better, meet their own needs, can be understood as discordant and even contradict this reality of devotion.22 However, one cannot

forget that psychological, emotional and social needs impacting greatly on motivation and worker’s satisfaction23, Therefore, these

aspects should be analyzed and worked in different work environments.

When comparing the thematic trends and year of publication, it appears that the most recent publications have focused on specific and concentrated knowledge of nursing as hematology, occupational nursing, teaching, home care study areas, opposing thus the work done in the '80s and '90s, which addressed most of the time the various sectors of the nurse's performance, not characterizing a specific population.

Allied to this fact, it appears, in nursing, the existence of 61 courses qualified graduate, between masters and doctorate, which are directly related to the development of research in nursing.12,24 Despite this

significant number of postgraduate courses, the number of publications related to job satisfaction of nurses is still small forward to the number of professionals and does not reflect all the fields of activity of this person, and neither the real needs of these individuals and what is needed to satisfy them.

Although the study does not address all domestic production of the subject, due to contemplate journal articles, allowed to outline an overview of the national scientific approach and highlight gaps and trends. In our search we found that across the multiplicity of factors involved in the phenomena under study and considering the different needs of each business and the various ways of satisfying them, using various methodological designs was present, being the most widely used quantitative approach. The results reveal that these studies are rising from the year 2003 and tend thematically associated with satisfaction of nurses with the focus more elaborate the various sectors and managers.

There was scarce national scientific production on the topic researched and found most articles present the hospitals in its different areas of activity, as a study. Even taking into account that the hospital environment is permeated by multiple factors that impact the lives of nurses consider of great importance to investigate different scenarios of such as health centers, clinics, highlighting the consequences that such environments have on the professional.

The conclusion is of utmost importance that nurses themselves undertake research and likewise publish scientific papers related to different scenarios of operation, as well as what your needs are that need to be addressed and how the institution contribute to and / or may contribute to that, with

adaptations and rearrangements of the local system. It is observed in the studies analyzed that there are still many gaps in knowledge about satisfaction / motivation of nurses, especially in the category industry and population. It is considered that studies investigating this issue can contribute positively to the qualification, and improves the performance of health institutions.
