equalize

recipient

chances

of

an

acceptable

match

DearEditor,

Thenumberofnamedallelesofthehumanleukocyte anti-gen(HLA)genes,HLA-A,HLA-BandDRB1are3399,4242and 1883,1,2respectively.Owingtoanalysesofthefrequenciesand

distributionsoftheseallelesworldwide,246HLA-Aalleles,367 HLA-Ballelesand226HLA-DRB1allelesarenowcommonor welldocumented.3Asthefirststepinfindingabonemarrow

matchforanewpatient,analgorithmscanstheentire reg-istryeverydayanddisplayspossibledonorsthatmatchthe recipient’sgenotype.Then,newandcomplementaryHLA typ-ingsare requestedforlociDQB1and Cand alleliclevel for ClassII,and ifthe donorisingood health,aconfirmatory high-resolutionHLAtestisperformed.

Whennocompatibledonorcanbefound,thefamilyand friends of the patient engage themselves in campaigns to recruitpossibledonors, hopingthata matchwillbe found among the new volunteers. These recruitments have sen-sitized many to the need for bone marrow donation, and approximately25%oftheentries(n=956,330)intheBrazilian BoneMarrowDonorRegistry(REDOME)wereobtainedfrom 1431campaignstorecruitbonemarrowdonorsin593 Brazil-iancities.In2010,morethan270suchcampaignsaccounted for30.1% ofnew donors. The Health Ministry has limited theannual reimbursementforHLAtypings to300,000 new entrantsinthe REDOMEsince2012 andincreased, propor-tionally, financial resources to improve hematopoietic cell transplantation(HCT).AlthoughREDOMEentriescover97.4% ofBrazilianMunicipalities,theproportionofentriespercity accordingtopopulationvariesgreatly,rangingfrom0.002%to 23.543%(median:0.584%).

Twomainfactorsmustguide recruitment:(1)the proba-bilityoffindingamatchbasedonthefrequencydistribution ofthealleles inthe population, and(2) theHLA allele fre-quencyindiseasesthat couldbenefitfrom HCT.In amore simplified model, an equal chance to find a donor match shouldbebasedontheBraziliandistributionofHLA haplo-types.Ashaplotypeshaveageneticbackground,citieswith well-definedethniccolonizationoralowmigrationindexwill haveasmallernumberofhaplotypeswithlessdiversity.The HLAdiversityamongthe volunteers recruited during18 of

thesecampaignswascomparedwiththediversityinrandom samples(consistingof350,1250,and2250entries)generated from the REDOME.Theresultsshowed thatthe numberof haplotypeswasdirectlyproportional tothe numberofnew donors(Figure1).Thenumberofhaplotypesalsodecreasesas afunctionoflinkeddisequilibrium.Thislossindiversitydue toimbalancedrecruitmentincreasesthecoststotheHealth MinistryintermsofreimbursementsforHLAtypingto labo-ratories.Italsoreducesthechancesoffindingabonemarrow match forsomeindividualswho share haplotypesthatare morecommonintheREDOME.Moreover,additionalcostswill alsobeincurredrelatedtocomplementaryteststodefine pos-sibledonorsamongthesenewentries.

registry,sothatheorshecanrethinkthedecision.This mea-surewillpermitustoascertainwhetherornottheindividual willactivelyparticipateinthesearchforbonemarrow compat-ibility.Anothermeasureistolimitthenumberofnewdonors incampaignsincitieswithlessthan20,000citizensandalow migrationindex.Thesemeasurescouldresultinthe follow-ing:(1)greatfidelityandimprovedavailabilityofdonors,as theircontactinformationwillbecheckedviathephonecall, (2)savingsfortheHealthMinistryandsociety,asthecostsof typingsamplesofpeoplewhodonotgivetheirconsentatthe momentofdonationwilldiminish,and(3)abetter represen-tationofhaplotypesincities,reducingduplicatesrelatedto familybackground.

Specificrecruitmentshouldalsobeimplementedtoreduce differences among self-declared race/color.6 Our

prelimi-naryresultsshowsignificantdifferencesrelatedtorace/color with compatibility in 9 of 10 and 10 of 10 loci in the REDOME.7 The rationaluse ofpublic resources toincrease

HCT since 2012 may be guided by technical information to define recruitment campaigns for the REDOME. New donors are still needed, but their recruitment should not beadjustedaccordingtothewishesofthepatient’s family neitherdefinedbytheJusticesysteminreimbursement cam-paignsforHLAtypingbasedonfamilyplea.Moreover,some citiesandevenstatesare underrepresentedintheREDOME (Table1).

Finally,thesize(numberofentries)oftheREDOMEshould be augmented to provide a reasonable chance of HCT for patients and complement the availability of samples in

Braziliancordandplacentabloodbanks(BRASILCORD)aswell asinternationally.
