Autoimmune thyroid disease as a risk factor for angioedema in patients with chronic idiopathic urticaria: a case-control study

Autoimmune thyroid disease as a risk factor for angioedema in patients with chronic idiopathic urticaria: a case-control study

CONTEXT AND OBJECTIVE: An association between chronic idiopathic urticaria (CIU) and autoimmune thyroid disease (ATD) has been reported. However, there have not been any reports on whether ATD raises the risk of angioedema, which is a more severe clinical presentation of CIU. Thus, the aim of the present study was to evaluate whether the risk of angioedema is increased in patients with CIU and ATD. DESIGN AND SETTING: Case-control study including 115 patients with CIU at a tertiary public institution. METHODS: The patients were evaluated with regard to occurrence of angioedema and presence of ATD, hypothyroidism or hyperthyroidism.

NEW DERMATOLOGICAL TREATMENT FOR CHRONIC URTICARIA

NEW DERMATOLOGICAL TREATMENT FOR CHRONIC URTICARIA

ABSTRACT – Chronic urticaria is a skin disorder with a very high impact on patients’ quality of life. It has a prevalence between 7,8 and 22,3% with a higher incidence in women between 20 and 40 years old. The intensity of the itch, the anxiety caused by its recurrent nature and also the incapacity caused by the unpredictable exacerbations are among the factors that mostly impact patient’s quality of life. Being a mast cell-driven disease, the most recent recommendations of the European Academy of Allergy and Clinical Immunology (EAACI), the EU-funded network of excellence, the Global Allergy and Asthma European Network (GA2LEN), the European Dermatology Forum (EDF), and the World Allergy Or- ganization (WAO) suggest a symptomatic treatment for chronic urticaria, with very limited routine diagnostic measures being recommended. Non-sedating anti-histamines H1 (AHH1ns), in approved dose, are recommended as first line of treatment followed by the utilization of omalizumab, cyclosporine A or montelukast as add-on treatment in patients that do not respond to high doses AHH1ns. The treatment must continue until the patient is symptoms free, in the most possible safe manner.

Autoantibody production in chronic idiopathic urticaria is not associated with Helicobacter pylori infection

Autoantibody production in chronic idiopathic urticaria is not associated with Helicobacter pylori infection

Forty-six sera from CIU patients, 15 males and 31 females ranging in age from 15 to 76 years, were evaluated in this study. All pa- tients had a clinical diagnosis of CIU, pre- senting urticarial skin lesions during physi- cal examination, as well as a conclusive history of repeated episodes of urticaria last- ing for more than 6 weeks, without an appar- ent definitive cause. A sporadic episode of angioedema was reported by some of them. Twenty-three CIU patients seronegative for anti-H. pylori IgG antibodies by DOT- ELISA (Orgenic, Yavne, Israel) represented the noninfected group, while 23 CIU pa- tients seropositive for the bacterium repre- sented the infected group. The control group consisted of 10 healthy adults of similar age without a clinical history of CIU. All of them had negative laboratory results for Chagas’ disease, syphilis, viral hepatitis and chronic diseases such as autoimmune thyroiditis and rheumatoid arthritis.

Diagnostic and Therapeutic Approach of Chronic Spontaneous Urticaria: Recommendations in Portugal

Diagnostic and Therapeutic Approach of Chronic Spontaneous Urticaria: Recommendations in Portugal

Chronic spontaneous urticaria is a complex disorder, of unclear etiology, easily diagnosed although often difficult to treat. It has a significant impact on the patients’ quality of life and results in high direct and indirect costs. The diagnosis of chronic spontaneous urticaria is mainly clinical and a limited number of tests is recommended for differential diagnosis and/or for the investigation/exclusion of possible causes. In addition to the complete blood count and C-reactive protein, and/or erythrocyte sedimentation rate, additional tests must be selected according to clinical criteria. The aim of therapy is the complete clinical control of chronic spontaneous urticaria. Evolution should be documented by weekly symptom scoring – Weekly Urticaria Activity Score (UAS7) –, as well as the assessment of quality of life. The therapeutic approach is based on second-generation H1 antihistamines (anti-H1) administered continuously in the approved doses (first line), and, in the absence of a clinical response, up to four times the daily-approved dose (second line). First generation H1 antihistamines are not recommended. Approximately 30% of patients are not controlled with second line therapy, and it is recommended to add a third line therapy. Of the two options, omalizumab and cyclosporine, only omalizumab is approved for chronic spontaneous urticaria and has a better safety profile, thus being preferably recommended. In Portugal there are no national-based recommendations applicable to clinical practice. The elaboration of these recommendations is justified by the need to standardize both the diagnosis and the treatment approach of patients with chronic spontaneous urticaria in Portugal, and for the referral of patients to specialized centers, in the most severe cases.

Assessment Of Some Acceleration Schemes In The Solution Of Systems Of Linear Equations.

Assessment Of Some Acceleration Schemes In The Solution Of Systems Of Linear Equations.

Chebyshev acceleration method [10] has been one of the favorite Krylov space methods for solving large sparse linear systems of equations in a parallel environment, since, unlike methods based on orthogonalization (such as Conjugate Gradient) it does not require computing computation-intensive inner products for the determination of the recurrence coefficients. The Chebyshev method, which in earlier literature has often been referred to as Chebyshev semi-iterative method, requires some preliminary knowledge about the spectrum of the coefficient matrix A, The concept of spectral radius allows us to make a complete description of eigenvalues of a matrix and is independent of any particular matrix norm [12]. Chebyshev acceleration method can be applied to any stationary iterative method provided it is symmetrizable. It requires the iteration matrix to have real spectrum. Given a nonsingular matrix Q, we define a basic

Evaluation of susceptibility of the ZRE1 alloy to hot cracking in conditions of forced strain

Evaluation of susceptibility of the ZRE1 alloy to hot cracking in conditions of forced strain

Typical temperatures for the process of crystallization and melting of the ZRE1 alloy, i.e. temperature of the beginning and the end of crystallization, and melting point for intermetallic phase and the Mg (α) solid solution were determined by differential thermal analysis (DTA). The tests were performed on the SETSYS thermal analyser manufactured by Setaram, using the TG-DTA head. The systems allows measurements of heat flow during phase changes related to melting and crystallization of tested alloys. The measurement of the temperature of the beginning and the end of the change were made with the method of two tangents’ intersection (“one set point”). The conditions of the experiment are given in table 2, and the DTA curves for the ZRE1 alloy during heating and cooling are shown in figure 2.

MODULAÇÃO IMUNOLÓGICA in vitro DE CÉLULAS DE LANGERHANS E MACRÓFAGOS POR DROGAS UTILIZADAS NO MANEJO DE REAÇÕES HANSÊNICAS

MODULAÇÃO IMUNOLÓGICA in vitro DE CÉLULAS DE LANGERHANS E MACRÓFAGOS POR DROGAS UTILIZADAS NO MANEJO DE REAÇÕES HANSÊNICAS

Chronic Urticaria in the Real-Life Clinical Practice Setting in Portugal: Baseline Results from the Non-Interventional Multicentre AWARE Study

Chronic Urticaria in the Real-Life Clinical Practice Setting in Portugal: Baseline Results from the Non-Interventional Multicentre AWARE Study

One important aspect of CU is the use of medical re- sources by patients, which not only has personal and social implications, but also impacts on direct and indirect costs. A patient suffering from a poorly controlled CU will use more medical resources and probably have higher absentee- ism from school or work. Before enrollment in the AWARE study, 51.0% of patients had gone to the ER at least once, of which 57.9% had used this resource three or more times. Being female was a predictor of going to the ER, which is consistent with women reporting higher DLQI scores than men. Supporting our results, women have been shown to have a greater burden of illness than men 15 and to visit the

A Strategic Analysis of Search Engine Advertising in Web based-commerce

A Strategic Analysis of Search Engine Advertising in Web based-commerce

Major players in this role are Google and Yahoo. This model is also based on the pay- per click strategy discussed above. The Ad-publishers are supposed to give permission to Search Engine to place advertisements in their website and for every click a user makes on the advertisement the website owner gets paid by the Search Engine. People with creative ideas can get rich relatively quickly by permitting advertisers to piggyback on any web site that attracts a lot of viewers. It has been analyzed that Google pays Adsense publishers 78.5 cents on the dollar as revenue share. Google never officially disclosed details on revenue sharing but it has been observed that, as per rule Google shares 50 cents of every dollar to partners and can range up to the full dollar, depending on the size of the relationship. The study on revenue generated by Website owner’s shows that most of the revenue is generated by Search Engine Advertising Industry e.g. Google Adsense program. The revenue generated by some websites using Adsense has been summarized in Table [2]. Many Indian companies are surviving on Adsense business and many Indian are earning month’s salary just through such pay per click programs. Many companies like ACT Media, Web Techies are specialized for successful Adsense campaign integration to the client’s website. The revenue generated by the program highly depends on the traffic of the website. Amit Aggarwaal (2006), owner of Digital Inspiration has revealed that his blog gets around 1.25 million hits per month with a majority originating from Google followed by direct traffic (like bookmarks, rss feeds, etc). The major traffic comes from four countries - US, India, UK and Australia (in the same order). The maximum site traffic comes from four countries - US, India, UK and Australia (in the same order) and the majority of revenue earned is from Adsense. Priya Shah, an Editor, Editor Http://EbizWhizPublishing.com in her article states that “Google's TOA prevents me from making any disclosures here, but I can safely say that I now make in a day what I used to earn in a month(over a year ago)”.

A Review Of Fault Tolerant Scheduling In Multicore Systems

A Review Of Fault Tolerant Scheduling In Multicore Systems

assignments task on the cores and tolerate processing component failures which could either be rogeneous or heterogeneous. The heterogeneity of the processors implies that they have diverse velocities or transforming abilities. The transient flaw likelihood that may happen in transistors, entryways and even a bit, is called Architectural helplessness variable (AVF) [5]. By averaging over some time, this component can characterize the rate of soft errors that can show up on a core; while it runs a task. This algorithm when compared with other techniques we found that the proposed fault tolerant method outflanks both TMR and DMR methods about 35% in average. In computing most common topologies in N-Modulo Redundancy are TMR and DMR. TMR method is used when reliability of task is important as it can only mask the faults. The problem is that voter can also be faulty. The other method DMR includes two cores running parallel and checks for the similarity in the output thus only indicate the mismatch doesn’t tolerate the fault so have to use separate mechanism for fault recovery [5]. In software based fault tolerance based approach replica of process are used so it is necessary that replicas use same memory addresses. We also need to ensure that leader and follower use same replica copies. Replica can be created using fork system call in which process generates same process as follower process and it works same as leader process. If the result of both leader and follower process is same then we can say that there is no error. But if there is difference between leader and follower process then error is there. We can use check points for efficiency. If after execution of leader and follower process result is same then previous checkpoint is eliminated and if result

Fabrication of in situ composite layer on cast steel

Fabrication of in situ composite layer on cast steel

The task was executed using Aldrich 99,8% titanium powder of 325 mesh and spectrally pure graphite powder of 325 mesh. The powders were next placed in an air-tight container to prevent titanium oxidation and the whole was mixed for 24h. After the lapse of this time, a colloidal slurry of the powders and alcohol was prepared in a 1:2 ratio, and the ready slurry was applied by washing on the internal surface of a ceramic mould cavity (Fig. 1). Thus prepared mould with a layer of reagents was dried at a temperature of 573K for the time of 10min, to be placed next in the chamber of a Balzers vacuum-type furnace. A charge of the composition corresponding to the composition of cast steel was placed in the furnace crucible and melting was conducted in the atmosphere of protective gas (argon). When the melt reached the temperature of 1823K, it was poured into a mould placed in the furnace chamber.

Analysis of Carbon Diffusion during Bainite Transformation in ADI

Analysis of Carbon Diffusion during Bainite Transformation in ADI

Suppose that the subunit denoted 1 of bainitic ferrite forms without diffusion, but any excess carbon is soon rejected into the residual austenite. Consequently, all the subunits denoted 1 were formed at the early stage of transformation from austenite whose carbon concentration is initially identical to that of bulk alloy (region of upper bainite). The subunits denoted 2 and 3 were formed from enriched austenite as a consequence of carbon redistribution occurring after the growth event (region of lower bainite). The transition between these two regions is not sharply defined. There is then the possibility of the reaction beginning with the growth of upper bainite but decomposing to lower bainite from the enriched austenite at the later stages of reaction. This explains why both upper and lower bainite sometimes can be found in the same temperature.

Several studies attempted to increase organ- ism’s resistance by introducing immunomodula- tors into the periapical area of teeth with chronic apical periodontitis [13-15]. The most frequently used drug in the consistency of paste is Levami- sole (Decaris), and also a combination of Thy- mogenum, Levamisole and Hydrocortisone [6]. The use of plant remedies, such as a 10-40% solu- tion of Erakond, introduced into the periapical tissues and left in the root canal for 24 hours, was proved to be effective [16, 17]. However, it would be premature to consider the problem solved.

Roughness of surface of vacuum castings prepared in plaster moulds

Results of measurements are presented in table 2 and 3. Examples of profilograms from the surface of bronze CuSn10 casting made in Gold Star XL plaster mould are presented in Figure 6. Analysis of the test results did not revealed a clear dependences between surface roughness of castings and methods of the preparation. Roughness values are random, independent on casting temperature and temperature and material of the molds. The one regularity that could be observed was that lower roughness parameter Ra showed test casts made of aluminum alloy AlSi11, next bronze casts CuSn10 and the higher value was observed for CuSn5Zn5Pb5 bronze.
