Neste estudo foi feita pela primeira vez a caracterização genética do HIV-1 em pacientes seguidos no Hospital Geral de Santo António. O subtipo B foi sem dúvida a variante HIV-1 predominante nesta amostra populacional, mas os subtipos não-B já representam uma grande percentagem nas infecções HIV-1 aqui encontradas. A subtipagem foi realizada apenas com base na sequência pol. A região pol do HIV-1 é duas a três vezes menos divergente que a env, uma vez que esta região codifica duas enzimas, a RT e a protease, que se excessivamente mutadas, levariam à ineficiência do vírus (Spira, S. et al, 2003). Assim, no caso das amostras em que se observou uma ambiguidade nos resultado da subtipagem, a sequenciação do gene env é um passo a seguir.
Como resultado da análise das sequências, também se constatou que em infecções não-B parece haver uma maior prevalência de mutações associadas a resistências a PIs e NRTIs. Em relação a mutações localizadas em locais aparentemente não associados a resistência também se verificou uma maior prevalência de algumas delas em infecções não-B que em infecções B.
O resultado desta caracterização genética vai de encontro com os estudos realizados até à data. Os subtipos não-B já têm uma elevada prevalência na Europa (Vandamme, A. M. et al, 2004). Determinadas mutações são mais frequentes em subtipos não-B que em subtipos B, sendo algumas delas características das variantes não-B (Holguin, A. et al, 2002; Perez-Alvarez, L. et al, 2003; Spira, S. et al, 2003; Turner, D. et al, 2004).
A distribuição geográfica dos subtipos do HIV-1 é um processo dinâmico, sendo importante a monitorização deste processo, pois dada a variabilidade genética deste vírus e dados os processos migratórios da raça humana, é natural que hajam alterações nesta distribuição geográfica, bem como surjam misturas de variantes de HIV-1. Já foi demonstrado que as variantes não-B têm diferentes padrões de mutações, relativamente ao subtipo B, mas ainda são necessários mais estudos para definir se estes padrões diferentes têm realmente grande influência na progressão da doença, bem como na resposta terapêutica. Daí a importância da caracterização genética do HIV-1 nas diferentes regiões do mundo, bem como o cruzamento destes resultados genéticos com a resposta terapêutica, principalmente em subtipos não-B.
