Nos últimos anos tem‐se assistido a um aumento do número de casos LVM diagnosticados, essencialmente devido aos acidentes de viação (proporcionado pelo aumento do tráfego a nível mundial e associados também ao aumento do consumo de álcool e o não uso do cinto de segurança), bem como o aumento da violência em determinados países e ao aumento da prática de desportos radicais, com potencial risco de lesão da coluna.
Provavelmente o futuro do tratamento da LVM será a fusão das duas técnicas, anteriormente descritas (remoção da aracnoídite na totalidade, repôr a circulação do LCR, alinhar o canal a 100% e as células estaminais) que irá trazer melhores resultados.
Tive oportunidade de assistir a cirurgias segundo a técnica descrita por Reis, bem como ao acompanhamento desses pacientes e seus resultados finais. Estes têm‐ se demonstrado bastante favoráveis e promissores.
Actualmente só se dá importância à parte óssea, estando as restantes estruturas da coluna perfeitamente negligenciadas. No entanto estas últimas representam um importante alvo terapêutico, visto terem um papel crucial para a recuperação dos doentes.
A esta conclusão tinha chegado Alfred Reginald Allen no princípio do Séc. XX, quando reportou em 1917 que o tratamento deveria incluir a coluna óssea e a medula
(Frazier CH, 1918).
No entanto, foram poucas as alterações que os conceitos iniciais de Alfred Reginald Allen sofreram, contudo, novas abordagens começaram a ser possibilidades, visto que só a parte da estabilização óssea e posterior fisioterapia não chega.
