A revisão sistemática realizada nesta dissertação incluiu 16 estudos completos que conduziram avaliação econômica do TAVI no tratamento de pacientes com EAo grave de alto e intermediário risco cirúrgico. As AE elencadas, em geral, compararam o implante de valva aórtica transcateter com a SAVR. Houve limitação para avaliar os desfechos de TAVI considerando o período insuficiente para avaliação do estudo, apenas um estudo utilizou horizonte temporal por toda a vida e os demais apresentaram horizontes temporais entre 1 e 15 anos.
A maioria dos estudos possuem limitações, considerando o TAVI uma tecnologia para tratar uma doença crônica, e haver ausência do uso de Markov e de avaliação de horizonte de tempo por toda a vida para avaliar os desfechos relacionados à doença de EAo.
A maioria dos estudos apresentam limitações metodológicas de acordo com o Checklist Cheers de avaliação de qualidade.
As análises que apresentaram maior qualidade e utilizaram modelo analítico demonstraram que o implante de válvula aórtica transcateter transfemoral é uma opção custo- efetiva em pacientes com EAo grave com risco cirúrgico intermediário ou alto, além de ser útil para pacientes que não podem ser submetidos a SAVR por qualquer outra causa.
Em países com menor investimento em saúde, essa tecnologia pode apresentar inviabilidade de uso devido ao elevado valor da válvula transcateter (prótese).
Em virtude de os estudos não incluírem a população brasileira, recomenda-se estudo multicêntrico em âmbito nacional para avaliar o custo-efetividade da população com EAo grave de alto, intermediário e baixo risco cirúrgico.
