4.2.6 Pontos fortes e limitações
Nosso estudo tem alguns pontos fortes relevantes. Os dados foram derivados de dois amplos registros multinacionais que utilizaram a mesma metodologia de armazenamento e controle de qualidade das informações. Um processo constante de monitoramento, que incluiu avaliações in situ nos centros de pesquisa, garantiu a qualidade dos dados. Esse procedimento reduziu a possibilidade de subnotificação dos eventos adversos graves. Além disso, o grupo controle foi interno e paralelo, acompanhado como o grupo de estudo das bDMCDs.

registros não compulsórios. Em específico, o risco maior foi a subnotificação, que não pode ser totalmente descartada, apesar de todos os esforços para evitá-la.
Outra limitação foi relativa às escassas informações que tivemos sobre as medicações associadas às bDMCDs. Por exemplo, sobre os corticosteroides e as sDMCDs, sabíamos somente se estavam sendo tomados no momento da inclusão do paciente no registro. Faltaram dados sobre dosagens e aderência ao longo do tempo, elementos que permitiriam investigar sua possível relação com as TI observadas de infecções graves. Essas limitações quanto às informações disponíveis foram consequência de uma escolha consciente. Aderimos à filosofia do BiobadaSER de manter o projeto simples para torná-lo viável. O motivo principal foi relacionado com o objetivo do projeto, que era envolver o maior número possível de investigadores, que, nos nossos países de dimensões continentais, frequentemente podem contar com recursos muito limitados. Em um futuro próximo, sempre seguindo as evoluções do registro BiobadaSER, migraremos para uma nova plataforma, que prevê a inclusão de um maior número de dados que devem ser obrigatoriamente registrados no acompanhamento.

5 CONSIDERAÇÕES FINAIS
Os medicamentos imunobiológicos com alvo definido e relevante no processo fisiopatológico da AR mudaram radicalmente a qualidade de vida e o prognóstico dos pacientes. Os estudos randomizados e controlados definiram o perfil de eficácia dessas drogas e a prática clínica confirmou a capacidade modificadora do curso da doença de todas elas.
Por serem drogas que inibem mecanismos específicos da resposta imune, desde a comercialização, há cerca de 20 anos, existe uma preocupação quanto ao perfil de segurança. Os estudos longitudinais demonstraram que os dados de segurança dos ensaios pivotais são insuficientes para definir o real risco de eventos adversos, assim como a natureza deles, especialmente dos mais raros. Na prática clínica, as condições do paciente, suas comorbidades, a forma de acompanhamento e o tempo de tratamento são totalmente diferentes, tornando necessários estudos de coortes de pacientes expostos aos biológicos e com seguimento longitudinal. Considerando que os eventos adversos mais frequentes e temidos nesses casos são os infecciosos, é importante que cada país tenha um banco de dados que monitore a segurança dos pacientes em suas próprias condições socioeconômicas, de organização do sistema de saúde, ambientais e de endemia de patologias infecciosas.
O presente estudo mostra, pela primeira vez, que, na América do Sul, o perfil de segurança dos biológicos é em grande parte similar ao encontrado nos países da Europa e da América do Norte. A triagem pré-tratamento e o acompanhamento proporcionados a esses pacientes são elementos determinantes dessa segurança, especialmente quando consideramos infecções oportunistas ainda endêmicas na América do Sul, como a tuberculose. Mesmo assim, as infeções graves continuam sendo uma preocupação relevante nesses pacientes, constituindo as mais frequentes entre os eventos adversos graves, especialmente nos primeiros seis meses de tratamento.
A boa notícia que nossos dados trazem é que a incidência de infecções graves teve uma redução significativa entre 2010 e 2016, refletindo um melhor critério de seleção dos pacientes expostos e uma maior expertise no acompanhamento desses pacientes.
Apesar desses dados confortantes, o monitoramento deve continuar atento, até pela comercialização constante de novas moléculas com efeito imunomodulador e imunossupressor, que necessitam de dados de segurança nas condições de uso clínico prático. Somente assim garantiremos que os avanços no tratamento da AR contem com drogas que, além de eficazes, sejam seguras, proporcionando uma real melhor qualidade de
