Veuillez vous assurer de signer la demande. Si vous apposez une marque en guise de signature, (par exemple : « X »), la signature d’un témoin est exigée.
Votre demande doit être accompagnée de tous les documents justificatifs requis. Veuillez soumettre ces documents. Omettre de remplir la demande ou de fournir les documents demandés pourrait retarder le traitement de votre demande.
Lorsqu’il est précisé qu’on exige les documents originaux, il vous faut soumettre les documents originaux avec votre demande. Vous devez conserver, pour vos dossiers, une copie certifiée conforme de tout document original que vous nous envoyez. Certains pays exigent des documents originaux qui ne vous seront pas retournés.
Vous pouvez soumettre l’original ou une photocopie certifiée conforme de tout document lorsque les originaux ne sont pas exigés. Il est préférable d’envoyer des photocopies certifiées de documents plutôt que les originaux. Si vous nous envoyez vos documents originaux, veuillez le faire par courrier
recommandé. Nous vous retournerons tous vos originaux. Nous acceptons les photocopies seulement si elles sont lisibles et certifiées conformes aux documents originaux. Le personnel de tout Centre Service Canada photocopiera vos documents et les certifiera sans frais. Si vous ne pouvez visiter un Centre Service Canada, vous pouvez demander à une des personnes suivantes de certifier vos photocopies : avocat; chef de bande des Premières nations; comptable; directeur d’un établissement financier; employé d’un Centre Service Canada qui agit à titre officiel; employé d’un ministère fédéral ou provincial ou de l’une de ses agences; enseignant; entrepreneur de pompes funèbres; greffier municipal; ingénieur; juge de paix; magistrat; maître de poste; député fédéral ou son personnel; député provincial ou son personnel; ministre du culte; notaire; policier; professionnel de la santé : chiropraticien, dentiste, médecin,
pharmacien, psychologue, infirmier autorisé et infirmier praticien; représentant d’une ambassade, d’un consulat ou d’un haut-commissariat; représentant d’un pays avec lequel le Canada a conclu un accord de réciprocité en matière de sécurité sociale; travailleur social.
La personne qui certifie la photocopie doit la comparer à l’original, indiquer son poste ou son titre officiel, écrire son nom en lettres moulées, indiquer son numéro de téléphone, inscrire la date à laquelle elle certifie le document et le signer.
Elle doit aussi inscrire ce qui suit sur la photocopie : Photocopie conforme au document original, qui n’a pas été modifié d’aucune façon.
Si le document a des renseignements des deux côtés, les deux côtés doivent être photocopiés et
certifiés. Vous ne pouvez pas certifier les photocopies de vos propres documents et vous ne pouvez pas demander à un parent de le faire pour vous.

A falta do cumprimento do compromisso ora assumido ou de qualquer declaração falsa, além de obrigar a devolução de importâncias recebidas indevidamente, quando for o caso, sujeitar-se-á às penalidades previstas no art. 171 e 299 do Código Penal.

GUIDE FOR APPLYING BRAZILIAN BENEFITS
CLARIFICATIONS ACCORDING TO BRAZILIAN LEGISLATION
1 – Documents proving dependency 1.1 – Preferred dependents:
a) Spouse and children - Marriage and birth certificates;
b) Partner – identity document and marriage certificate with an annotation regarding legal separation or divorce, when one of the partners or both have already been married, or death [of the previous spouse], if applicable;
c) Equated child – judicial proof and, by means of a statement by the insured person, in the case of a stepchild, the marriage certificate of the insured person and the dependent's birth certificate.
1.2 – In the absence of preferred dependents, the following individuals may be considered dependents: a) Parents – birth certificate of the insured person and his/her identity documents;
b) Siblings - birth certificate.
2 – For proof of the relationship and economic dependency, as applicable, at least three of the following documents must be submitted, when applicable:
a) birth certificate of the child from the relationship; b) religious marriage certificate;
c) income tax return of the insured person, identifying the interested person as his/her dependent; d) last will and testament;
e) note in the Social Security Card (CP) and/or in the Employment and Social Security Card (CTPS), issued by a competent institution;
f) special statement made before a notary public; g) proof of same residence;
h) proof of residential service fees and existence of a partnership or civil life activities in common; i) power of attorney or reciprocally granted guarantee;
j) joint bank account;
k) enrolment in an association of any nature, stating the interested person as a dependent of the insured person;
l) regular annotation in employee card or booklet;
m) insurance policy identifying the insured person as the party securing the insurance and the interested person as his/her beneficiary;
n) record of treatment in a medical institution identifying the insured person as the responsible person; o) deed of purchase and sale of property by the insured person for the dependent;
p) statement of non-emancipation of dependents under 21 age;
q) any other documents that may lead to determination of the fact to be verified.
3 – Information on the death of an insured person residing in Canada who is a holder of benefits paid by the Brazilian Social Security is required, and must be provided by a Canadian authority to the National Institute of Social Security (INSS) through liaison agencies between the two countries, as this involves legal matters that require cessation of the benefits held. Continued payments to someone other than the holder is not provided for by law and represents losses to the public treasury of the Brazilian Social Security system, which results in irregular collection and renders the recipient liable for repayment of the entire amount received, with interest and indexation. It should also be noted that the collection of amounts unduly paid by the INSS is costly and detracts from the image of the Institution.
4 – If the insured deceased person is survived by dependents with pension rights, those dependents should request benefits through the Liaison Agency in Canada, but will not be able to continue receiving the previous benefit to which only the insured deceased person was entitled, and not the dependents.
Note: Validation of the information recorded by the competent unit in the _____________ field will waive the requirement to send copies of supporting documents.
LIST OF DOCUMENTS THAT MUST BE ATTACHED TO THIS APPLICATION.
1 – OLD-AGE RETIREMENT
To apply for an old-age retirement, the insured person must submit the documents according to his/her category and, in the event that he/she carried out activities in more than one category during all of his/her period of contribution, then he/she must submit the respective documentation for each activity.
l- Insured Employee, Unemployed or Temporary Worker: Documentation:
• Employee Identification Number - NIT (PIS/PASEP);
• Identification document (ID Card and/or Employment and Social Security Card); • Individual Taxpayer Registry (CPF);
• Birth or marriage certificate;
• Employment and Social Security Card or another document that confirms the activities carried out and/or period of contribution;
• Certificate from the Temporary Worker's Union or from a Labour Management Agency.
NOTE: Presentation of the Individual Taxpayer Registry (CPF) is mandatory in order to meet the requirements for Brazilian
Social Security benefits.
a) Brazilian beneficiaries or even non-resident foreigners in Brazil, or those with residency in Brazil, who are presently abroad and do not hold an Individual Taxpayer Registry Card (CPF) may acquire it by submitting the form to a Brazilian Diplomatic Representation or by requesting it directly via the Internet;
b) The application does not generate a CPF card for individuals with residency abroad, and in this case, the proof of CPF application will be made upon presentation of the "Proof of Application and CPF Registration Status", printed from the Brazilian Revenue Service website (http://www.receita.fazenda.gov.br), provided that it is submitted together with the registrant's identification documents;
c) To request a CPF card via the Internet, the electronic address is: http://www.receita.fazenda.gov.br/Aplicacoes/ATCTA/CpfEstrangeiro/fcpf.asp. Select the country and click the button "ficha" (entrance form); choose the option "inscrição" (application); fill out the form and complete the submission. Print the CPF registration voucher for future follow-up and submit it to Social Security within 60 days after requesting the benefit, under penalty of cessation of benefits.
ll- Individual/Optional Taxpayer: Documentation:
• Employee Identification Number - NIT(PIS/PASEP) or the individual/optional taxpayer registration number;
• Identification document (ID Card and/or Employment and Social Security Card); • Individual Taxpayer Registry (CPF);
• All receipts presented to Social Security (Guides or Booklets of contribution payments, Individual Taxpayer's Proof of Payment (GRCI), Social Security Payment Guide (GPS));
• Original and copy of Birth or Marriage Certificate. lll – Domestic Employee:
Documentation:
• Employee Identification Number - NIT(PIS/PASEP) or the individual taxpayer / domestic employee registration number;
• Identification document (ID Card and/or Employment and Social Security Card); • Individual Taxpayer Registry (CPF);
• Birth or marriage certificate;
• All receipts presented to Social Security (Guides or Booklets of contribution payments, Individual Taxpayer's Proof of Payment [GRCI], Social Security Payment Guide [GPS]);
• Employment and Social Security Card. 2 – RETIREMENT PENSION DUE TO DISABILITY
Insured Employee - To apply for retirement pension due to disability, the insured person must submit the documents according to his/her category and, in the event that he/she carried out activities in more than one category during all of his/her period of contribution, then he/she must submit the respective documentation for each activity, as follows:
• Employee Identification Number - NIT(PIS/PASEP);
• Medical Certificate, Laboratory Exams, Certificate of Hospitalization, Outpatient Treatment Certificates, and others that provide proof of medical care;
• Identification document (ID Card and/or Employment and Social Security Card); • Individual Taxpayer Registry (CPF);
• Birth Certificates of children under 14 years of age. 3 – SURVIVOR'S PENSION
If the insured settlor in the moment of the death was in benefit, the dependent must be submitted, the following documents: • Employee Identification Number - NIT(PIS/PASEP) or the Rural Worker/Optional/Domestic/Individual taxpayer registration number, if in the applicant’s possession;
• Identification document (ID Card and/or Employment and Social Security Card); • Death Certificate;
• Individual Taxpayer Registry (CPF); The dependents must also be submitted the following documents:
• Employee Identification Number - NIT(PIS/PASEP) or the Rural Worker/Optional/Domestic/Individual taxpayer registration number, if in the applicant’s possession;
• Identification Document;
• Individual Taxpayer Registry (CPF);
A partner is considered an individual who maintains a common-law relationship with the insured person, including a same-sex relationship.
A common-law relationship is considered a union between a man and a woman who act as a family unit, whether unmarried, legally separated, divorced or widowed, or have a child together, while in the relationship.
4 - TO PROVE THE COMMON-LAW RELATIONSHIP, AT LEAST THREE COPIES AND THE ORIGINALS OF THE FOLLOWING DOCUMENTS MUST BE SUBMITTED:
• Income tax return of the insured person showing the interested person as his/her dependent; • Last will and testament;
• Permanent note in the Social Security Card (CP) and/or in the Employment and Social Security Card (CTPS), issued by a competent agency;
• Special statement made before a notary public (public declaration of economic dependence); • Regular annotation in employee card or booklet;
• Birth certificate of a child from the relationship; • Religious marriage certificate;
• Proof of residing in the same house;
• Proof of residential service fees and existence of a partnership or civil life activities in common; • Power of attorney or reciprocally granted guarantee;
• Joint bank account;
• Enrollment in an association of any nature identifying the interested individual as a dependent of the insured person; enrolment in any type of association identifying the individual as a dependent of the beneficiary;
• Insurance policy identifying the insured person as the party securing the insurance and the interested person as his/her beneficiary;
• Record of treatment in a medical institution identifying the insured person as the responsible person;
• Deed of purchase and sale of property by the insured person for the dependent; • Any other documents that may lead to determination of the fact to be verified.
NOTE: A partner is a person who maintains a same-sex common-law relationship with the beneficiary.
A common-law relationship is considered to exist when it can be verified that the man and woman act as a family unit, whether unmarried, legally separated, divorced or widowed, or have children in common, through to the dissolution of that relationship.
GUIA PARA SOLICITAÇÃO DE BENEFÍCIOS BRASILEIROS
ESCLARECIMENTOS DE ACORDO COM A LEGISLAÇÃO DO BRASIL
1 – Indicar documentos que provem a dependência 1.1 – Dependentes preferenciais:
a) cônjuge e filhos - Certidões de casamento e de nascimento;
b) Companheira ou companheiro – documento de identidade e certidão de casamento com averbação da separação judicial ou divórcio, quando um dos companheiros ou ambos já tiverem sido casados ou óbito, se for o caso;
c) Equiparado a filho – certidão judicial e, mediante declaração do segurado, em se tratando de enteado, certidão de casamento do segurado e de nascimento do dependente.
1.2 – Inexistindo os preferenciais, consideram-se dependentes
a) pais – certidão de nascimento do segurado e documentos de identidade dos mesmos b) Irmãos - Certidão de nascimento
2 – Para comprovação do vínculo e da dependência econômica, conforme o caso, devem ser apresentados no mínimo três dos seguintes documentos:
a) certidão de nascimento de filho havido em comum; b) certidão de casamento religioso;
c) declaração do imposto de renda do segurado, em que conste o interessado como seu dependente; d) disposição testamentária;
e) anotação constante na Carteira Profissional – CP e; ou na Carteira de Trabalho e Previdência Social -CTPS, feita pelo órgão competente;
f) declaração especial feita perante tabelião; g) prova de mesmo domicílio;
h) prova de encargos domésticos evidentes e existência de sociedade ou comunhão nos atos da vida civil i) procuração ou fiança reciprocamente outorgada;
j) conta bancária conjunta ;
k) registro em associação em qualquer natureza, onde conste o interessado como dependente do segurado
l) anotação constante de ficha ou livro de registro de empregados ;
m) apólice de seguro da qual consta o segurado como instituidor do seguro e a pessoa interessada como sua beneficiária /;
n) ficha de tratamento em instituição de assistência médica da qual conste o segurado como responsável o) escritura de compra e venda de imóvel pelo segurado para o dependente;
p) declaração de não emancipação do dependente menor de 21 anos ;
q) quaisquer outros documentos que possam levar a convicção do fato a comprovar
3 – A informação sobre a ocorrência de óbito de segurado residente no Canadá, que seja titular de benefício pago pelo Regime Geral de Previdência Social do Brasil (RGPS), prestada por Autoridade Canadense ao INSS através dos Organismos de Ligação dos dois Países, é imprescindível por se tratar de motivo legal que obrigue à cessação do benefício mantido. A continuidade do pagamento a outrem, que não seja o titular, não está prevista em lei e, além de significar prejuízo aos cofres públicos da Previdência Social Brasileira, torna este recebimento irregular sujeitando o recebedor a devolver todo o valor recebido, com juros e correção monetária. Também há que se ressaltar que a cobrança dos valores pagos indevidamente por parte do INSS é onerosa e desgastante para a imagem da Instituição
4 – Caso o segurado falecido tenha deixado dependente com direito à pensão, este deverá solicitar o benefício através do Organismo de Ligação no Canadá, mas não poderá continuar recebendo o benefício anterior que era de titularidade do segurado falecido e não do dependente.
Obs. A validação das informações cadastrais pela unidade competente no campo_____________ dispensará o envio das cópias dos documentos comprobatórios.
RELAÇÃO DOS DOCUMENTOS QUE DEVEM SER ANEXADOS JUNTOS A ESTE REQUERIMENTO
1 – APOSENTADORIA POR IDADE
Para requerer a aposentadoria por idade o segurado (a) deverá apresentar documentos de acordo com sua categoria e, caso tenha exercido atividade em mais de uma categoria em todo seu período contributivo, deverá apresentar a documentação respectiva a cada atividade.
l- Segurado(a) Empregado(a), Desempregado (a) ou Trabalhador(a) Avulso(a) Documentação:
• Número de Identificação do Trabalhador - NIT(PIS/PASEP)
• Documento de Identificação (Carteira de Identifidade e / ou Carteira de Trabalho e Previdência Social) • Cadastro de Pessoa Física – CPF
• Certidão de nascimento ou de casamento
• Carteira de Trabalho e Previdência Social ou outro documento que comprove o exercício de atividades e/ou tempo de contribuição
• Certificado do Sindicato dos Trabalhadores Avulsos ou do Órgão Gestor de Mão de Obra
NOTA: A apresentação do CPF é obrigatória para os requerimentos dos benefícios da Previdência Social Brasileira.
a) Os beneficiários brasileiros ou mesmo estrangeiros não residentes no Brasil ou residentes no Brasil que se encontrem no exterior que e não possuírem Cadastro de Pessoas Físicas-CPF poderão adquiri-lo mediante apresentação do formulário em uma Representação Diplomática brasileira ou solicitar diretamente na internet.;
b) A inscrição de pessoas físicas residentes no exterior não gera cartão de CPF, neste caso a comprovação da inscrição no CPF será feita mediante a apresentação do "Comprovante de Inscrição e de situação Cadastral no CPF", impresso na página da Receita Federal do Brasil (http://www.receita.fazenda.gov.br), desde que acompanhada de documentos de identificação do inscrito. ;
c) Para a solicitação do CPF via internet, o endereço eletrônico é:
http://www.receita.fazenda.gov.br/Aplicacoes/ATCTA/CpfEstrangeiro/fcpf.asp. Selecione o país e clique no botão “ficha”; marque a opção “inscrição”; preencha o formulário e conclua a transmissão. Imprima o comprovante do cadastramento do CPF, para acompanhamento futuro e apresente-o à Previdência Social no prazo máximo de até 60 dias após ter requerido o benefício, sob pena de ter o benefício cessado.
ll- Contribuinte Individual/Facultativo: Documentos:
• Número de Identificação do Trabalhador - NIT (PIS/PASEP) ou número de inscrição do contribuinte individual/facultativo;
• Documento de Identificação (Carteira de Identidade e/ou Carteira de Trabalho e Previdência Social); • Cadastro de Pessoa Física– CPF;
• Todos os comprovantes de recolhimento à Previdência Social (Guias ou Carnês de recolhimento de contribuições, Guia de Recolhimento do Contribuinte Individual – GRCI, Guia da Previdência Social – GPS);
• Cópia e original da Certidão de Nascimento ou Casamento. lll – Empregado (a) Doméstico (a):
Documentação:
• Número de Identificação do Trabalhador – NIT(PIS/PASEP) ou numero de inscrição do contribuinte individual /empregado doméstico;
• Documento de Identificação(Carteira de identidade e / ou carteira de Trabalho e Previdência Social); • Cadastro de Pessoa Física– CPF
• Certidão de Nascimento ou Casamento
• Todos os comprovantes de recolhimento à Previdência Social (Guias ou Carnês de recolhimento de contribuições, Guia de Recolhimento do Contribuinte Individual – GRCI, Guia da Previdência Social – GPS);
• Carteira de Trabalho e Previdência Social
2 - APOSENTADORIA POR INVALIDEZ
Segurado(a) Empregado(a)- Para requerer aposentadoria por invalidez o segurado (a) deverá apresentar documentos de acordo com sua categoria e, caso tenha exercido atividade em mais de uma categoria em todo seu período contributivo, deverá apresentar a documentação respectiva a cada atividade , conforme abaixo:
• Número de Identificação do Trabalhador - NIT(PIS/PASEP)
• Atestado Médico, Exames de Laboratório, Atestado de Internação Hospitalar, Atestados de Tratamento Ambulatorial, dentre outros que comprovem o tratamento médico;
• Documento de Identificação(Carteira de identidade e / ou Carteira do Trabalho e Previdência Social) • Cadastro de Pessoa Física – CPF
• Certidão de Nascimento dos filhos menores de 14 anos 3- PENSÃO POR MORTE
Caso o segurado instituidor foi aposentado ou no momento da morte estava em outro benefício, apresentar os seguintes documentos:
• Número de identificação do trabalhador – NIT (PIS/PASEP) ou número de inscrição do Contribuinte Individual/Doméstico/Facultativo/Trabalhador Rural, se possuir;
• Documento de identificação (Carteira de Identidade e/ou Carteira de Trabalho e Previdência Social); • Comprovante com o número do benefício (cartão magnético, recibo bancário);
• Certificado de óbito;
• Cadastro de Pessoa Física– CPF
Os dependentes também devem apresentar os seguintes documentos:
• Número de identificação do trabalhador – NIT (PIS/PASEP) ou número de inscrição do Contribuinte Individual/Doméstico/Facultativo/Trabalhador Rural, se possuir;
• Documento de Identificação; • Cadastro de Pessoa Física– CPF
Considera-se companheira ou companheiro a pessoa que mantenha união estável com o(a) segurado(a), mesmo que homossexual.
Considera-se união estável aquela verificada entre o homem e a mulher como unidade familiar, seja solteiros, separados judicialmente, divorciados ou viúvos, ou tenham prole em comum, enquanto não se separarem.
4 - PARA COMPROVAR A UNIÃO ESTÁVEL, DEVEM SER APRESENTADOS CÓPIA E ORIGINAL, DE NO MÍNIMO TRÊS DOS SEGUINTES DOCUMENTOS CONFORME O CASO:
• Declaração de Imposto de Renda do segurado, em que consta o interessado como seu dependente; • Disposições Testamentárias;
• Anotação constante na Carteira Profissional - CP e/ou na Carteira de Trabalho e Previdência Social - CTPS, feita pelo órgão competente;
• Declaração especial feita perante tabelião (escritura pública declaratória de dependência econômica); • Anotação constante de ficha ou Livro de Registro de empregados;
• Certidão de nascimento de filho havido em comum; • Certidão de Casamento Religioso;
• Prova de que reside na mesma casa;
• Prova de encargos domésticos evidentes e existência de sociedade ou comunhão nos atos da vida civil; • Procuração ou fiança reciprocamente outorgada;
• Conta bancária conjunta;
• Registro em associação de qualquer natureza onde conste o interessado como dependente do segurado; Registro em qualquer tipo de associação indicando a pessoa como dependente do beneficiário;
Apólice de seguro da qual conste o segurado como instituidor do seguro e a pessoa interessada como sua beneficiária; Ficha de tratamento em instituição de assistência médica da qual conste o segurado como responsável;
Escritura de compra e venda de imóvel pelo segurado em nome do dependente; Quaisquer outros documentos que possam levar à convicção do fato a comprovar.
É considerado união estável quando há possibilidade de verificar se o homem ou mulher desempenham como uma unidade familiar, quando são solteiros, legalmente separados, divorciado ou viúva, ou tem filhos juntos, até essa união se dissolver.

Renseignement à soumettre:
• Votre numéro d’employé brésilien – (Número de Identificação do Trabalhador-NIT)
Documents originaux ou copies certifiées à produire :
• Certificat de naissance (demandeur, enfants âgés de moins que 14 ans) • Certificat de mariage (le cas échéant)
• Document d’identification – Carte d’identification brésilienne : Registro Geral (RG) ou Carte d’emploi et sécurité sociale : Carteira de Trabalho e Previdência Social (CTPS) • Registre de contribuable - Cadastro de Pessoa Física (CPF) ou preuve de demande et
de statut d’inscription au CPF
• Documents à l’appui des périodes d’assurance brésiliennes et des types d’activités exercées, par exemple, Carte d’emploi et sécurité sociale : Carteira de Trabalho e Previdência Social (CTPS) ou autres documents
• Certificat de travailleur temporaire ou d’agence d’emploi (le cas échéant) • Documentation médicale, tel que : certificats médicaux, résultats des tests de
laboratoire, dossiers d’hospitalisation, dossiers de traitement ambulatoire, etc.
Le cas échéant, photocopie des documents suivants à produire :
• Documents médicaux antérieurement soumis à l’appui d’une demande de prestation d’invalidité du Régime de pensions du Canada
IMPORTANT: Si vous avez déjà soumis n’importe lequel de ces documents
nécessaires à l’appui de votre demande de prestation du Régime de pensions du Canada ou de la Sécurité de la vieillesse, vous n’avez pas à les soumettre de nouveau.
