Oneinfivephysicianscurrentlypracticingmedicinein theUnitedStatesreceivedtheirinitialmedicaltrainingina foreigncountry.1 Nearly25%ofphysiciansonUSpostgradu-ate training programs in 2002-2003 were foreign medical school graduates (FMSGs).2 Each year, many residency positionsinpediatrics,anesthesiology,psychiatryandfam-ilymedicineintheUnitedStatesremainvacantorarefilled byforeign-born/foreign-trainedphysicians.3Whilepossibly reflecting an overabundance of such residency positions, thissituationmayalsosuggestalackofUSmedicalstudent interestinthesespecialties.
While there seems to be no general consensus about physiciansupply/demandratiointheUnitedStates,andthe impactofforeign-trainedphysiciansonhealthcaredelivery inU.S.hospitalsisstillpoorlydefined,thesteadilyincreasing participationofFMSGsintheAmericanhealthcaresystem has recently become a politically charged and sensitive is-sue.4First,ithasbeenreportedthatalmostallstatesinthe UnitedStatesrequirethatFMSGspassadditionallicensure examination(s) and complete more years of postgraduate medical training than their US-born/US-trained counter-parts. Second, growing evidence of discrimination against FMSGsinresidentrecruitment,licensingrequirementsand unfairlybiasedperceptionsofdisciplinestrength/weakness basedonthenumberofenrolledFMSGsintheresidency programshasbeencollectedfromseveralspecialties,includ-ingpsychiatryandanesthesiology.5-7
Balon et al.5 sought to determine whether there is a selectionbiasagainsttheapplicantswhoareFMSGs,for US residency training positions in psychiatry. Identical requestsforaprogramapplicationformweresentto193 residencytrainingprogramsbytworesidentapplicants,one FMSGandonegraduateofaUSmedicalschool(USMSG), andtherateandcharacteroftheresponseswereanalyzed. Theresponseratetorequestsforanapplicationformwas significantlyhigherfortheUSMSG(159responses)than theFMSG(87responses).Thequalityofresponseswasalso differentinseveralcases.Examplesofqualitativelydifferent responsestotheFMSGsincludedstatementssuchas“We donottakeinternationalmedicalgraduates”;requestsfor furtherinformationorcredentialstobereviewed,follow-ingwhichanapplicationformwouldbesentifthereview weresatisfactory;requirementofanunrestrictedlicensein
anotherstate;alistofvariousrequirements,suchasknowl-edgeofAmericanculture;requestsforacurriculumvitae; andrequirementofaUSmedicallicenseexaminationscore ofaminimumoftheupper80s.Examplesofqualitatively differentresponsestotheUSMSGsincludedaletterfrom a program director asking the candidate to call with any questions;handwrittennotesfromaprogramdirector;vari-ousmaterialsaboutaprogramandthecity;ahandwritten notefromaseniororchiefresident;afollow-upletterfrom achairmanaskingtheUSMSGtosubmittheapplication; asecondapplicationform;andavideotapeofaprogram. The authors concluded that some residency programs in psychiatry were attempting to limit the influx of FMSG applicantsattheveryfirst(entry)level:therequestfora residency application form.The reasons for this practice werenotknown,butdiscriminationbasedontheapplicant’s nameandthemedicalschoolfromwhichshe/hegraduated couldnotberuledoutasapossibleexplanation.5
InanarticleentitledAre“international”medicalgradu-ates second-class anesthesiologists?7 a French-born/trained anesthesiologistwrote:“Idonotthinkthattheeducation IreceivedinaFrenchmedicalschoolisinanywayinferior to the one that students get in this country (USA).” An FMSGhimself,theauthorofthepresentletter,whosepur-suitofhisAmerican(medical)dreamhastakenhimfrom his native Poland to American medical “heaven”, cannot agreemorewiththisstatement/conclusion.Havingfaced manyquestionsabouttherole/situation/positionofforeign-born/foreign-trainedphysiciansinAmericanmedicinefrom manyyoungFMSGs(orFMSGstobe)eagertotastethe “American(medical)pie”,whilelecturingatinternational anesthesiology meetings around the world, the author of this letter wishes to leave the reader with the following twocomments.First,onapersonalnote,thefirsttunethis author heard upon his arrival at the JF Kennedy Airport
inNewYork,USAwastitledBornintheUSA(byBruce
Springsteen), hence part of the title of this communica-tion(Not) “Born in the USA” (and proud of it). Second, onaprofessionalnote,itappearstomethatthetimehas cometoputtoresttheissueofquality/equalityofmedical education(e.g.internationalmedicalschoolgraduatesare notasgoodphysiciansastheirAmericancounterparts7)in this“meltingpot”countryofours(USA)whosesuccessin
