[Abstract

Introduction: The in luenza A ( N was responsible for the pandemic, especially with severe pulmo-nary complications. Objective: To describe characteristics of patients in a university hospital in Curitiba - PR with laboratory diagnosis of in luenza A ( N and its post hospital discharge in the lung function pandemic Methodology: A retrospective observational study. )t was used as a data source the institution Epidemiology Service SEP)( and spirometry tests of patients who were admitted in , years without lung disease associated and non-pregnant. Descriptive statistics were used and applied Fisher's exact test for relationship between comorbidity and spirometry tests. Results: There were con irmed cases, of these were eligible for the study with a mean age of . years ± . and . % males. . % of the patients had comorbidi-ties associated with systemic arterial hypertension . % , diabetes . % and late postoperative period of kidney transplantation . % were the most frequent. Most patients . % had BM) ≥ kg / m². The Spirometry test was performed approximately . ± . days after discharge, of these, had restrictive pattern and all had abnormal chest radiograph results. There was no statistically signi icant difference between the results of Spirometry and comorbidities p= . . Conclusions: The group evaluated in this research did not show a direct relationship between Spirometry and comorbidities, but changes in Spirometry in some patients after hospital discharge stood out, suggesting changes in lung function due to in luenza A ( N .

Resumo

Introdução: O vírus in luenza A (H1N1) foi responsável pela pandemia de 2009, com graves complicações principalmente pulmonares. Objetivo: descrever características dos pacientes de um hospital universitário da cidade de Curitiba – PR com diagnóstico laboratorial de in luenza A (H1N1) e a sua função pulmonar pós alta hospitalar na pandemia de 2009. Metodologia: estudo retrospectivo observacional. Utilizou-se como fonte de dados o Serviço de Epidemiologia da instituição (SEPIH) e exames de espirometria de pacientes que estiveram internados em 2009, maiores de 18 anos, sem doença pulmonar associada e não gestantes. Foi utilizado es-tatística descritiva e aplicado o teste exato de Fisher para relação entre comorbidades e exames de espirome-tria. Resultados: 84 casos con irmados, destes 11 foram elegíveis para o estudo com média de idade de 44,27 anos (±9,63), sendo 63,63% do gênero masculino. Dos 11 pacientes 54,54% possuíam comorbidades associadas sendo a hipertensão arterial sistêmica (54,54%), diabetes (18,18%) e pós operatório tardio de transplante renal (18,18%) as mais frequentes. A maioria dos pacientes (81,81%) apresentaram o IMC ≥25kg/m². O exame de espirometria foi realizado cerca de 40,09 (±15,27) dias após a alta, destes, 5 apresentaram padrão restritivo e todos apresentaram alterações na radiogra ia do tórax. Não houve diferença estatisticamente signi icativa entre os resultados apresentados pela espirometria e a relação com comorbidades (p=0,24). Conclusão: o grupo avaliado por esta pesquisa não apresentou relações diretas entre a espirometria e comorbidades asso-ciadas, porém destacou-se alterações na espirometria em alguns dos pacientes pós alta hospitalar, sugerindo alterações da função pulmonar em decorrência da in luenza A (H1N1).

Introduction

)n the end of the 's there was an outbreak of infection by the )n luenza A virus ( N in a region of the United States of America USA especially at-tacking young people with a signi icant morbidity , bringing about a subsequent mass vaccination. A few decades later this virus re-appeared in an overpower-ing manner in mid . The irst cases were detected precisely on April th in the city of Veracruz

in Mexico, and it went on to becoming a level pan-demic in the beginning of June of the same year, thus de ining the gravity of the pathology caused by the virus and its geographical extension , .

According to the World (ealth Organization W(O , from April to December , countries were noti ied, in total , deaths from )n luenza A ( N con irmed by laboratory exams. Brazil registered a total of , deaths. The southern region presented a higher rate of mortality, being

/ thousand inhabitants deaths , . The symptoms of )n luenza A ( N , similar to the common cold, were stronger and harder to control. Many of the affected individuals evolved to death in a matter of hours, even with the best support of an )ntensive Care Unit )CU . )n the more serious cases, survivors pre-sented great bilateral pulmonary damage in or more

quadrants and signs of aveolar in iltrates with a sub-stantial systemic in lammatory response, evidenced by radiography. These alterations had a great impact on oxy-genation and induced severe pulmonary lesion , , , . The research of Marchiori et al. and Nin et al.

showed pulmonary histopathology in patients contaminated with ( N after death. )n general they presented diffuse alveolar exudative damage, varying degrees of hemorrhage and alveolar edema, necrosis and sloughing of the epithelium of the bronchioles, atelectasis of the alveoli and records of thrombus.

807 A recent study showed that the pandemic of

in luenza A ( N presented different character-istics from the seasonal in luenza . Seeking to better comprehend the process of this pathology, the aim of this study was to describe the characteris-tics of patients in a university hospital in the city of Curitiba – PR with the laboratory diagnosis of in lu-enza A ( N and their lung capacity after hospital discharge in the pandemic.

Methods

This retrospective observational study was con-ducted in the (ospital de Clinicas of Univesidade Federal do Paraná, with the approval of the Ethics in Research Committee (C-UFPR CAAE: . . . . As a database, the Epidemiology Service of the same institution was used SEP)( . Data was collected only from hospital records referent to the period of April to December obtaining a total of con irmed cases of in-luenza A ( N . This information was gathered by only one researcher, protecting all the ethical prin-ciples set by the / resolution. The data collect-ed was identi ication information, anthropometric measurements, associated lung diseases, comorbidi-ties, smoking history, length of hospital stay, use of invasive or noninvasive mechanical ventilation, extra supply of oxygen, x-ray pattern according to the

noti-ication form of the Noti iable Diseases )nformation System S)NAN , use of Oseltamivir, and results of Spirometry exams for volume of forced exhaled air in the irst second FEV , forced vital capacity FVC ; ratio of forced exhaled volume in the irst second and forced vital capacity FEV /FVC ; total lung capacity TLC and functional residual capacity FRC . The pa-tients included in the study were hospitalized in the (C – UFPR hospital during the referred period with a laboratory diagnosis of in luenza A ( N , who did not die during hospitalization, with age above years, without associated lung disease, non-pregnant and who underwent the Spirometry exam in . All the data was compiled in a Microsoft Excel database and descriptive statistics were applied with the use of mean and standard deviation. For the sta-tistical analysis BioSat . software was used and Fisher's Exact Test was applied for comorbidities and Spirometry exams. The statistical signi icance level adopted was p < . .

Results

)n the )n luenza A ( N pandemic, cases were con irmed in the (C-UFPR. Among these . % were male and . % were female. After applying the established inclusion criteria, of the initial patients were excluded from the research; due to death patients , pregnancy patients , being under years old patients , presenting associated lung disease patients or not undergoing the Spirometry exam patients , leaving a total of apt patients.

Out of the patients who underwent the lung function exam, the mean age was . years ± . , being . % of male gender and . % female. The comorbidities found were hypertension . % , diabetes . % , kidney transplantation . % , acute myocardial infarction . % , dyslipidemia . % , systemic lupus erythematosus . % , hy-pothyroidism . % , depression . % and alco-holism . % . The remaining individuals . % did not present comorbidities Table . The body mass index BM) was classi ied separately, according to the World (ealth Organization , the patients were classi ied as ideal weight . % , overweight . % , grade ) obesity . % , grade )) obesity . % and morbid obesity . % .

Regarding smoking, . % were smokers with an average smoking history of . years/pack ± . . The average time of hospitalization was . days ± . . As for the location, patients were in the )ntensive Care Unit for approximately . days ± . , one being in invasive mechanical ventilation for days and two in non-invasive mechanical ventilation for on average , days, the other patients demanded extra oxygen supply of low or high low and were hospitalized in the in irmaries and in the Semi-)ntensive Care unit . The x-ray pattern according to S)NAN was classi ied presenting interstitial in iltrate . % ; consolidation . % and the re-maining patients . % a mixed pattern Table . All used the medication Oseltamivir during hospitalization.

Discussion

The )n luenza A ( N pandemic impelled the worldwide population, there was a broad description of signs and symptoms of a pathology unknown at the time, and its management became important for subsequent actions . All around the world the con irmed cases and registers of )n luenza A ( N permitted detailed analyses of the evolution of this pandemic, seeking its prevention and minimizing the risk .

The con irmed cases in (C-UFPR during the pe-riod of April to December are similar to some other indings in specialized literature, however they do not characterize a pattern. Of the patients in-cluded in this study all were young adults and most of the male gender.

Quispe-Laime et al. presents a study with subjects months following hospital discharge for )n luenza A ( N , the age group was similar, ± years old, and most were also males % . )n other publications the average age was of years with a variation between and years, being that most, . %, were females and another in which the average age was . years, being . % females , both studies regarding data from the second semester of in Brazilian cities.

)n and international study with patients, . % presented comorbidities, being the most

frequent lung diseases, diabetes and dementia . The present study found that . % of the patients had associated comorbidities, the most present be-ing systemic arterial hypertension and diabetes. Zarogoulidis et al. in his study with patients did not ind a signi icant number of associated co-morbidities, and when present, the most related were lung diseases, cardiac coronary disease and diabetes. Lenzi et al. showed that out of , patients who used Oseltamir on average . days after the be-ginning of the symptoms, % evolved to death. Based on this data, it was calculated that each pass-ing day to the beginnpass-ing of the treatment, raises by

% the chance of death. Furthermore, patients who had associated comorbidities had twice the chance of death than healthy patients without comorbidi-ties . And each additional comorbidity raised by % the chance of death. )n this study the most common was diabetes.

809 For the identi ication of breathing disorders, presence of the restrictive pattern was veri ied in out of the evaluated patients, the exam was done around ± . days after hospital discharge. Similar to our indings, Toufen et al. in his study followed-up patients with Severe Acute Respiratory Syndrome SARS at the time of hospitalization for )n luenza A ( N who required invasive mechani-cal ventilation, two months after discharge a lung function exam was completed demonstrating a re-strictive pattern in patients and after months the condition remained for patients. Zarogoulidis et al. also discovered abnormal results in lung function immediately after discharge, he veri ied that FVC and FEV were reduced, after months there was improvement which remained similar months after discharge. Liu et al. evaluated patients year after hospital discharge. Finding who had normal lung function despite later relating respira-tory infections, however not serious. )n addition, showed abnormal results in later lung function exams especially in diffusion.

Quispe-Laime et al. and Edgeworth et al. evaluated patients who had remained in )CU and un-der mechanical ventilation, months after hospital discharge neither presented abnormal results in Spirometry exams. Luyt et al. evaluated pa-tients with SARS and who remained under mechani-cal ventilation. )n the study, the patients were divided into groups, the irst using extra corporeal mem-brane and the other not. )n the evaluation after year neither of the groups presented abnormal results in Spirometry exams, despite not holding statistical signi icance, when compared, the group which used extra corporeal membrane presented lower numbers than the other group.

Conclusion

The results suggest that the )n luenza A ( N pandemic of affected especially young adult pa-tients, having or not associated comorbidities, most of them were overweight and all presented abnormal re-sults in radiographic exams. The patients in this study remained hospitalized in different in irmaries and all demanded extra oxygen supply, being only one in invasive mechanical ventilation. The group evaluated through this research did not present direct relations between Spirometry and associated comorbidities, had a BM) of > kg/m². On the other hand, Nicolini

et al. found only overweight patients out of the evaluated.

Research shows that among the special groups of population with a risk potential to evolve with serious forms of the disease, are patients with chronic dis-eases, the morbidly obese and pregnant women . Obesity can in luence the measure of lung volumes because it interferes in the diaphragmatic movement such as the chest wall, leading to the reduction of the maximum voluntary ventilation , , . The pre-disposition to infections which obese individuals pos-ses has been described in research, however regarding the respiratory system, precise data has not yet been disclosed, making more research necessary .

The habit of smoking was present in . % of the sample, different from the indings of the research of Luyt et al. , which analyzed patients, . % of which were smokers. Lenzi et al. related that only . % of the hospitalized patients smoked. These same authors af irmed that smoking was one of the risk fac-tors leading to the hospitalization of the patients along with obesity and other comorbidities. Analyzing the subjects' physical characteristics and comorbidities data, the presented table demonstrates that, in their majority, they it into a group which is susceptible to hospitalization, on account of most being overweight and presenting some type of comorbidity.

Permanence in hospital raises the risk of compli-cations, studies like those of Nicolini et al. and Luyt et al. present the pro ile of patients who remained in )CU under mechanical ventilation. Our patients remained on average . ± . days hospitalized in different units, only of which in )CU, however all needed extra oxygen supply.
