Anuário de Investigação do IPO de Lisboa referente ao a n o d e 2 0 2 0 r e f l e t e a capacidade de resistência de uma instituição sólida aos momentos do tempo. Em plena pandemia de COVID-19, uma boa desculpa para nada fazer, o IPO fez mais do que no ano p a s s a d o . N o m e i o d e t a n t a s dificuldades, a produção científica de 2020 compara favoravelmente com a dos anos anteriores. Claro que houve menos participações em certames, aqueles encontros a que carinhosamente se chama congressos, deliciosamente descritos no seminal romance de David Lodge, “O Mundo é Pequeno”. Logo, c o n f e s s o , c o m m a i s o u m e n o s virtualidade conectiva, não é a perda de resumos e abstratos que me poderá incomodar. É mais importante deixar l a u d o à m u l t i d i s c i p l i n a r i d a d e apresentada e à cobertura de áreas c o m p l e m e n t a r e s d e c l í n i c a , epidemiologia e laboratório.E m v e z d o s h a b i t u a i s e n c ó m i o s , certamente merecidos, entendo que esta Introdução deve ser uma oportunidade para fazer sugestões e deixar um alerta.

The 2020 Research Yearbook of IPO Lisboa reflects the resilience of a solid institution. Amidst the COVID-19 pandemic, a good excuse not to do anything, IPO Lisboa achieved more than last year. Amongst so many difficulties, the scientific production of 2020 compares favorably with that of previous years. Of course, there were fewer p a r t i c i p a t i o n s i n t h o s e m e e t i n g s affectionately called congresses, as deliciously described in David Lodge's novel, “Small World”. So, I confess with more or less connective virtuality, it is not the loss of abstracts that may bother me. It is more important to report on the multidisciplinarity and the coverage of areas complementary to the clinic, epidemiology, and laboratory.Instead of the usual – most certainly d e s e r v e d – e u l o g i e s , i t i s m y understanding that this Introduction poses an opportunity for some suggestions as well as an alert. I will not cling to complaints and lamentations either.

Também não me ficarei apenas pelos lamentos. Enfim, lá terei de tecer dois.
Tarda a construção de um novo edifício onde poderemos instalar um centro de investigação que aproveite todas as capacidades do IPO e, em defesa do esforçado trabalho de uma equipa notável, já seria tempo de dotarmos o centro existente com mais recursos humanos. Vá lá, ao menos mais uns coordenadores de estudos.
Mas como as coisas se vão fazendo, aqui ficam algumas dicas para que se possa fazer ainda melhor. No nosso IPO, com bancadas curtas e mais doentes do que aqueles que porventura poderíamos agora tratar, a ligação da “bancada à cabeceira da cama” tem funcionado. Não obsta este reconhecimento de boas conexões entre assistência e investi- gação, desde logo espelhadas neste Anuário, para que não se tenha de sugerir um maior investimento e celeridade na captação de investiga- d o r e s p r o f i s s i o n a i s d e e l e v a d a qualidade. Temos perdido gente e há tempo demasiado até que se contrate quem quer vir trabalhar connosco. Sei do que falo.

W e l l , m a y b e j u s t a c o u p l e . T h e construction of a new building able to install a novel Research Centre to take full advantage of the capabilities of IPO Lisboa is tardy. Additionally, in defense of the hard work of a remarkable team, it is probably time to provide the existing Centre with more human resources, at least a couple more Study Coordinators.
But since things get done, here are some recommendations so that we can do even better. At IPO Lisboa, with shorter lab benches and more patients than we could possibly treat, the “bench to bedside”
a p p r o a c h h a s b e e n w o r k i n g . T h e recognition of this good connection between care activities and research, as manifested in this Yearbook, does not prevent the suggestion of a higher investment and celerity in hiring and retaining high quality professional researchers. We have been losing people and it takes too much time to hire whoever wants to come and work with us. I know what I am talking about.

Logo, porque não alocar cada interno de especialidade, logo no início do seu processo formativo, a um programa de investigação clínica? Por que não assumir a necessidade de remune- rarmos os investigadores, incluindo médicos e enfermeiros, pelos doentes recrutados? Sem incentivos, a coisa não mexe. Porque não aceitar que há patologias em que temos de investir mais, incluindo na aceitação de doentes para tratamento, em detrimento de outras que poderão ser assistidas e investigadas em outros lugares? Porque não investir mais em áreas que usem o Registo Oncológico Nacional? Porque não trabalhar mais em conjunto com os outros IPO, os “irmãos” do nosso grupo hospitalar? Porque não investir em áreas ainda pouco cobertas, como as
The quality of the clinic in an institution providing for cancer healthcare is highly dependent on the research activities it conducts. The ongoing research improves processes and outcomes, increases rigour and knowledge to learn and to teach, plus you get the opportunity to justify and make structural changes happen.
Therefore, why not assign every resident to a clinical research programme at the beginning of their residency? Why not assume the need to pay researchers, including medical doctors and nurses, for patient recruitment? Without incentives there is no action. Why not accept that there are pathologies in need of greater investments, including in the number of patients accepted for treatment, in detriment of others that may be supported and investigated elsewhere? Why not invest more in areas making better use of the Registo Oncológico Nacional [the portuguese national cancer registry]?.
Why not work in closer proximity with the other national oncology hospitals (IPO's),

análises de risco e segurança? Outras existirão com certeza. E porque não reivindicar que o IPO seja financiado como centro de referência, em vez de nos comprazermos com uma distinção que n o s t r a z m a i s p r o b l e m a s d o q u e soluções?
O contexto em que o IPO se move, no p a n o r a m a d o s i s t e m a d e s a ú d e Português é potencialmente adverso.
Há forças competitivas a quem o deslace da nossa capacidade de investigação, com a consequente perda de prestígio e capacidade de recrutamento de inves- tigadores, interessa muito. Há a quem sirva sermos piores. Não nos iludamos.
Estamos bem, poderíamos estar melhor e não podemos deixar de fazer por isso. A luta continua.

“brothers” of our hospital group? Why not invest in poorly covered areas like risk and safety analyses? I am sure there are others to consider. And why not ensure IPO to be financed as a Reference Centre instead of wallowing in a distinction that brings more problems than solutions?
The context in which IPO Lisboa takes part, within the framework of the portuguese health system, is potentially adverse. There are competitive forces for whom the loss of our research capacity and the consequent loss of prestige and capacity to recruit researchers is of great interest.
Some may take advantage from us doing worse, let us not delude ourselves. We are fine, we could be better, and we cannot stop trying. The fight goes on.

anuário de Investigação Científica é uma publicação organizada pelo Centro de Investigação do Instituto Português de Oncologia de Lisboa Francisco Gentil (IPO Lisboa) desde 2011 que reúne de forma sistematizada a produção científica deste Instituto.O Anuário está organizado por capítulos, a que correspondem grandes áreas de i n v e s t i g a ç ã o : “ C a n c r o F a m i l i a r ” ,
“Epidemiologia”, “Investigação Clínica”,
“Investigação Básica e Translacional”, destacando-se também “Outras Áreas de Investigação”, “Atividades de âmbito Académico” (estudos de formação avançada) e “Prémios”.
Da análise global dos indicadores do ano 2020, evidenciada na Fig. 1, pode constatar-se uma variação positiva em relação ao ano anterior na ordem dos +18% para os artigos científicos e de +1%
no número de projetos em curso, tendo diminuído o número de participações em congresso (-30%, em média), por força do cancelamento e/ou adiamento da maioria das reuniões organizadas para 2020 ainda devido à pandemia COVID-19.

he Research Yearbook is a publication organized since 2011 b y t h e R e s e a r c h C e n t r e o f Instituto Português de Oncologia de Lisboa Francisco Gentil (IPO Lisboa), assembling in a systematic manner the scientific production of the Institute.The Yearbook is organized in chapters corresponding to major research areas:
“Familial Cancer”, “Epidemiology”,
“Clinical Research”, and “Basic and Translational Research”, also highlighting
“Other Areas of Research”, “Academic Activities” (Higher education studies) and
“Prizes/Awards”.
The overall analysis of the R&D key production indicators (KPIs) of 2020 as shown in Fig. 1 reveals a positive variation when compared to the previous year:
around +18% for the scientific papers and of +1% for the number of active projects; in c o n t r a s t , t h e n u m b e r o f c o n g r e s s participations decreased by -30% in average, mainly because of cancelation and/or postponement of most meetings due to the COVID-19 pandemic situation.

Detalhando a distribuição dos artigos científicos por categoria de Fator de Impacto (IF) , verifica-se um aumento 1
entre 21 e 43% em todas as categorias exceto na de IF superior a 10 (-48%), como se pode constatar na Fig. 2.
The detailed distribution of scientific papers by category of Impact Factor (IF) reveals an increase around 21 and 43% in all categories except in that of IF higher than 10 (-48%), as shown in Fig. 2.

Durante o ano de 2020 decorreram 247 Estudos Observacionais (EO) e 64 Ensaios Clínicos (EC), totalizando 311 projetos. Nesse mesmo período, foram aprovados/iniciados 64 EO e 11 EC, e terminados 34 EO e 14 EC. A percentagem de EO da iniciativa dos serviços e investigadores foi ligeiramente inferior à do ano anterior (91%, -2%) tendo-se verificado um aumento no número de EO com apoio da
In 2020 there was a total of 311 ongoing projects of which 247 Observational Studies (EO) and 64 Clinical Trials (EC). In that same period were approved/initiated 64 EO and 11 EC, whilst 34 EO and 14 EC were terminated. (+6 EO, 19%), subindo a sua represen- tatividade em 1 ponto percentual face a 2019.
Encontra-se representada na Fig. 4 a distribuição dos EO por grandes áreas de estudo (Fig. 4a) e por patologia (Fig. 4b), destacando-se os estudos de âmbito clínico (41%) e académicos (30%). De salientar o destaque alcançado pelo número de estudos de investigação básica e translacional (20%) onde se incluem alguns dos projetos de formação avançada, e outras áreas de investigação (8%), onde se enquadram estudos em áreas como a infeção hospitalar, medicina física e reabilitação, nutrição e administração hospitalar.

We highlight the significant portion of clinical (41%) and academic (30%) studies, being also noteworthy the number of basic and translational research studies (20%) where some of the higher education projects are included, as well as the number of studies in other areas of research (8%) that address subjects like hospital infection, physical medicine & rehabilitation, nutrition, and hospital administration.

No que respeita à categoria dos estudos académicos, encontram-se detalhados em capítulo próprio 55 projetos de formação avançada em curso durante o ano de 2020.
Destes, foram iniciadas cinco teses de doutoramento (Serviços de Cirurgia da Cabeça e Pescoço, Unidade Investigação em Patobiologia Molecular e Unidade de Transplante de Medula) e 13 projetos de m e s t r a d o ( S e r v i ç o s d e A n a t o m i a Patológica, Cirurgia da Cabeça e Pescoço, Cirurgia Geral, Medicina Física e Reabilitação, Radioterapia e Unidade Investigação em Patobiologia Molecular);
e t e r m i n a d o s c i n c o p r o j e t o s d e doutoramento (Serviços de Anatomia Patológica, Gastrenterologia e Radiologia) e seis projetos de Mestrado (Serviços de Administração, Anatomia Patológica, M e d i c i n a F í s i c a e R e a b i l i t a ç ã o , Otorrinolaringologia, Pediatria e Unidade Investigação em Patobiologia Molecular).
Representam-se na Fig. 5 os projetos de âmbito académico discriminados por ciclo de estudos.
Regarding the academic studies, the 55 higher education projects developed at IPO Lisboa are further detailed in a specific chapter. During 2020 were initiated five PhD theses (in head & neck pathology, basic & translational research and in bone marrow transplantation) and 13 Master dissertation projects (in a n a t o m i c a l p a t h o l o g y , b a s i c &
translational research, general surgery, head & neck surgery, physical medicine &
rehabilitation, and radiotherapy); and were terminated five PhD projects (in anatomical pathology, GI pathology and radiology) as well as six Master projects (in anatomical pathology, basic &

Embora fosse previsível o aumento da produção científica em resultado do número de projetos em curso no ano anterior, os resultados de 2020 superaram o esperado, quer em termos de publicações, quer de projetos.
Ainda assim, os indicadores não deixam de refletir pela negativa as medidas de contingência vividas durante o ano transato, especialmente as atividades que exigiam interação com presença física, como é o caso da participação em congressos ou em ensaios clínicos.
Espera-se, no entanto, que este seja um reflexo passageiro, sinónimo de uma fase de adaptação e que, gradualmente, também estes indicadores venham a retomar, ou até suplantar, a sua expressão.

Although an increase in scientific production was foreseeable as a result of the number of projects already in progress, the results of 2020 exceeded expectations, both in terms of publications and projects.
Nevertheless, the KPIs also reflect on a negative note the contingency measures practiced during the past year, especially regarding activities requiring as much interaction as physical presence, like congress participation or clinical trials.
This is expected, however, to also become a transient and adaptive response that will gradually resume or even supplant their usual expression.
