O n e x a m i n a t i o n ther e w a s a m a c u l o p a p u l a r r a s h o n hi s face , anterio r t h o r a x a n d l i m b s ; o n h i s c h e e k s th e e r y t h e m a w a s diffuse , s l i g h t l y r a i s e d a n d e d e m a t o u s ; o n h i s t h o r a x a n d l i m b s a r e t i c u l a t e p a t t e r n w a s seen ( F i g . 1). T he s e c o n d p a t i e n t w a s a four y e a r old girl , sister o f the firs t p a t i e n t, w h o w a s e x a m i -n e d a t th e s a m e t i me a -n d h a d a three d a y h i s t o r y of a c u t a n e o u s r a s h . E x c e p t fo r m i n i m a l f a c i a l e r y t h e m a , th e e r u p t i o n s h o w ed th e s a m e p a t t e r n as d e s c r i b e d fo r he r brother . N o c o n s t i t u t i o n a l s y m p t o m s wer e r e c o r d e d i n eithe r case .
D u r i n g th e a c u t e p h a s e o f th e i l l n e s s , s e r u m s a m p l e s , f a e c a l s p e c i m e n s a n d t h r o a t s w a b s were t a k e n f r o m e a c h c h i l d . O n e m o n t h late r further s e r u m s a m p l e s wer e o b t a i n e d fro m b o t h c h i l d r e n . t i o u s m o n o n u c l e o s i s , t o x o p l a s m o s i s a n d a r b o v i r u s d i s e a s e s . W i t h r e s p e c t t o th e -l a t t e r v i r a -l a g e n t s , e p i d e m i c s o f O r o p o u c h e a n d M a y a r o f e v e r s a n d D e n g u e ( w i t h r a s h b e i n g c o m m o n l y seen) h a v e b e e n e x t e n s i v e l y reporte d i n ou r re -g i o n1 3' u -1 9.
T h e r e h a v e frequentl y b e e n c a s e s o f " r u b e l l a l i k e " illnesse s i n w h i c h a l l th e a b o v e m e n t i o n e d test s ar e c o n s i s t e n t ly n e g a t i v e , a nd thei r ae -t i o l o g y s-til l r e m a i n s -t o b e e l u c i d a -t e d , p a r -t i c u l a r l y i n c h i l d r e n . I t i s p o s s i b l e t h a t B 1 9 v i r u s is r e s p o n s i b l e fo r a p r o p o r t i o n o f s u c h u n d i a g -n o s e d cases . C l i n i c a l e v i d e n c e s u g g e s t s t h a t the diseas e o c c u r s m a i n l y d u r i n g th e firs t h a l f of th e y e a r , w h en th e h i g h e s t r a i n f a l l a n d h u m i d i t y ar e r e g i s t e r e d .

O u r p r e l i m i n a r y f i n d i n g s i n d i c a t e t h a t "fift h d i s e a s e " o c c u r s in th e A m a z o n r e g i on w i th a pos s i b l e s e a s o n a l p a t t e r n . F u r t h e r s t u d i e s s h o u l d be c o n d u c t e d i n o r d e r t o e v a l u a t e th e p u b l i c h e a l t h i m p o r t a n c e o f t h is disease . It i s i m p o r t a n t t h a t i t s h o u l d n o t b e m i s d i a g n o s e d as a n " a l l e r g y " , t h u s l e a d i n g t o th e u n w i s e u s e o f s t e r o i d s ; this w a s see n i n on e p a t i e n t o f ou r serie s w h o s e p e r i o d o f i l l n e s s h a d b e e n p r o l o n g e d a s a r e s u l t of c o r t i c o s t e r o i d t h e r a p y .
