A literatura aponta diversos desafios para integrar a avaliação espiritual na prática clínica da Enfermagem, dentre eles a falta de tempo para o diálogo, a sobrecarga de trabalho, os turnos rápidos, a falta de continuidade do cuidado espiritual, os sentimentos de incompetência para reconhecer necessidades espirituais e promover intervenções, além da ausência de instrumentos adequados ao desenvolvimento de adolescentes, validados para mensurar a espiritualidade e religiosidade dessa população (COTTON; MCGRADY; ROSENTHAL, 2010; EDWARDS et al., 2010; RASSOULI et al., 2015). A disponibilidade de escalas, para avaliar o bem-estar espiritual em adolescentes com doença crônica no Brasil em que os resultados são autorreportados e reportados por um observador parental, será útil para obter mais detalhes sobre o bem-estar espiritual de adolescentes, apoiando enfermeiros e pesquisadores no planejamento e avaliação de intervenções que promovam o aumento dos níveis de bem-estar espiritual neste grupo.

Porque as versões adolescente e parental da escala FACIT-Sp-12 são curtas, levando em média três minutos para serem respondidas, e foram consideradas compreensíveis e relevantes pelos pais e adolescentes, elas podem ser incorporadas à prática clínica pelos enfermeiros para a avaliação do bem-estar espiritual. Além disso, seus itens podem nortear conversas sobre assuntos relativos à espiritualidade com os adolescentes, envolvendo seus pais e/ou cuidadores. Dessa forma, o cuidado integral de Enfermagem poderá ser prestado ao adolescente a partir da instrumentalização dos enfermeiros para conduzir uma avaliação do cuidado espiritual, inclusive com a possibilidade de considerar a perspectiva da família (pais ou cuidadores principais).

As versões adolescente e parental da escala FACIT-Sp-12 validadas poderão ser utilizadas para testar intervenções clínicas, explorando o constructo nessa população, por exemplo, a partir da avaliação dos níveis de bem-estar espiritual na perspectiva de adolescentes com diferentes condições crônicas e fases da doença ou de seus pais e/ou cuidadores, bem como avaliar os fatores que influenciam o nível de concordância entre eles.

Conclusão

110 Os 12 itens da FACIT-Sp-12 foram adaptados para adolescentes brasileiros com doença crônica e foi desenvolvida uma versão parental para mensurar o bem-estar espiritual de adolescentes. As versões adolescente e parental mostraram validade de face, validade de conteúdo e validade convergente e níveis aceitáveis de consistência interna para a escala geral e o modelo de dois fatores. A validade de constructo discriminante lançou dúvida sobre o modelo de três fatores da FACIT-Sp-12 para adolescentes brasileiros, devido à falta de discriminação entre "Significado" e "Paz" em ambas as versões adolescente e parental. No entanto, estudos adicionais com amostras maiores de adolescentes e pais são necessários para confirmar a equivalência e validação de constructo das versões adolescente e parental da escala FACIT-Sp-12. Estudos futuros devem testar se as estruturas fatoriais das escalas consistem na estrutura psicométrica desenvolvimental de dois ou três fatores, pois ainda se desconhece se as dimensões são mensuradas semelhantemente à escala em adultos. A validação da medida para uso com adolescentes deve ainda ser explorada em relação à invariância da estrutura fatorial entre grupos etários e respondentes.

Nenhum efeito floor e ceiling foi observado nos escores de resposta das escalas. Uma correlação significativa e moderada foi observada entre as respostas na escala geral e na dimensão “Fé” da versão adolescente e parental. As dimensões “Significado” e “Paz” ou “Significado/Paz” mostraram uma significativa e fraca correlação. As escalas apresentaram ainda validade de grupos conhecidos, pois adolescentes praticantes de uma religião apresentaram maiores escores do que adolescentes não praticantes na dimensão “Fé” e na escala total para bem-estar espiritual. Similarmente, pais ou cuidadores praticantes de uma religião atribuíram maior escore para adolescentes na dimensão “Fé” e na escala total do que pais não praticantes de uma religião. Ambas as versões adolescente e parental diferenciaram significativamente os adolescentes diagnosticados com diabetes, fibrose cística e câncer. Estes grupos diferiram significativamente na dimensão "Fé", em ambas as versões. Adolescentes com câncer tiveram maior nível de bem-estar espiritual do que adolescentes com diabetes em ambas as versões. Similarmente, adolescentes com câncer mostraram mais fé do que adolescentes com diabetes, e pais de adolescentes com câncer atribuíram mais fé para seus adolescentes do que pais de adolescentes com outras condições.

Considerar a dimensão espiritual do paciente no planejamento do cuidado é parte das atribuições da Enfermagem. As duas escalas, versão adolescente e parental, da FACIT-Sp-12 para avaliar o bem-estar espiritual em adolescentes brasileiros com doença
