2.4 SÍNDROMES CORONÁRIOS AGUDOS COMO CONDIÇÃO MUITO HETEROGÉNEA E O EXEMPLO DO USO DOS SCORES DE RISCO NA SUA ESTRATIFICAÇÃO
Os SCA representam uma condição muito heterogénea, englobando um largo espectro de doentes em diferente risco de eventos CV, sendo essencial a sua estratificação de risco. Assim, constituem um cenário clínico exemplar para o desenvolvimento e aplicação de scores de risco, ferramentas essenciais para a tomada de decisão no manejo destes doentes, podendo tirar-‐se daqui varias lições úteis para o desenvolvimento e implementação de scores noutras áreas da DC.
Em 2005 o nosso grupo publicou um estudo onde foi comparado o desempenho de 3 scores de risco (GRACE, TIMI e PURSUIT) na estratificação dos doentes com SCA (30). Foi possível demonstrar que o score de risco GRACE era o que tinha um desempenho superior, fruto não só do tipo de variáveis que incluiu, mas também de algumas delas serem valorizadas de forma contínua. Este era o score que melhor previa a ocorrência de eventos cardíacos a 1 ano e permitia identificar os doentes que colhiam maior beneficio da revascularização, sendo actualmente recomendado pelas guidelines europeias para a estratificação destes doentes (31). No entanto, o score de risco TIMI pela sua simplicidade, continua a ser usado na pratica clínica e mesmo em vários estudos, como é o caso dos recentemente publicados sobre o uso da angio TC cardíaca na avaliação dos doentes no serviço de urgência (32, 33).
Uma das lições retiradas desta experiencia, foi que um score para entrar na pratica clínica tem que ter um balanço entre complexidade e desempenho. Apesar de o desempenho ser directamente proporcional ao número de variáveis incluídas, também a sua complexidade aumenta, diminuindo assim a sua potencial aplicabilidade clínica. Da mesma forma, o modo com as variáveis são operacionalizadas (continuas ou categóricas) também é determinante, ficando os scores tendencialmente mais simples, mas perdendo algum poder discriminativo, quando se opta por usar as variáveis como categóricas em vez de continuas.
Este conceito esteve no racional de desenvolvimento do CT-‐LeSc onde tentamos procurar esse equilíbrio difícil entre complexidade e desempenho, com vista ao seu uso futuro como ferramenta para quantificação da carga aterosclerótica coronária sendo de esperar que possa traduzir a informação prognostica da angio TC cardíaca e ser adoptado na pratica clínica.
