BIOMARCADORES DE LESÃO RENAL NAS DOENÇAS TROPICAIS

1. Introdução

2.9 USO DOS NOVOS BIOMARCADORES NAS DOENÇAS TROPICAIS

Apesar do risco aumentado para doença renal das doenças tropicais, a aplicação de novos biomarcadores para detecção e avaliação do prognóstico nesses pacientes tem sido escassa e não acompanharam o ritmo como em outros contextos clínicos. De fato, isso também caracteriza essas doenças tropicais como doenças negligenciadas no serviço de saúde.

Nosso grupo tem estudado a fisiopatologia renal nessas doenças, e nos últimos anos foram avaliados diferentes tipos de biomarcadores que puderam ampliar o conhecimento a respeito do tipo de lesão renal, a detecção precoce, sobre grupos de risco para doença renal e de mecanismos de lesão renal. Já foram avaliados pacientes com leishmaniose tegumentar e visceral, leptospirose, esquistossomoses e hanseníase.

Em 2011, foi mostrado pela primeira vez a disfunção tubular renal em pacientes com leishmaniose Tegumentar (LT) e associação com a expressão alterada de transportadores tubulares renais e mecanismos compensatórios para essas disfunções(OLIVEIRA et al., 2011). Nesse estudo foram avaliados exossomas urinários para a elucidação da etiologia em defeitos de concentração e acidificação urinaria encontrados nos pacientes aparentemente assintomáticos. Foi evidenciado que os defeitos na concentração urinaria foram causadas por uma diminuição da expressão de aquaporinas (AQP2), sendo acompanhada do aumento compensatório do transportador NKCC2 que reabsorve sódio, potássio e cloreto para o interstício, apontando para problemas na produção de AQP2 no ducto coletor. Por outro lado, defeitos da acidificação foram evidenciados pelo aumento do trocador apical pendrina de células -intercaladas do ducto coletor cortical. A pendrina secreta bicarbonato no lúmen e aumenta o pH urinário. Além disso, foi demonstrado

também mecanismos compensatórios devido ao aumento do trocador NHE3, que secreta íons H+ no lúmen (OLIVEIRA et al., 2011). Contudo, a estratificação dos pacientes com tais alterações e seus impactos na função renal a longo prazo não foi estudada.

Na leishmaniose visceral, em um estudo de caso controle com 16 pacientes adultos, foi demonstrado que os níveis de MCP-1 urinários estavam significativamente mais elevados nos pacientes, em comparação aos do grupo controle, associado também a alterações de concentração e acidificação urinárias (OLIVEIRA et al., 2014). O MCP-1 também demonstrou ser um biomarcador útil na hanseníase, estando associado à carga bacilar (quanto maior a carga bacilar, maiores os níveis de MCP-1 urinários) e à albuminúria (MENESES et al., 2014). Mais recentemente, outro estudo com pacientes com leishmaniose visceral (com 50 pacientes hospitalizados) evidenciou o papel do NGAL sérico como um bom biomarcador precoce de lesão renal, sendo este o melhor marcador para LRA, dentre aqueles que foram pesquisados (NGAL sérico, NGAL urinário, KIM-1 urinário, MCP-1 urinário), com uma sensibilidade de 82,6% e especificidade de 74,1% (MENESES et al., 2018). Por fim, outra condição que, apesar de não ser doença infecciosa, mas que está na lista de doenças tropicais negligenciadas da OMS desde 2017, é o acidente ofídico, no qual é frequente a ocorrência de lesão renal aguda. Neste contexto ainda há poucos estudos investigando os novos biomarcadores de lesão renal. Em um estudo com 63 pacientes vítimas de envenenamento por serpentes do gênero Bothrops, foi evidenciado que os biomarcadores urinários MCP-1 e NGAL estiveram associados ao desenvolvimento de lesão renal aguda, proteinúria e fração de excreção de sódio(ALBUQUERQUE et al., 2019). A identificação precoce da lesão renal nestes acidentes é fundamental, uma vez que um percentual significativo dos pacientes evolui com necessidade de diálise e doença renal crônica.

3. CONSIDERAÇÕES FINAIS

As doenças tropicais representam um dos principais problemas de saúde pública do mundo, gerando grandes impactos ao sistema gênito-urinário. Novos biomarcadores são promissores em detectar precocemente as lesões renais no contexto destas doenças e são

combinação entre diferentes biomarcadores, para a detecção de lesão em diferentes segmentos renais, no intuito de diagnosticar precocemente o acometimento renal em diferentes doenças tropicais, para que se possam adotar medidas mais específicas e eficazes de tratamento.
