Inhaled nitric oxide and high concentrations of oxygen in

pediatrics patients with congenital cardiopathy and pulmonary

hypertension: Report of five cases

Five patients w ith ages ranging from 6 m onths to 3 years w ere analyzed. All received inhaled nitric oxide (N O - 20 parts per m illion (ppm ) ) and oxygen (0

2 - at a concentration of 90-95% ) by m eans of an oxygen hood. M ean Pulm onary Artery Pressure (M PAP), M ean Aortic Pressure (M AoP), Pulm onary Vascular R esistance (PVR ) and System ic Vascular R esistance (SVR ) w ere m easured and the calculation of their relationship to pulm onary/system ic flow (O p/O s) w as perform ed by the catheterization' of the fem oral artery vein. Three patients presented reduction in PVR and increase in O p/O s. There w ere no system ic alterations or any side effect from using N O .

PH (I). Pulm onary hypertension contributes to the

w orsening of hypoxem ia and overload of the right

ventricle. T hese cardiopathies lead to hypertrophy and

hyperplasia of the vascular m usculature and consequent

vasoconstriction. C onventional treatm ent after, surgery

consists of using hyperventilation, alkalization, increase

in the inhaled oxygen fraction, inotropics and vasodilators.

T he great advantage in using inhaled nitric oxide

(N O ) is that, contrary to system ic vasodilators, it only

reaches the ventilated alveoli, thereby causing selective

pulm onary vasodilation (2,3) w ith deviation of the flow

to these areas, thus im proving oxygenation and

intrapulm onary shunt, in a m anner sim ilar to w hat occurs

w ith the use of high oxygen concentrations in the

evaluation prior to surgery, during catheterization.

p r o c e d u r e a s lo n g a s s o m e p r e c a u tio n s a r e ta k e n : tr a in in g

o f th e p r o f e s s io n a l g r o u p r e s p o n s ib le f o r u tiliz a tio n o f th e

g a s , a n d c o n s ta n t m o n ito r in g o f N O /N 0 2 co n cen tratio n s
