The aimofan editorialis tomakeusthink!Ichose,from amongseveraltopics,onethatIconsiderveryimportantfor ustoreflectontogether:beingaphysiciantoday.
Weworkinourprofession,intensely,formanyhoursof theday.Treatingpublicorprivatepatients,whileothersare doingresearch,teachingandevenworkingwithinstitutional management.
Asthemajorityworkscaringforpatients,Iwillprioritize thisdiscussion.
Patient carein publicinstitutionsis ina chaoticstate. MedicalschoolsarebeingcreatedthroughoutBrazil, with-out at least havinga teachinghospital, which is ourtrue classroom.School---hospitals thatcoexistwithother public hospitalsareinastateofdereliction,regardingequipment, withoutmedicationsforareasonablecaretobeoffered,in contrasttoademandthatgrowsexponentially.
Wefaceendlesslinesandshiftswithpoorconditionsof service.
Whataboutpatientsfromprivatehealthplansandhealth insurance?
Toanswerthisquestion,Irememberhowmanychildren with adenotonsillectomy indication due to upper airway obstructionandevenapneaarenotoperatedduetoalow medicalhonorarium.Whilepatients withhealth insurance undergoprocedureswiththe abusiveuse of laser, naviga-tors, robotic surgery, nerve monitoring, biological glues, sophisticatedtools and productsfor hemostasis.All these increasecostsunnecessarily,leadingtoan‘‘armwrestling’’ betweenprivateinstitutions,doctorsandhealthinsurance companies.
Wearenotagainsttheadvancesoftechnologyinfavorof ourpatients,butthesustainabilityofsupplementaryhealth dependsongoodmanagement.
Wecannotforgetthatthisentirecontextisproperly sea-soned withthe judicialization of our service. There is an actualincreaseinthenumberoflawsuitsagainstphysicians

Idonotthinkso.Thefuturebelongstous,doctors,with energeticandpreciseattitudes. Thecohesionoftheteam isessential.Withinourspecialty,thiscanbeachievedwith thejoining ofallotorhinolaryngologiststotheABORL-CCF, whichisourofficialrepresentativeorgan.
Increasingthenumberofworkers’cooperativesinsmall towns, changingthe current fees,ensuring the qualityof medicaleducationandspecialties...Wehavetoappreciate
theeffortwehavemadetobecomedoctorsandhowmuch weloveandarededicatedtoourprofession.
Politics is not made exclusively in Brasilia; it is made mainlyinthecommunities,ininstitutions.Wehaveto main-tainourethicswhilebeingrespected,byoccupyingspaces. Ifwedonotoccupythem,otherswilldoit.
