Rever as evidências disponíveis sobre a efetividade da antibioticoterapia profilática em cirurgias urológicas

FUNDAMENTAÇÃO
a. Objetivos da antibioticoprofilaxia
1. Prevenir infecções genito-urinárias como pielonefrite, cistite, prostatite, epididimite e urosepse
2. Prevenir infecções de ferida operatória b. Considerações gerais:
1. A profilaxia antimicrobiana é apenas um dos componentes da prevenção de infecções pós-cirúrgicas.Outros fatores como sistema de drenagem de urina fechado e cuidados com o cateter urinário não podem ser negligenciados.
2. Os benefícios da profilaxia em reduzir a carga bacteriana no sítio cirúrgico deve ser contrabalançada com seus efeitos negativos: seleção de bactérias resistentes e efeitos adversos dos antibióticos 3. A permanência de sonda vesical deve ser a menor possível
4. A profilaxia antibacteriana deve ser aplicada na indução anestésica, para que, no momento da cirurgia, se tenha níveis teciduais adequados do antibiótico para prevenir a colonização da ferida operatória
5. A presença de infecção urinária no pré-operatório é o maior fator de risco para infecção cirúrgica em urologia(Int J Urol 2004;11(11):941-7) 6. São fatores de risco para bacteriúria pós-RTU(Eur J Urol
