Practicingmedicineisanindividualactivity,sinceethicsand professionalresponsibilityare personal.Thereisnowayin whichwecanshareresponsibilityregardingmedicalacts,nor canweascribeethicalresponsibilitytoinstitutions.

On many occasions, situations with consequences that affectphysiciansbutinwhichtheirparticipationissmall,can beseen.Themostfrequentoftheseisinadequacyof health-careequipment,whichgoesfromchoosingimplantmaterials tothestateofhospitals.

Ourindividualityisminimizedthroughourprofessional associations,whichinrealityareofrelativelyrecentorigin.

The Brazilian Medical Association (Associac¸ão Médica Brasileira,AMB)originatedfromtheSãoPauloMedical Associ-ation(Associac¸ãoPaulistadeMedicina,APM)in1951,founded byProfessorJairoRamos,anditsfirstpresidentwasthegreat surgeonProfessorAlipioCorreaNetto.Itwasinitiallyasociety ofscientificnature,buttodayitsroleistodefendthemedical professioninpoliticalandinstitutionalarenas.

TheJournaloftheBrazilianMedicalAssociation(Jornalda Associac¸ãoMédicaBrasileira,JAMB)wasinitiallyascientific journalbuttodayitisdedicatedtosubjectsofprofessionaland politicalnature.Itiswidelyread,andisprobablythemedical publicationthatismostreadinBrazil.

Overthese64years,medicinehasevolvedandmany spe-cialtieshaveemergedandhavebecomeautonomous.Inthis manner,medicineinthefullnessofitsextenthasbecome frag-mentedintomanyfields,andtheAMBisleftwiththetaskof overalldefenseofmedicine,withthevariousspecialtiesasits subsidiaries.WeattheBrazilianSocietyofOrthopedicsand Traumatology(SociedadeBrasileiradeOrtopedia e Trauma-tologia,SBOT)are thesubsidiaryofthe AMBinthefield of orthopedicsandwehaveanexcellentrelationshipwiththe AMB.TheSãoPauloregionaldivisionoftheSBOThashadits mainofficeonthepremisesoftheAPMformanyyears.

The AMB is a body that protects and brings together the medical profession and acts in all fields of medical activity whenever requested. Recently, Brazilian specialist medicaljournalswerehavingsomeassessmentandvaluation difficulties with the government funding agency CAPES (Coordenac¸ãodeAperfeic¸oamentodePessoaldeNível Supe-rior;CoordinationOfficeforImprovementofUniversity-level Personnel),anditwastheAMBthathasbroughtustogether toseeksolutionsfortheseproblems.

ThecongressesoftheAMBusedtobeexcellentandthere wasevenadepartmentofcongresseswithintheAMB,headed by Silvia Mangabeira Albernaz and by Celina Fleury, who definedpracticallyeverythingthatexiststodaywithregardto organizingcongressesinBrazil.

Today,AMBcongressesnolongertakeplace,andthereis nodoubtthatthishasweakenedtheinstitutionandhas dis-tancedusfromourprofessionalactivity.Wearephysicians beforeweare orthopedists, butweare unawareofwhatis goingoninotherspecialties,regardlessofhowmuchthese might influenceourpatients and consequentlyour profes-sional activity.Weevenhavedifficulty inrecommending a colleagueinanotherfieldforourclients.

TheSBOTwasfoundedin1935,wellbeforetheAMB,andit isturning80thisyear.Ithelditsfirstcongressin1936,inSão Paulo.

In this eightieth year, we are holding our 47th Brazil-ian Congress of Orthopedicsand Traumatology (Congresso Brasileirode OrtopediaeTraumatologia, CBOT),alsoinSão Paulo.

Whenallthespecialtiesreachmajorityandautonomy,the CBOTwillprobablyhavetochangeitsfocus.

TheDayoftheSpecialty,anideaimportedfromthe Amer-icanAssociationofOrthopedicSurgeons(AAOS),needstobe givenduevalue,sothatwecanattractspecialistsand com-plementgeneralists’training.Wecannotneglectmattersof generalinterest,sincethesemaintaintheunityofthe spe-cialtiesandthusmaketheCBOTviable.

Thesurvival ofthe CBOT willdepend on scientificand commercialfactors.Ifthereisscientificinterest,therewillbe economicinterest.

We will be remaining alert, so that what happened to theAMB congressdoesnothappentotheCBOT,sincethis

is the time whenthe SBOT comestogether and shows its strength.

Asindividualistsnotbychoicebutthroughconsequence, weneedourstronginstitutions.
