Abstract

Introduction:Treatmentsusedinheadandneckcancergreatlyimpactthephysical, psycholog-icalandfunctionalstateofpatients.Evaluationofqualityoflifehasbecomeanintegralpart ofthetreatment.

Objective:This retrospectivestudy evaluatesfeatures involvedinchanges inqualityoflife aftermajorsurgeryforheadandneckcancerwithinsixmonths,accordingtoself-reported outcomes.

Methods:OnehundredandthirtypatientscompletedtheUniversityofWashingtonQualityof Lifequestionnaireoneandsixmonthsaftermajorsurgeryforheadandneckcancer.A multi-variatemodelwasusedtoevaluatewhichdiagnosticandtherapeuticfeatureswererelatedto improvementofqualityoflifewithinasix-monthperiod.

Results:Significantimprovementinmostfeaturesrelatedtoqualityoflifewasalready rec-ognizable at six months. Patients submitted to more invasive treatment had the biggest improvement inquality of life between time-points, as well asthose patients with bigger tumors.

Conclusion: Aftermajorsurgery,patientsmayundergofastrecovery,withoverallqualityoflife likelytoimproveintheshort-term.Cliniciansmustbeawareoftheimportanceofdealingwith treatment-relatedissuesimmediatelyaftersurgery,withhopefulpossibilityofon-the-upgrade results.

Resumo

Introduc¸ão: Ostratamentosadotadosemcâncerdecabec¸aepescoc¸ocausamgrandeimpacto nos estadosfísico, psicológicoe funcionaldospacientes. A avaliac¸ãoda qualidadede vida tornou-separteintegrantedotratamento.

Objetivo: Esteestudoretrospectivoavaliouosaspectosenvolvidosnasmudanc¸asdaqualidade devidapós-cirurgiadegrandeporteparacâncerdecabec¸aepescoc¸onoperíododeseismeses, deacordocomosresultadosautorrelatados.

Método: CentoetrintapacientesresponderamaoQuestionáriodeAvaliac¸ãodeQualidadede VidadaUniversidadedeWashington,umeseismesesapóscirurgiadegrandeporteparacâncer de cabec¸a e pescoc¸o. Ummodelo multivariado foi usadopara avaliar quais características terapêuticasediagnósticasestavamrelacionadasàmelhoradaqualidadedevidanoperíodo deseismeses.

Resultados: Melhorasignificativanamaioriadosaspectosrelacionadosàqualidadedevidajá podiaserpercebidaemseismeses.Ospacientessubmetidosaumtratamentomaisinvasivo apresentaram osmelhoresavanc¸osnaqualidadedevidaentreostemposdeavaliac¸ão,bem comoospacientesportadoresdetumoresmaiores.

Conclusão:Apósumacirurgia degrandeporte,ospacientespodemterrecuperac¸ãorápida, commelhoradaqualidadedevidaglobalempoucotempo.Osmédicosdevemestarcientesda importânciadelidarcomquestõesrelacionadasaotratamentoimediatamenteapósacirurgia, devidoàpossibilidadederesultadosmelhores.

Introduction

Oral/oropharyngealsquamouscellcarcinoma(OSCC)isthe fifth most frequent cancer among males and the seventh amongfemalesworldwide,withprevalencebetween2%and 4%ofallmalignanttumorsinEurope,1andoverallfive-year survival rate of around 60%.2 The etiology of oral cancer isunknown,althoughvariousfactorshavebeenconsidered partofitsdevelopment,suchassmoking,alcoholand onco-genicviruses.3,4Beingasymptomaticforalongtime,OSCCs areoften diagnosed at an advanced stage. Copious extir-pation achieving tumor-free margins,as well aseffective removalofaffected/suspectlymphnodes,arevitalstepsto ensurelong-termsurvival.Althoughtreatmentand recons-tructiveabilitieshaveimprovedtosuchanextentthatmany patients can be fully integrated into society after treat-ment,OSCCcanhaveaprofound impactonthequalityof life(QoL)ofpatientssinceitsresectionremainsassociated withdisfigurementanddysfunctions.5,6Forthisreason,QoL is actuallyconsidered amulti-dimensionalconcept,which comprisestheindividual’sperceptionoftheirstateofhealth

withintheculturalcontextandvaluesystemwheretheylive andinrelationtotheirgoals,socio-demographicparameters andsocialrelations,7andanintegralpartoftheoutcomeof treatment.8

Many studies report that OSCC patients suffer from a worse cosmetic outcome, restricted independence in dailyactivitiesandrecreation,seriousdeficitsinchewing, swallowing and speech, and frequent mood and anxiety disorders.9 This is due to the fact that advanced OSCC requires highly destructive surgery with wide resections that involve transmandibular approaches, at times with resection,neck dissectionand flapreconstruction aswell asadjuvant radiotherapy (RT), with inevitable functional impairment.10

Time-points chosen were one month (T1) and six months (T6).Differently fromthe original versionof UW-QoL, we propose a more focused analysis of the questionnaire in order to analyze in depth every single outcome investi-gated. To our knowledge, a few studies have considered variationsofQoLparametersintheshortterm.11,12

Methods

The study was conducted at the Head and Neck Depart-mentof‘‘OspedalediCattinara’’,incollaborationwiththe Departmentof OralMedicine and Pathology of‘‘Ospedale Maggiore’’ (34100, Trieste, Italy), to evaluate the post-operativeQoLofpatientssubmittedtoinvasiveoncological treatmentforOSCC.Asampleof140patientswasselected. Inclusioncriteriawere:surgicaltreatmentbetween2001 and 2008; with/without adjuvant RT, diagnosis of OSCC (accordingtoTNM);ASAI,II,III.

Specifically,allthepatientsincludedinthepresentstudy hadnotstartedRTatT1andhadalreadyfinishedRTatT6. Exclusion criteria were: neo-adjuvant RT; ASA IV12; concomitant/previouschemotherapy.

Diagnostic andtherapeutic features were recorded via examinationofhospitalrecords.

Evaluation of QoL was carried out using the UW-QoL questionnaire, which is composed of 16 multiple-choice questions.Eachquestionhasthreetofivemultiplechoices, as indicated in Table 1: the higher the score, the worse therelatedcondition. The lastthreequestions,regarding overallQoL,wereconsideredasawhole.Questionnairewas handedouttopatientsatT1andT6aftersurgery.

Differently from the original version of the questionnaire,13 each question was considered a ‘‘QoL’s outcome’’andusedtoevaluateimprovementbetweenT1 and T6 through a regression model. This kind of analysis givesmore focused results and isolatesevery single issue presented in the questionnaire, thus providing an imme-diate clinical implication. Being all patients Italians, the original version of the questionnaire was translated into Italianbyabilingual(ItalianandEnglish)translator.

The study wasconductedafter approvalof the ethical committee(prot.No.383/2009;54/2009).

Statisticalanalysis

The SPSS® software 11.0 (Chicago, Illinois, USA) was employed.Foreach oftheQoLoutcomes,thesignificance of thedifference between thetime-points was evaluated throughaWilcoxonpairedsignranktest.

Subsequently, theadjusted correlationsof each of the diagnosticand therapeutic featureswith each ofthe QoL outcomes were evaluated by backward multiple logistic regressions,building13separateregressionmodels.In par-ticular,thechangesineachoftheQoLoutcomes,computed asthedifferencebetweenthescoresrecordedatT6andT1 i.e.positive valuesfor improved quality,were considered asthedependentvariables.Theexplanatoryvariables (cat-egories)enteredineachmodelwere:age,sex,tumorsite; Tstage;Nstage;treatmenttype;neckdissection;surgical approach;reconstruction.Thecut-offlevelsofsignificance

were0.05and0.10for entryandremoval,respectively. A p-value<0.05wasconsideredstatisticallysignificant.

Results

A total of 140 patients were included, and 130 (33.8% females and 66.2% males, with cumulative mean age of 65±11)completedthequestionnaire atbothtimepoints. Death of patient, incomplete questionnaire or missing a timepointjustifiedadropout.Descriptivefrequenciesand univariateanalysesaresummarizedinTable2.Descriptive analysisofdiagnosticandtherapeuticfeaturesispresented inTable2.

Table3showsimprovementofQoL’soutcomesbetween timepoints(p<0.001).

Multiple regression was performed to correlate QoL’s improvement and diagnostic and therapeutic features for eachoutcome(Table4).

improvement (OR 0.3 and 0.9 respectively) instead. Con-cerningpainsensation,malesimprovedlessthanfemales’ (OR0.3) at T6. Patientswhowere submittedtoadjuvant RT experienced a greater improvementof pain over time (OR 6.4). Moreover, a tumor in the oral cavity proved to bemore painfulat T6than atT1 ifcomparedtoatumor of theoropharynx(OR3.1). Consideringappearance, per-formanceofneckdissectionandtypeofsurgicaltreatment determinedanimprovementbetweentimepoints:themore invasivetheintervention,thegreaterimprovement experi-enced (p<0.05).To mention, conservativeand demolitive transmandibular interventions triggered greater ameliora-tionofappearancewhencomparedtotransoralapproaches (OR 3.4 and 5.5, respectively). Concerning the capacity of continuingdailyactivity,at T6malesfeltmore limited comparedtofemales(OR0.3),asdidpatients who under-wentsurgicalinterventionwithoutadjuvantRT.Inaddition,

patients diagnosed with T3 cancer resumed their daily activities more easily than patients with T1 cancer (OR 4.0), as did patients who were submitted to more inva-siveintervention(OR 3.9).Regardingrecreationoutcome, T2 (OR 5.1) and T3 (OR 7.0) tumors had major improve-mentbetweentimepoints;thesamehappenedforpatients whowent through more invasive surgical approaches (OR 4.9formicrovascular freeflap).Themaintenanceofgood swallowingcapacityovertimewashigherinfemalesthanin males(OR0.4)aswellasinpatientswhowerenot submit-tedtoneckdissection(OR3.9).Regardingchewingcapacity, greater improvement was evidenced after microvascular free flap (OR 5.3). In regards to speech capacity, older patients improved more than younger ones, and patients affectedby oral cavity tumorshad a better speech func-tion at T6 than patients affected by oropharynx tumors (OR0.5).Accordingtoreconstructivetechnique,theworst speechfunctionat T6wasregisteredinpatients operated throughlocalflaps(OR0.1).Shoulderpainimprovedmore at T6 in patients operated for oral tumors compared to patientsoperatedfor oropharynxtumors(OR0.4),aswell as in patients who did not undergo neck dissection (OR 5.2).Demolitivetransmandibular approachwasassociated tohighershoulder painimprovement over time (OR1.7). Inaddition,adjuvantRTwaslinkedtosalivation’s improve-ment at T6 (OR 3.2). Regarding mood, females improved morethanmales(OR0.4)at T6.T2tumorstageappeared tobethemostinfluencingcategoryforthesameoutcome (OR3.3).Accordingtoanxiety,femalesexperiencedgreater improvementthanmen(OR0.3)atT6.Eventually, conser-vativetransmandibularapproachwastolerated worse(OR 0.7)thandemolitiveinterventions.

evaluationwasperformedinashorttermofsixmonths. Pre-operativeevaluationwasnotconsidered,sincetheobjective of the present investigation was tooutline if therewere significant differences between time points in the short termfollow-up.Nevertheless,abaseline(beforetreatment) indicator would be of great interest, in conjunction to a psychologicalexamination,whichwarrantsfuturestudies.

The UW-QoL questionnaire was chosen asit may offer a platform to identify those with significant problems in anyofthe13domainsassessed.14 The modifiedversionof thequestionnairewe propose offersthepossibilityof iso-lating every singleoutcome asa specific feature, sothat treatmentispotentiallymoreandmorefocused. Neverthe-less,themultivariatemodeljoinsalltheoutcomes,givinga comprehensiveandaccuratereproductionofpatients’state ofhealth.Recently,QoLmeasureshave beenincorporated intoroutineclinicaloncologytomonitorandscreen individ-ualsforsignificantdysfunction/problems,whichtheywould be expected to encounter, thereby triggering healthcare intervention.15

Shown below, each of the diagnostic and therapeutic featureshasbeenanalyzedseparatelyandcorrelatedto rel-evantQoLoutcome,derivedfromthemultivariateanalysis. Thisapproachmayhelpthecliniciantoestablisha hypothe-sisonhowQoLcouldvarybetweenoneandsixmonthsafter surgery,simplyconsideringthe diagnosticandtherapeutic features.

Sex/age

Men showed less improvement than females and proved to bemore anxious and moodyat T6.This can be corre-latedtotheconditionofdistress,whichisfrequentinOSCC patients and more likely to develop in men.16 In regards toage,prolongedmedicaltherapies,surgicalcomplications andreconstructionsmustbecarefullyevaluatedinelderly patients,sincetolerationcouldbelower.Unlikeour expec-tations,QoLoutcomeswerenotaffectedbyage,incontrast topreviousstudies.17

adjustinglifebeyondcancer.19Thisconditioncanbedefined as ‘‘distress’’. Generally, distress strongly correlates to painscoresandoccurrenceofphysicalsymptoms,aswellas tobadmood/anxiety.20Moodpositivechangeswerealready evident in our patients at T6, almost in all stages, most significantly in T2 patients. This occurs proportionally to acceptanceofoneself,declineofsymptomsandgoingback todaily activity.Consequently, psychological supportmay be recommended immediately after surgery. Surprisingly, no correlation between nodes’ stage and QoL domains wasevidenced in our regression model. Actually, surgical techniquesforselectivenodedissectionaremoreandmore advancedandtumorsite/size-specific;thisallowsthebest outcomeoftreatment.21

Tumorsite

Inthe present study,oral andoropharyngeal tumorswere considered.Incaseoforopharyngealtumors,improvement atT6wasregisteredforpainandactivity,whereasspeech and shoulder reported minor improvement in respect to oral tumors.This canbecorrelatedtothefactthat orop-haryngeal tumors are often diagnosed at an advanced stage,due todifficultiesin objectiveexaminationaswell asto scarcity of symptoms, inevitably leading to demoli-tiveinterventionandlong-lastingproblemssuchasspeech dysfunction. Although long-term rehabilitation is usually performed,completespeechrecoveryisusuallynot achiev-able,andspeechintelligibilityremainsthemajorobjective oftreatment.22

Treatmenttype

Patients treated for OSCC undergo different thera-pies, including surgery and neo-adjuvant/adjuvant RT/chemotherapy (CT). Surgery for OSCC is demoli-tive and invasive, due to anatomic characteristics of maxillofacial district as well as to evolution pattern of OSCC. The systemic metastases of OSCC occur via ipsi-lateral/contralateral lymphatic channels, so that lymph nodes’dissectionisperformedtoensurethatriskofoccult metastasis’ risk is minimized. Serious complications as esthetic appearance, changing in daily activity, social concerns andfunctional impairmentmayoccur.23 Patients usuallyundergoadjuvantRTincaseofextracapsularspread and/or lymph nodes involvement.24 RT, in addition to surgical treatment, related to increased pain sensation anddifficulty indaily activity.Aftersurgical resectionfor oral cavity OSCC, adjuvant RT may be recommended for patientsathigherriskforlocoregionalrecurrence.25Newer protocols recommend the association of adjuvant RT and CT aftersurgery, although this increases therisk of early andlatecancertherapy-related sideeffects.26 Infact, RT isundoubtedlyassociatedwithacutesideeffects,suchas mucositis, xerostomia, taste loss, swallowing difficulties, cariesandtrisma,aswellaswithchroniccomplications.In OSCCpatients,xerostomiarepresentsalatesideeffectof majorconcernforRT-treatedpatients.Wehaveregistered amelioration between time points in patients submitted to RT in association with surgery. Although subjective xerostomiaseemstoimproveat T6inRT-treatedpatients,

only32% of patientsactually improved. This resultis not reliable on the long term, since chronic complications usuallystartsix to12months afterRT.The present study didnotconsideredthedevelopmentofsideeffectsrelated toRT,despitethefactthatthiscouldbeofgreatinterestfor furtherinvestigation.Inanycase,someRT-relatedfactors, suchaspainsensationandperformingofdailyactivity,are likelytoamelioratewithinsixmonths,socliniciansshould beconcerned withpatients’ needs according to the time periodconsidered.

Surgicalapproach

SeveraltypesofsurgicalapproachesexisttomanageOSCC. Actualtechniquesare‘‘defect/patientorientated’’; conse-quently,welldefined aswellasmore andmoresurgically advancedreconstructive techniques have been developed tocompensatesurgical/functionaldefects.27Thecomplete conservation of mandibular bone continuity is achieved through ‘‘transoral approaches’’. In case of pharyngeal tumors,itisnotrecommended,sinceit isassociatedwith many post-operative complications such as hemorrhage, fistulae, dehiscence and nerve damage. On the contrary, thetransmandibularapproaches,althoughmoredemolitive, allow greater intraoperatory visibility and surgical man-agement. Lots of advantages are associated with direct visualizationoftumor,derived fromawidefieldof access suchasgoodcontrolof bleedingand lightinsertion; also, some studies evidenced less post-operative complications inpharyngealtumors.28

In our study,patients who were submitted to demoli-tivesurgeryimprovedmoreintheshorttermthanpatients submitted toconservative intervention. This implies that althoughtransmandibularinterventionisfarmoreinvasive andsurgicallycomplicated, itis associatedwithtolerable recoveryatT6.Inaddition,transmandibular interventions wereassociatedwithdecreasedshoulderpainatT6. Shoul-der pain is frequently recognizable in patients who go through neck dissection due to tumor spread, but it is generallymanagedwithphysiotherapyandfrequentlyfast recovering.29 Afterthediagnosisofcancerandinthe peri-operativephases,asensation ofdistressmayarise,andit issometimes difficulttoeradicate.The distressisrelated to fear of death, sensation of decline, pessimism.30 The improvementinvariousQoLoutcomes,whichwehave pre-sented so far, may contribute to general diminishing of anxiousbehavior.

reported an improved overall QoL at T6 (p<0.05). Func-tional impairment may be noticed among OSCC patients immediatelyaftersurgicalintervention.Ingeneral,chewing andspeechfunctionimprovedbetweenT1andT6,butthese factors were undoubtedly associated with reconstructive technique,since patients submitted tomore complicated andinvasive interventionsexperiencedagreater improve-ment at T6. In agreement with literature, a six-month evaluationisreliabletoassertthatfunctionalimprovement canbeexpectedaftercomplexreconstructivetechniques, withlong-term stability.32 Reconstructive techniques that exploitfreeflapreconstructionshaveincreasedlocal con-trolandlong-term survivalincancerpatients.33 Moreover, microvascular free tissue transfer has revolutionized the approachtothereconstructionofcomplexdefects, provid-ingasafe,reliable proceduretorestorefunctionalityand QoL for patients. Our results evidenced that a six-month periodaftersurgeryisareliableindicatorofQoL improve-ment. Manystudies assert that reconstructive techniques areassociatedwithworstQoLoutcomesimmediatelyafter surgery,sincesuchproceduresimplypain,swellingand func-tionalimpairment.34,35AtT6,microvascularfreeflapproved optimalforgradualimprovement,attimeswithsurpassing ofpre-operativescores.36

Neckdissection

While radical neck dissection was formerly seen as an essential measure for securing local tumor control and improvementofprognosis,itisnowadaysreplacedby selec-tiveneckdissection. Thisis associatedwithacomparably low morbidity and acceptable functional results, without havinga negativeimpactonthe prognosis.37 Inany case, the dissectionof the accessorynerve and great auricular nerve can easily cause long-term complications, includ-ing numbness, shoulder pain and motor dysfunction.38 In ourstudy, patients who underwentneck dissection had a lessevident QoLameliorationbetween T1and T6, proba-blyduetotheabovementionedcomplications.Conversely, patients had gone through neck dissection experienced major improvement in appearance at T6 compared to patientsnotsubmittedtoanydissection.Neckdissectionis usuallyperformedwithaninvasiveapproach,whichleaves avisibleandnon-estheticscar,butthankstoadvanced sur-gicaltechniques,estheticresultbecomesacceptable once tissuehealinghasoccurred.39Significantly,acorresponding improvementin anxiety sensation wasregistered at T6in thesamegroupofpatients.Swallowingimproved atT6in patientswhounderwent neckdissection. This is in accor-dance with literature, which states that in case of neck dissection,swallowingfunctionrehabilitationallowsa con-tinuousprogressbetweenT1andT6.40

Ofcourse,furtherinvestigationssuchasconsideringthe development of acute/chronic side effects during cancer therapyandevaluationoflong-termQoLandsurvival,would becomplementary to the presented results, aswould be inclusionofmoreexplanatoryvariablessuchasevaluation ofeducation,maritalstatus,comorbidities,riskfactorsfor OSCC.Inourfutureplans,wehaveinmindtoperformthe sameQoLanalysisinapre-operativephase,butalsotoadd apsychological analysisthroughadedicatedquestionnaire

before and aftertreatment. We believe this would be of great help to perform the best supportive care possible, togetherwithaprolongedfollowup.

Conclusions

This studydiscusses theissueof subjectivepost-operative QoL improvementaftermajorsurgery for OSCCwithinsix months.Infact,despitethefactthatthelackofevaluation ofQoLbeforesurgerycouldbeconsideredalimitation,the addressedobjectivewasthedynamicchangingofpatients’ referred QoL outcomes in the short term. Moreover, the modified versionof the questionnairewe propose is more andmoreaccurateinreproducingpatients’stateofhealth betweentimepoints.Ourresultssupporttheneedfor cli-nicianstoestablish adequatesupportivecare immediately aftertheendofsurgicalphases,accompaniedbya patient-oriented andoutcome-orientedtherapy onthelong term. The more invasive the intervention, the more likely it is that the situation will improve within a short period of time.Inotherwords,patientsaffectedbybiggertumorsand submitted tomore invasive interventions, especiallywith complicatedreconstructions,usuallyexperiencesevereQoL alterationsimmediatelyaftermajorsurgery,butarelikely toimprovequicklywithinthefirstsixmonths.
