Abstract

Introduction:Anongoingdiscussionisfoundinmedicalliteratureaboutthereasonsforchanges inthyroidcarcinomaincidencepatternsoverthelastdecades.

Objective: Toanalyzetheclinicalandpathologicalcharacteristicsofthyroidcarcinomacases overadecade.

Methods:Cross-sectionalstudyoveranhistoricalcohort.Medicalrecordsof628thyroidcancer casesinasinglecenterwerereviewed.597patientswereincluded.Microcarcinomacaseswere selectedforaqualitativeanalysisphase,inwhichmedicalrecordswerereviewedforbetter understandingofthyroidnoduleandthyroidcancerdiagnosisprocess.

Results:An increase inthe proportionofcases with thyroidcancer diagnosiswas observed throughout the decade; new cases were predominantly tumors of less than 2cm, with histopathological signs oflow aggressiveness. Therewas anincrease inproportion ofcases withmalignantcytologicalresultsamongmicrocarcinomas.

Conclusion: Thereisatrendforincreaseinthyroidectomiesduetocancerinthisinstitution, withproportionalincrementofcaseswithhistopathologicalcharacteristicsindicativeofearly disease. Among microcarcinomas,thereisanincreasing group representedby cancercases thatwerenotincidentallydiagnosed,relatedtoanenhancementinpreoperativediagnostic methods.

Resumo

Introduc¸ão:Persisteumadiscussãonaliteraturasobreasrazõesparaasmudanc¸asnopadrão deincidênciadocarcinomadetireoidenasúltimasdécadas.

Objetivo:Analisarascaracterísticasclinicopatológicasdoscasosdecarcinomadetireoideao longodeumadécada.

Método: Estudotransversalsobreumacoortehistórica.Osregistrosmédicosde628casosde câncerdetireoidedeumúnicocentroforamrevisados.Foramincluídos597pacientes.Oscasos demicrocarcinomaforamselecionadosparaumafasedeanálisequalitativa,naqualosregistros médicosforamrevisadosparamelhorentendimentodoprocessodediagnósticodonóduloedo câncer.

Resultados: Observamosum aumento naproporc¸ãode casoscom diagnóstico decâncer de tireoideao longo dadécada; osnovoscasos forampredominantementede tumores <2 cm, comsinaishistopatológicosdebaixaagressividade.Houveaumentonaproporc¸ãodecasoscom resultadocitológicomalignoentreosmicrocarcinomas.

Conclusão:Há uma tendência de crescimento nas tireoidectomias por câncer na nossa instituic¸ão,comincrementoproporcionaldecasoscomcaracterísticashistopatológicas indica-tivasdedoenc¸aprecoce.Entreosmicrocarcinomas,háumgrupoemascensãorepresentado porcasoscomdiagnósticonão-ocasionaldecâncer,relacionadosàmelhoradosmétodos diag-nósticospré-operatórios.

Introduction

By far, thyroid carcinoma is the most common type of endocrine cancer.1,2 Papillary carcinoma (PC) is the most

commonsubtypeofthyroidcarcinoma.Althoughmalignant neoplasmsofthethyroidcorrespondtoonly1%ofall can-cers and to 0.5% of all deaths caused by cancer in the world,3,4 autopsystudieshaveshown thatPCisextremely

common and underdiagnosed.One of the best-conducted studies from a methodological point of view was that by Harack et al. In their paper, these authors analyzed 101 glandssectionedat2---3mmintervals.Neoplasticfociwere foundin36%ofcases,allinindividualswithnoprior knowl-edgeofthyroid pathology. Inmost casesthetumorswere represented by small microcarcinomas, and many others mayhavebeenlost,solelyduetothepredeterminedinterval section.5

AlthoughthyroidPCisadiseaseoflowmortality,6,7itstill

remainsacauseofconcern.Thegrowingincidenceofa dis-easewithahigh curerate increaseditsprevalence rates, resultinginsignificantfinancialimpactonsociety.8An

epi-demiologicalreviewamongUSindividualsrevealedthatthe incidenceofthyroidcancerhasincreasedmorethantwicein thelast30years,makingitthetumorwithmostpronounced increaseinincidencebetweentheyears1992and2002.9

Moststudieshaveattributedthisincreasedincidenceto thegreateruseandbetterqualityofdiagnostictools,which couldlead to an increase of incidental diagnoses of pre-viouslyhiddencarcinomas.10 Otherstudies have produced

resultsopposedtothishypothesis, astheyalsoshowedan increaseintheincidenceoftumorslargerthan1cm,leading theirauthorstobelievethat thisincreasedincidencemay alsobe occurring among tumors that werenot diagnosed

occasionally.11 Theseresults arebasedoninferencesfrom

datafrompreoperativecytologicalstudiesandfromtumor diameters.Althoughtodatethereisnostrongevidenceof a global and uniform increase in exposure to risk factors historicallyknownforthisdisease,itstillremainsuncertain whethertheincreaseofnewcasesofthyroidcancerreflects ahigherriskofdevelopingthisneoplasia,orwhetherthisis onlyanartificialresultrelatedtodiagnosticprocedures.

A great number of international studies are based on regional or national statistics. Only a few institutional papersstudied featuresassociatedwiththediagnosis pro-cess, and none of them witha qualitative approach. The authors’ aimwastoanalyzethe patternof occurrence of thyroidcanceroveradecade,inaquantitativeand qualita-tivedocumentarystudybasedonaninstitutionalhistorical series, with a detailed description of clinical, epidemio-logical, and histopathological data, and with information regardingthediagnosticprocessfornodulesandcancers.

Methods

Patients

Age, gender, fine-needle aspiration biopsy (FNAB) results, histopathology findings, stagingand total number of thy-roidectomiesperformedinthesameperiodwereintroduced in a specific database. All data were collected by the same investigator (Girardi FM). Carcinomas were classi-fied according to their major groups: papillary, follicular, medullary, poorly differentiated, and undifferentiated or anaplastic tumors. Patients treated at other institutions, patients witha diagnosis differentfrom carcinoma, cases of concomitantcarcinomaswithdifferent histologyin the samegland,casesofunresectabletumors,andcases with-out tumor diameter information were excluded from this study.

Among thecases included, a convenience sample con-taining all patients diagnosed with microcarcinoma and whosepreoperativeconsultationhadoccurredatthis insti-tutionwasselectedforthequalitativephaseofthestudy.At thisstage,itwasdecidedtoanalyzeonlycasesof microcar-cinoma,simplytodeterminewhetheritcanbegeneralized thatsmalldiametertumorsusuallyrepresentnodesor can-cersof incidental diagnosis. In thispart of thestudy, the diagnosticprocessofthyroidnodulesandcancerswas eval-uated.Forthispurpose, acontentanalysiswascompleted after reading andinterpreting clinic, epidemiological and histopathologicaldata,aswellasanalyzingadmissionnotes, anamnesis,medicalevolutions,surgicaldescriptions, ultra-sonographic reports and clinical impressions described in thepatients’medicalrecords.AccordingtoBardin(1977), theanalysiswasdividedintothreestages:(1)pre-analysis, which included a‘‘floating’’ reading andthe delimitation ofdocumentsfor furtheranalysis;(2)evaluationand clas-sification of the material and aggregation to the content analysis, in order to allow the emergence of hypotheses and the classification of the units; and (3) interpretation of the obtained results.12 The units and subunitsderived

fromthisstageofthestudycanbefoundinTable1.After sortingofresultsinunitsandsubunits,thechi-squaredtest was used to compare proportions between the halves of the decade. Incidentally diagnosed nodules were defined as those diagnosed incidentally in clinical examinations, imagingstudies,orduringneckexplorationbyothercauses unrelatedtothyroiddisease.Incidentallydiagnosedcancers weredefinedasthosewhosediagnosisoccurredwithout sus-picionor confirmationof malignancyinpreoperative tests andevaluations.

Statisticalanalysis

Descriptive analysiswasusedtosummarize thedata.The Kolmogorov---Smirnovtest wasusedtoassessthenormality of continuousvariables. Continuous variables withnormal distribution were expressed as mean and standard devi-ation; those without normal distribution were expressed asmedianandminimumandmaximumvalues.Categorical variableswereexpressedasabsoluteandrelativefrequency. Student’s t-test was used tocompare means of age; the Mann---WhitneyU-testwasusedfor diameteranalysis,and thenon-parametric chi-squaredtest wasusedtocompare categorical variables. Statistical analysis was performed usingEpiInfosoftware,version3.4.3(2007). Alltests con-sideredasignificancelevelof5%.

Pathology

During the study period, the surgical specimens of total andpartialthyroidectomywerereviewedbyoneofthe11 pathologistsofthismedicalservice.In93.2%ofcases,the histopathologicalanalysiswasperformedbythesame pro-fessional,amedicalpathologist withtrainingin headand neckpathologyandwithspecialdedicationtothyroidgland diseases. The pathologicalstaging wasperformed accord-ingtotheseventheditionoftheAmericanJointCommittee onCancerpTNMstagingsystem.13 Tumorswereconsidered

multifocal when two or more foci were found in one or bothlobes.Hashimoto’sthyroiditiswassuggestedbasedon histopathologicalfindings.Microcarcinomasweredefinedas tumorsofdiameterequaltoorlessthan1cminthe histo-logicalexamination.Lymphnodestatuswasdefinedbased onevidenceof metastasesinlymphnodesremoved. Inva-sion or extraglandular involvement was defined based on evidence of tumor cells infiltrating beyond the glandular capsule on microscopic examination. The term follicular lesionwasusedonly forbenigntumorswithfollicularcell groups arranged in thyroid follicles, excluding follicular carcinoma and the follicular variant of thyroid PC.14 The

cytologicaldiagnosiswasmadeaccordingtoBethesda crite-riapriorto2007,in whichthelesionswereclassifiedinto fivecategories:Unsatisfactory(I),Benign(II),Undetermined (III),Suspectformalignancy(IV),andMalignant(V).15

Results

Quantitativephase

BetweenJanuary1st,2001andDecember31st,2010,3536 thyroidectomyprocedures wereperformed at this institu-tion,628(17.7%) ofthem due tocancer.Seventeen cases were excluded for being malignant neoplasms of other histological subtypes (thyroid lymphomas, sarcomas, and metastasestothethyroidgland),twocaseswereexcluded for having been treated outside of this institution (one follicular carcinoma and one medullary carcinoma), four for being unresectable cases (all anaplastic carcinomas), five cases for having more than one concomitant tumor subtype in the same gland, and three cases due to lack ofdiameter information(two PC andone follicular carci-noma).Atotal of 597patients met theinclusion criteria. The male/female ratio was 1:3.9. The average age was 48.11±14.97years(range:12---91years).Theaveragetumor diameter was 2.6±1.99cm (range: 0.04---5cm). In this universeof597patients,549(91.9%)underwenttotal thy-roidectomy,whiletheremainingweretreatedwithpartialor subtotalthyroidectomy.Between2001and2010,the num-berofthyroidectomyoperationsfromallcausesincreased 1.52-fold.Inthesameperiod,anincreaseof2.15-foldwas observedinsurgeriesforthyroidcancer(p=0.0002)atthis institution.There were189 casesoperated onin thefirst halfofthedecade,and408casesinthesecondhalf.

Thyroid PC remained the predominant tumor in both periods. The percentage of cases with signs of multifo-cal disease increased in the second half of the decade. Conversely,adecreaseincapsularinvasionandin extraglan-dularinvolvementrateswasobservedin thesameperiod, as well as a reduction in tumor size. An increasing pro-portion of cases in an early stage of disease over the years was also observed, although with marginal signifi-cance (x2=3.811; p=0.0509). In addition, an increase in

thepercentageofcasesofmicrocarcinomaassociatedwith follicularlesionsbetween theanalyzed periods (26.4%vs.

33.3%,respectively)wasfound,althoughwithoutstatistical significance(p=0.1112).Themeandiameterofnodules dif-feredbetweenpatientswithconcomitantfollicularlesions

vs.patientswithglandswithusualcharacteristics,except for thepresence of carcinoma (U=132,654.0; p=0.0182). OtherdataaresummarizedinTable2.

Overthetenyearsofstudy,220casesofthyroid micro-carcinomaswerediagnosedandunderwentsurgery;179of themwerewomen(81.3%)withmeanageof48.91±12.31 years.Ofthesecases,89%(196cases)underwenttotal thy-roidectomy asdefinitive treatment. In 214 cases (97.2%), the histopathological diagnosis revealed PC; five (2.2%) weremedullarycarcinomas;andonlyone(0.4%)was diag-nosedwithafollicularthyroidcarcinoma.Themeantumor diameterwas0.58±0.26cm.Amongmicrocarcinomas,146 (66.3%)met criteriafor inclusionin the qualitative phase of thestudy, 28 belongingto thefirst half of the decade and118 tothesecond. The analysiswasdivided intotwo

parts:dataassociatedwiththediagnosticprocessfor nod-ules; and data associated with the diagnostic processfor cancers.Anyother subunit wasorganizedandcoded.The descriptivedataandcomparisonsbetweenthehalvesofthe decadearesummarizedinTables2and3.

Qualitativephase

Data relating to the diagnostic process for nodes (A): from 146 records, 117 (80.1%) did not contain informa-tion relatedto thediagnostic process of thyroid nodules. Intheremaining29cases(19.8%),twocategoriesemerged: patients with an incidental diagnosis of nodule (A-1) and patientswithanon-incidentaldiagnosisofnodule(A-2).The authorsdidnotfindstatisticallysignificantdifferenceswhen comparingtheA-1andA-2units(2=0.279,p=0.579),and

theA-1.1,A-1.2,A-1.3,andA-1.4(thelatternotapplicable) unitsbetweenthetwohalvesofthedecade.

Discussion

The increasedincidenceof thyroidcancer in thelast two decades has been observed in several studies.16---20 Few

studies in Brazil analyzed incidence variations in thyroid carcinoma.21,22Datafromvariousregionsoftheworldhave

shown an increasingincidence, from60% to 90%,or even more.Althoughthepresentdataarebasedonasingle cen-ter,thisstudyalsoobservedan increaseintheoccurrence ofthyroidcancerovertheyears,inproportiontothetotal numberofthyroidectomyproceduresperformed, afinding whichisinlinewithotherinstitutionalseries.17,18

Just as observed by other authors,23,24 the proportion

of small tumors (<1cm) increased dramatically over the past years in this sample. There was a predominance of histopathological features of low tumor aggressiveness amongnewlydiagnosedcases,suggestingtheestablishment ofearlierdiagnoses.Intheliterature,thereisstillan ongo-ingdiscussionaboutthereasonsforthisphenomenon.There is no way to infer the level of impact of each factor on changesinthepatternofdiseaseoccurrence.Manystudies haverelatedthevariationsinincidencewiththeincreased useandtothebetteraccuracyofdiagnostictools,16 which

wouldleadtoanincreaseofnewcasesofincidental diagno-sis.However,theparametersfordefinitionofacarcinoma asan incidental eventvarysignificantly.Some studiesuse thecriterion‘‘lackofsuspiciousorconfirmatorycytological results for malignancy’’25; others use the incidental

find-ingofcancerinthyroidectomyproceduresformultinodular

goiter,10andfinallyothersarebasedonanodulefound

occa-sionally in imaging studies performed for other causes.26

Moststudies donotconsiderclinical andultrasonographic dataanalysis,neitherdifferentiatecaseswithanincidental diagnosisofnodulevs.cancer.Suchinformationisdifficult tomeasure andmay beaccessed solelyby studies witha qualitativeapproach.

Somestudies supporttheir findings oninferencesfrom preoperative cytology and tumor diameter data. Even amongthesestudies,itisrecognizedthatareviewofthose clinicalcircumstancesthatledtotheidentificationand ther-apeuticdecisionineachcaseshouldbeconducted,withthe aimtoverifytherateofincidentaldiagnosis.11 Case

stud-ieswithanincidentaldiagnosisofcanceronimagingstudies, suchasthatbyShettyetal.,27showthatthepresenceofnew

diagnosesofnoduleor canceramongpatientswithtumors withdiametersgreaterthan1cmdoesnotexcludethe pos-sibilitythatmanyofthesecasesmayhavebeendiagnosed occasionally.

Burgessetal.analyzedthechangeinincidenceofthyroid cancerinTasmaniabetween1988and1998.Theseauthors observed an increase in the use of FNA in the preopera-tiveperiodinpatientsundergoingthyroidectomy,bothfor tumorsgreater thanor equalto1cm,andfor tumorsless than1cm;thissuggeststhatmorecasesarebeingtreatedby surgery,thankstothepreoperativesuspicionofmalignancy, even among patients with microcarcinomas.11 Sosa et al.

observedthat intheUnited States,theuseofFNAB more thandoubledinaperiodoffiveyears.28Similarresultswere

foundbyBuscoetal.inItaly.29Inthepresentstudy,although

withthyroid nodules, butalso that therewasa trendfor surgical treatment of newcases of microcarcinomas with strongsuspicionorcytologicalconfirmationof malignancy. Patientswithmicrocarcinomasdiagnosedbycytologyinthe preoperative period represent an increasing group among thenewcasesthathavebeendiagnosed,probablyreflecting improvedaccuracyofdiagnosticmethods.

In1993Martinezetal.observedarelationshipbetween histopathological analysis method and frequency of diag-noses of microcarcinoma.30 According to reports of the

presentpathologists’group,thenumberofhistological sec-tions in each surgical specimen has also increased over theyears,although theannotation ofsection thicknessin the histopathologicalreport is notroutinely performed in allcases.EventheAmericanCollegeofPathologists(ACP) protocols31 do not mentiona section thicknessthat must

beroutinelyused---afactthat,asiswellknown,can influ-encetheincidenceofocculttumors.Mostoftheinformation related tohistopathological analysismethods is based on indirect results. In a study by Haymart et al., a higher percentage of incidental microcarcinoma detections was foundamongpatientsundergoingsurgeryforfollicular neo-plasms, compared toother benign conditions (9% vs. 6%, respectively).25 Similarly, anincreasingrateofcarcinomas

associatedwithfollicularlesionswasalsoobserved, when the two halves of the decade were compared, although withoutstatisticalsignificance.Usually,agreaternumberof histologicalsectionsarecarriedouttodifferentiatebenign follicularlesionsvs.carcinomaswithafollicularpattern.It wasalsonotedthatthemeandiameterofthenoduleswas significantlydifferentamongpatientswithassociated follic-ularlesionsandwithglandswithoutotherfeatures,except forthepresenceofcancer.Similarly,inthequalitative anal-ysisphase,arisinggrouprepresentedbypatientsoperated for goiteror ecographicallysuspicious noduleswasnoted, with or without previous FNAB, showing follicular lesions orHashimoto’sthyroiditisinnodularform,associatedwith thepresenceofmicrocarcinomasincidentallydiagnosedin histologicalsections.

Based on these results, the authors cannot suggest another reason for the changes in pattern of occurrence observedbesidesthoserelatedtomedicalbehavior,andto the accuracy and dissemination of diagnostic methods. It wouldbeunlikelytoobserveanincreaseintheoccurrence ofdiseaseinsuchashortperiodoftime,ifwerenotfor a relationshiptomedicalpracticesortoanychangein insti-tutionalpolicies,withreferencetosurgicalservicesinthis hospital.The probability ofcancer diagnosis among cases undergoingthyroidectomy hasincreasedover thedecade. Thenewcasestendtopresentwithsmallerdiametersand withlessaggressivecharacteristics.Theseresultssuggesta trendtowardanearlierdiagnosisofthedisease,which,as it is believed,may alsoberelated totheavailability and accuracy of diagnosticmethods. The authors believe that thelackofstatisticalsignificance---whenthedatafromthe qualitativephasewereanalyzed---isduetothelossesandto thesmallnumberofanalyzablecasesineachunitorsubunit, especiallyinthefirsthalfofthedecade.

Otherfactorsmayhaveinfluencedthepatternof occur-rence observed in this study, although it is difficult to establishhoweachofthesefindingsmayhaveaffectedthe finalresult.Increasingly,theprofessionalresponsiblebythe

surgicaltreatmentofthyroidcancerinthisinstitutionisthe headandneck surgeon.Furthermore,thereisa tendency forcompletionofthehistopathological analysisofthyroid specimensbythesamepathologist,whichalsocould inter-ferewithvariationsofoccurrenceoftumorswithborderline characteristics.

Conclusions

Manyfactorsmayhave influencedthechangesinthe pat-ternofoccurrenceofthyroidcancerinthisinstitution.An increasein the proportionof malignantneoplasms among cases undergoing thyroidectomy was found, as well as a trendtoearlierdiagnosesovertheyearsofthisstudy.Itwas observedthatitispossibletofindsmall-diameternodules and neoplasias non-incidentally. Among microcarcinomas, an increase in the proportion of cases with preoperative suspicion of neoplasia was observed, suggesting a trend towardimprovementintheaccuracyofpreoperative diag-nostic methods among small tumors. Cases of incidental diagnosisofmicrocarcinomasinthyroidectomiesforbenign or indeterminate conditions are also increasingly com-mon,suggesting changes in the analytical method and in histopathologicalroutine.Longitudinalstudiescan comple-menttheseresults,reducinglossesbyimprovingthequality ofregistrationofdatarelatedtothediagnosisprocess.
