uring 2003, the first complete year in the new premises inaugurated at the end of the previous year, one of the Department’s oldest projects was accomplished: the launching of the thoracic organs transplantation programme. The first heart transplantation was performed on 26 November, immediately followed one month later by a second procedure, both equally successful. This was clearly only the beginning of a programme which we wish to make coherent and efficient. If the estimations of the national needs in this field are correct - 70 to 80 procedures annually - the implementation of a new transplantation programme is highly justifiable, since the three other programmes already running were clearly unable to cover these needs. In the last ten years, only 12 heart transplants were performed on average each year. Our aim, if the need is there, is to develop a programme which may achieve an annual average of 25 procedures within the next three years. Naturally, the fulfilment of this goal will depend on the cooperation of the Departments of Cardiology of our region, and in particular of that of our hospital, which is part of the programme. If everything goes according to plan, following the experience obtained from the first heart transplant cases, we intend to proceed with lung, and possibly cardio-pulmonary, transplantation, thus honouring the programme’s title: thoracic organ transplantation. As far as the other fields of surgical activity are concerned, there was no significant change by comparison with previous years, other than a slight increase in the procedures performed (1,567). There was, however, an increase in the number of thoracic, non-cardiac (mainly pulmonary) surgeries, which places the Department in a leading position also in this specific field. On the other hand, a qualitative change in the type of pathology was noticed. As expected, the number of coronary revascularisation procedures dropped slightly as a result of the increase in the activity of interventional cardiology. Nonetheless, this was counter-balanced by a significant rise in the number of valve surgeries carried out, in particular the valvulopathies of the elderly. There was a significant rise in the average age of the population treated. On the other hand, the surgery of congenital cardiopathies remained unaltered, albeit with a slightly greater incidence of the more complex malformations. Interventional cardiology also played a significant part in this context.
The end of the year was marked by another important event worth mentioning: the Department performed its 20,000th procedure since it began functioning, in 1988. This means an average of 1,350 surgeries per annum, unequalled by any other cardiothoracic department in the Country. If we add to this a global mortality rate of 1.0%, we have a clear idea of the progress achieved in these last fifteen years, all due to the effort and commitment of the Department’s staff.
The results achieved, as described in this report, indicate a slight drop in the mortality rate, which had previously achieved record lows, favourably comparable with the best observed in other institutions of the kind, both at national and international level, despite of the previously mentioned increasingly complex pathology and age of the
regard to the peri and post-operative treatment. It also shows that in spite of the excellent figures already recorded, it is always possible to achieve better results, for which a greater investment in the technical preparation of all team members from the several professional groups is required.
For all of the above, and for its performance in improving health management, the Portuguese Industrial Association awarded the Centre (one of only two public units at national level) a prize: “To the CENTRE OF CARDIOTHORACIC SURGERY, its Directors, Staff and Workers, in acknowledgement of the quality of the management of factors determining the productivity and of the results achieved”. The Centre was one of 15 case-studies selected in the book ”Produtividade em Portugal, Medir para Gerir e Melhorar” (Productivity in Portugal, Measuring to Administer and Improve).
The academic and scientific activity was maintained. The Head of the Department was appointed Professor of Surgical Pathology for the 5th-year of the Medical School, whilst still in charge of teaching Cardiothoracic Surgery and supervising Vascular Surgery. Dr. Pedro Antunes and other medical staff are responsible for the daily practical sessions of this subject. Some members of the Perfusion group lecture the discipline of Perfusion in the Technical School of Health of Coimbra and are also responsible for the practical sessions to these students in our Centre. During the past year, the Head of the Department was also an invited lecturer in Masters and Post-graduate Courses at the Universities Moderna, Lusíada, Portucalense and Católica, in Lisbon and Porto. Finally, 10 scientific papers were published and another 6 were accepted and await publication in peer-reviewed Journals or as chapters of books. Thirty nine presentations were also made at congresses and scientific meetings in the Country and abroad, of which a significant number by invitation of the respective the organising committees.
