A 10-year-old female was admited with complaining of burning in the mouth for a week. Ater 3 days of the initial symptom, the patient started to present knee and ankle pains and had a temperature of 38ºC. he oral cavity revealed the presence of leucoplakia lesions on the tongue (top and anterior bilateral sides) accompanying the anatomy of the papillae and could not be removed clinically and were painful when chewing and swallowing. he lesions were suggestive of candidiasis in cytopathology exam (Figure A, B and

C). A heliotropic rash was observed on her face and Gotron’s sign was conirmed. Ater substantial clinical and laboratorial evaluation, a diagnosis of dermatopolymyositis was made. he patient was submited to intravenous corticotherapy with high doses of corticoids over a short period. Ater this, oral administration of the medicine was carried out for 5 days, with a gradual decrease in the dosages until its suspension. For the oral lesions, Daktarin® gel (miconazole)

was administrated topically, and on the 5th day of treatment the

lesions had healed completely. Follow up cares were also prescribed as: a hyposodic and hypocholesterolemic diet, antimicrobial use for 15 days and the use of a sun protection cream daily. he patient remained hospitalized for 20 days and all manifestations improved. his case suggests that the knowledge concerning oral lesions and their early identiication are of great importance, mainly because oral manifestations have been widely described as precursors and signs of systemic illnesses, immunosuppressive conditions and infections promoted by diferent pathogenic agents.

Uma criança de 10 anos de idade, do sexo feminino, foi admitida com queixa de queimação na boca, por uma semana. Após 3 dias do sintoma inicial, a paciente começou a apresentar dores no joelho e no tornozelo, associada à temperatura de 38ºC. Na cavidade oral, revelou a presença de lesões do tipo leucoplásicas na língua (sobre e nas bordas bilateralmente) que acompanhavam a anatomia das papilas e que não podiam ser removidas, associadas à dor tanto na mastigação, quanto na deglutição. As lesões foram sugestivas de candidíase em exame citopatológico (Figura A, B e C). Clinicamente, foi observada a presença de heliótropo e do sinal de Gotron. Após substancial avaliação clínica e laboratorial, o diagnóstico de dermatopolimiosite foi realizado. A paciente foi submetida à corticoterapia intravenosa com doses elevadas de corticóides, ao longo de um curto período. Depois disso, a administração oral do medicamento foi realizada durante 5 dias, com uma redução gradual nas dosagens até a sua suspensão. Para as lesões orais, Daktarin® gel (miconazol) foi administrado por via tópica, e no quinto dia do tratamento das lesões, essas desapareceram completamente. Também, foram adotados os seguintes cuidados para a paciente como: uma dieta hipossódica e hipocolesterolêmica, utilização de agentes antimicrobianos, durante 15 dias e à utilização de protetor solar diário (creme). A paciente permaneceu internada por 20 dias e ocorreu melhora das manifestações clínicas. O presente relato enfatiza que o conhecimento sobre lesões orais e sua identificação precoce são de grande importância na prática clínica, principalmente, porque manifestações orais têm sido amplamente descritas como precursores e ainda podem atuar como sinalizadores de doenças sistêmicas, condições imunossupressoras e infecções promovidas por diferentes agentes patogênicos.
