Проведені нами комплексні гістологічні та імуногістохімічні дослідження у хворих на вітіліго, крім відсутності меланоцитів в епідермісі в осередках ураження, показали відсутність різко виражених дистрофічних змін (спонгіоз та вакуолізація базальних кератиноцитів, гіпер- та ортокератоз, гранульоз). ), різного ступеня вираженості судинних розладів мікроциркуляторного шару дерми та вогнищевих, переважно периваскулярних запальних лімфогістіоцитарних інфільтратів з імуногістохімічними маркерами Т-лімфоцитів CD3, CD4 і CD8, при цьому кількість Т-лімфоцитів CD4 CD8. При імуногістохімічному дослідженні при фарбуванні на клітини CD3 позитивна реакція спостерігалася лише в одному або кількох Т-лімфоцитах, розташованих периваскулярно, в цих же ділянках позитивна реакція спостерігалася на Т-лімфоцити CD4 і CD8, тоді як кількість CD4 Т-лімфоцити CD8. The article presents a clinical case of traumatic damage to the distal duodenum as a result of blunt abdominal trauma in the form of complete rupture and segmental post-traumatic thrombosis of the proximal part of the small intestine with analysis of literature data for diagnosis and choice of surgical correction tactics. Rupture of the duodenum after blunt wheel-associated trauma to the abdomen is a statistically rare injury. Early diagnosis is important to reduce complications and mortality, as most severe duodenal injuries require surgical treatment, the choice of which depends on the severity of the injury.
The anatomically deep location of the duodenum provides strong enough protection for anterior and posterior injuries due to the spine and the massive muscle layer. However, the complexity of the diagnosis leads to a delay in determining the damage to the duodenum > 12 hours in 53%, and in 28% the diagnosis time exceeds > 24 hours.
