Transderm al glyceryl trinitrate (nitroglycerin) in healthy

persons: acute effects on skin tem perature and

hem odynam ic orthostatic response

In order to find an explanation for individual reactions to transderm al glyceryl trinitrate (G TN ) w e studied the skin tem perature and hem odynam ic reactions in 63 healthy persons. The data w ere obtained before and after the application of G TN and G lycerin (G L) placebo patches, during one hour. The skin tem perature w as m easured on both forearm s, the local (left sided) and system ic (right sided) reaction on G TN w as related to the skin fold and the calculated body fat content. The bilateral rise of skin tem perature and its duration w as higher and longer in obese than in lean persons m ainly in obese w om en. The U V induced therm o and the later phototherm o-reaction (E rythem a) w as reduced on the left forearm after the application of G TN and G L patches. The observed hem odynam ic G TN effect confirm ed know n postural reactions, such as decreased arterial pressure (i.\m A P

=

-2.9% ), increased heart rate (i.\H R

=

+7,4% ) and aTc prolongation (i.\Q Tc = +4,9% ) in upright position. A n adverse drug effect w ith increased m ean blood pressure (i.\m A P

=

+12% ) and increased heart rate (i.\H R

=

+ 10.4% ) m ainly in supine position w as observed in 11% of the participants, but only in m en. S uch a reaction w as already described by M urell, 1879. Individual G TN effects w ere analyzed and related to habits and fam ily history. In m ale sm okers and in persons w ith hypertensive and diabetic close relatives, the hypotensive G TN effect w as accentuated in supine position. In the upright position the group w ith hypertensives in the fam ily presented a m oderate hypotensive reaction w ithout secondary tachycardia and the sm okers presented only a slightly increased heart rate. O ur observations suggest that individual reactions to transderm al glyceryl trinitrate (G TN ) w ith its active com ponent nitric oxide (N O ) depends on physiological conditions, related to endogenous vasoactive substances, m ainly the interaction w ith E D R F (the endogenous N O ) and the activity of the R enin-A ngiotensin S ystem .
