Through this, we hope to prevent further child deaths and understand if there are modifiable factors that can prevent child deaths in the future. Monitor child deaths occurring as a direct or indirect result of Covid-19 and make appropriate recommendations for action to the Bradford District COVID Outbreak Control Board. In Bradford District, all child deaths up to the age of 18 are reviewed in a Child Death Review (CDR) process and quality assured by the Child Death Overview Panel (CDOP).

During the COVID-19 pandemic period, there was no increase in child mortality in Bradford County (or nationally). This is taken from the national review of child mortality and not from Bradford county analysis. Modifiable factors associated with child mortality in Bradford were smoking during pregnancy, unsafe swimming, suicide, genetically inherited conditions, unsafe sleeping practices and COVID-19 infection (Table 2).

Consanguineous union or marriage occurred in 30% of all chromosomal, genetic and congenital abnormalities (category 7) child deaths in 2019/20 and 2020/21. Two-thirds of all child deaths in 2019/20 and 2020/21 occurred in the most deprived fifth of the national population in Bradford (Figure 12). Over the past two years, Keighley Central has been the only ward to have a statistically significant high infant mortality rate compared to the Bradford average.

Action to prevent child deaths locally

The Emotional Well-being Practitioners (EEWP) in Schools Workforce which has been extended to 2022. It is normal for children and young people to have mild to moderate emotional well-being problems such as worry, anxiety and low mood. , especially during the transition between school, college and work and during times of uncertainty (e.g. the COVID-19 pandemic). A regional series of workshops on self-harm is planned for 2022, attended by professionals working with children with social, emotional and mental difficulties from across the county.

A local suicide prevention campaign is being planned that will involve young actors, including those with experience and insight. The Suicide Prevention Strategy group provides local input into joint research with Bradford University to understand risk and prevention for youth suicide. A range of mental health support for young people in Bradford District can be accessed through Bradford Healthy Minds https://www.healthyminds.services/content-.

These initiatives are part of the local Suicide Prevention Strategy supported by the Suicide Prevention Strategy Group. This group also undertakes surveillance of suicides across all ages (confirmed and suspected) and suicide risk factors in partnership with West Yorkshire Police and the West Yorkshire Suicide Prevention Strategy. When a child aged over 4 and under 18 dies, the legal process undertaken by CDOPs (Child Death Overview Panels) is initiated and accepted as a LeDeR (learning from deaths of people with learning disabilities) report due to the process comprehensive now. in place.

In 2019 following work with the CDOP Co-ordinator for Bradford District reviews were reported covering several years and since then deaths of children in Bradford with a Learning Disability have been regularly reported to the LeDeR system. This has enabled the learning points from these CDOP panels to be immediately addressed by the LeDeR team and acted upon. This means that in 2020/2021 the program can be confident of receiving notifications of all deaths of children over 4 with a diagnosed learning disability and in 2020-2021 8 reviews of children aged 4 to 18 year olds were notified to Bradford District and Craven CCG via the LeDeR Process.

Positive key working relationships have been maintained with the Bradford and Airedale CDOP team with the local LAC (Local Area contact) for LeDeR. Where possible, the Primary LAC for LeDeR now participates in CDOP panels, which has improved triangulation with the CDOP and LeDeR processes.

Membership of Bradford District CDOP

Additional members are admitted to the panel when appropriate for the cases scheduled for review. Since the establishment of the CDOP in 2008, the panel has consistently endeavored to increase the number of cases reviewed each month, and additional meetings are held if necessary to ensure that a backlog does not occur. This also allows for modifiable factors and problems to be identified more quickly and changes in practice to be implemented.

A separate neonatal CDOP meeting is being organized to scrutinize the neonatal deaths in the district with the participation of a consultant neonatologist. This is widely used in most of the 64 CDOPs across the country and was commissioned by Bradford in 2020. This allows any professional who learns of a child's death to notify the Child Death Manager at the Office of Child Death Review through the eCDOP portal.

The Coroner's Office and the Registrar of Births Deaths and Marriages have a statutory responsibility to participate in the Child Death Review process by notifying the Manager of all deaths reported to them. You can therefore be confident that information on all deaths is captured by the Child Death Review Manager. The eCDOP system enables all deaths to be recorded and information is automatically transferred to the National Child Mortality Database (NCMD).

Information requests are sent to agencies to gather known information which is then passed on to the CDR and CDOP meetings.

Terms of reference of Bradford District CDOP

The group may select additional members or specialists as needed for the purposes of specific parts of the work. The CDOP will be chaired by Public Health and will be directly responsible to the Welfare Board. Meetings will be considered quorum or otherwise, in light of the material to be considered and decisions to be made, at the discretion of the Chair.

Permanent meetings of the CDOP will be held twice a month, and additional meetings will be held as needed. Administrative support will be provided by the Child Death Review Office located at Bradford Teaching Hospitals. Agendas and related documents will be circulated at least 5 days before the meeting.

Conflicts of interest will be declared at each meeting regarding the involvement of panel members. Bradford SCB is committed to listening to the views of children and young people who use services and benefit from our protocols. Presented to the Wellbeing Board, the Children and Youth and Family Partnership Council and the Act as One Better Births programme.

Review the Terms of Reference every 3 years (unless appropriate to do earlier) and propose amendments to the Bradford District Health Board. In the event of a dispute or conflict of interest arising between agencies across or within groups, which cannot be resolved, the chairman will bring it to the attention of the chairman for appropriate action and the BSCB Escalation Policy for the resolution of professional differences of opinion will be invoked.

Definitions (preventable, modifiable and category of death)
