Abstract
Background: Emergency department (ED) is an area of high-risk errors in every hospital. Therefore, applying measures to identify and correct errors in admission and transportation process is of great importance.
Aim: To investigate common errors in the admission and transportation processes of patients in emergency department of Imam Reza hospital by failure mode and effects analysis.
Methods: In this descriptive qualitative research, the central emergency department of Imam Reza hospital admission and transportationprocess based on the team members’ decision was selected for evaluation of its errors. This was performed by holding team weekly meetings, using group interview and brainstorming the data were collected and recorded on the final worksheet. The Data were analyzed by determination of Risk Priority Number (RPN), multiplied by the three indicators of severity, probability and possibility of error.
Results: In total, 11 failure modes with RPN 140 and 2 failure modes with high importance in terms of high severity and occurrence were identified as high-risk errors in three domains of errors related to nurses, physicians and patients. Regarding the origin of the error, the most frequent error in three domains was related to the performance. Regarding the type of an error, the most frequent errors in two domains of patient and physician’s errors were the time and defect errors and the most frequent error in nursing domain was false errors.
Conclusion: Failure modes and effects analysis method can help to detect and prioritize errors in admission and transportation process in emergency departments.
