Câncer de Mama Triplo Negativo
• RE negativo, RPg negativo, HER2 negativo
– Dependendo dos limites usados na definição de positividade
de RE e RPg e dos métodos para teste de HER2
• TN responde por cerca de 10% a 17% de todos os cânceres de
mama
• De um modo geral, mais agressivo que os demais subtipos
moleculares, na sua maioria grau 3 e de pior prognóstico que os
tumores RE+
• Cerca de 73% dos CMTN tem carcterísticas basal-like
• Mais frequentemente, CDI sem um tipo específico
• Outras histologias: adenoide cístico, medular, metaplásico,
apócrino, carcinoma neuroendócrino

Biomarcadores
• Necessários biomarcadores capazes de predizer a
resposta às terapias sistêmicas, inclusive derivados de
platina
• A neoadjuvância pode auxiliar do desenvolvimento de
biomarcadores úteis no tratamento do CMTN
• Derivados de platina e biomarcadores mais estudados
– Mutações germinativas de BRCA 1 e 2
– HRD (deficiência de recombinação homóloga)
– TILs (linfócitos infiltrantes de tumor)
Defeitos da Recombinação Homóloga no Câncer de Mama
• HRD caracteriza o câncer de mama em portadores de mutação de BRCA 1 e 2
– Incidência pode chegar a 19% dos CMTN
• HRD também implicada em CMTN esporádico
– Metilação

Tratamento atual do CMTN
• Quimioterapia citotóxica é o tratamento padrão
– Adjuvância e neoadjuvância: antraciclinas,
ciclofosfamida e taxanes
– Doença metastática: taxanes, doxorrubicina
lipossomal, capecitabina, vinorelbina, gemcitabina
• Principais estudos recentes com quimioterápicos

Stromal TILs should be reported as a percentage (the schematic images might provide some guidance). If the percentage of TILs is questionable, discuss the case with a second pathologist. In heterogenous tumors, evaluate different regions and report the average. For this standardized graphic, images were selected that are representative of different TILs levels, based on the results of three pathologists as well as image analysis. The stromal area was marked in each image. The central images are digitally generated graphics showing the same region of interest (ROI) and a similar density of TILs as the corresponding histological image. Please note that the central images contain idealized TILs generated graphically with comparably density, but not with the exact configuration and distribution as the TILs in the histological images.

Conclusões
• Aumento das taxas de pRC
• Biomarcadores e história familiar podem ajudar na seleção de pacientes
que mais se beneficiam de derivados de platina
• Ainda falta demonstrar que o aumento da pRC se traduz em aumento da
SLD e SG e, se o possível beneficio se restringe a subgrupos
selecionados (BRCA 1/2, HRD alta, TILs, etc)
• Qual esquema recomendado? Uso sequencial em pacientes com doença
residual após tratamento padrão?
• Indicações hoje no CMTN com doença localizada:
– Estadios IIIA e B
– LN+
– BRCA 1/2 e/ou historia familiar de câncer de mama/ovário
– Câncer de mama com predominio linfocitário (?)

The HRD score was available for 195 patients, with 81 patients classified as HRD “high” (≥ 42) and 114 as HRD “low” (< 42). (For 25 patients, the test failed.) Dr. Tutt explained that this score was the mean of three analyses by Myriad Genetics and is now the company’s
published cut-point. “The dichotomized
when conducted on the primary tumor DNA,” Dr. Tutt indicated. “ .”
Response rates were 38.2% in HRD-high patients receiving carboplatin and 42.6% for those receiving docetaxel (absolute difference = –4.4%, 95% CI = –26.0% to 17.2%, P = .82).
Among the HRD-low patients, response rates were 29.2% to carboplatin and 34.7% to docetaxel (absolute difference = –5.4%, 95% CI = –22.7% to 11.9%, P = .55).

“We feel that further development of homologous recombination defect assays for BRCA status in non-BRCA1/2 tumors is required, especially in the metastatic setting,” Dr. Tutt suggested.
