A través d a análise de cortes seriados e corados pela hem atoxilina-eosina, foi estudadas a ocorrên
cia de ninhos de T . c r u z i n a veia central e no
parênquim a das supra-renais, n a parede do ventrículo esquerdo e n a v eia cava inferior. D eterm inou-se p ara cada uma destas sedes a área de tecido exam inado e a densidade do parasitism o, bem com o a relação topo gráfica entre o ninho e o exsudato inflamatório. F oi tam bém testad a a correlação entre o parasitism o supra-renálico e a quantidade de focos de exsudados leucocitários no m iocárdio. O s resultados obtidos, associados aos dados d a literatura, perm item -nos concluir que:
a. a m usculatura lisa da veia central das supra- renais é, dos tecidos exam inados, o local m ais
intensam ente parasitado pelo T . c r u z i no chagá-
sico crônico;
b. com o a diferença básica entre o leiom iócito dá veia supra-renálica e d a veia cava está n a riqueza em corticóides do sangue que nutre a parede da primeira, e com o o corticóide tem efeito imunos- supressor e antiinflam atório, favorecendo a so- brevida dos parasitas nos tecidos, é lógico admitir- se que esta prevalência do fleboparasitism o supra-
renálico pelo T . c r u z i seja devido ao am biente
hormonal;
c. o parênquim a supra-renálico n ão se m ostrou pa rasitado em nenhum dos chagásicos, apesar de sua área ter sido a m ais extensa de todos os tecidos examinados;
d. os chagásicos crônicos com fleboparasitism o supra-renálico têm m aior núm ero de focos de exsudato leucocitário n o m iocárdio que os sem parasitas, havendo correlação significativa entre a densidade do fleboparasitism o e a do exsudato focal no m iocárdio, sugerindo que a colonização de parasitas na veia central, de algum a forma, influencia no aparecim ento da m iocardite no chagásico crônico;
e. a agressão inflam atória ao m iocardiócito parasi tado é significativam ente m ais freqüente e acen tuada que ao leiomiócito supra-renálico parasi tado. E ste fato que estaria relacionado à ação dos corticóides. to m a a veia central im portante reser

The occurrence o f T . c r u z i nests in the central
vein and in the parenchym a of the adrenal glands, and in the wall o f the left ventricle and the inferior vena cava was studied b y analysis of serial sections stained by „ hem atoxylin-eosin m ethod. T he area o f tissue exam i
ned from each o f these sites and the density of parasitic infestation w ere determ ined, as well as the topographic al relationship betw een the am astigote nest and the inflam m atory exudate. T he correlation betw een adre nal parasitism and the num ber o f foci o f leucocyte exudates in the m yocardium was also noted. T he results obtained here, taken together w ith those repor ted in the literature, perm it us to conclude that:
a. am ong the tissues exam ined, the sm ooth m uscu lature o f the central vein o f the adrenals is the site
o f the m ost intensive. T. c r u z i m ultiplication
chronic chagasic patients;
b. since the basic difference betw een the leiomyocy- tes o f the adrenal vein and o f the vena cava resides in the abundance o f corticoids in the blood irriga ting the w all o f the former, and since corticoids have an im m unosuppressor and anti-inflam m atory effect favoring p arasite survival in tissues, it is logical to propose th a t this prevalence o f T . c r u z i parasitism in the adrenal vein m ay be due to the horm onal environm ent;
c. the adrenal parenchym a did no t show any parasitic infestation in any o f the chagasic patients, even although its are w as the m ost extensively investi gated among all tissues examined;
d. chronic chagasic patients with adrenal phlebopa-rasitism have a larger num ber o f leucocytic exu date foci in the m yocardium than unparasitized subjects, w ith a significant correlation betw een the density o f phleboparasitism and the density of focal m yocardial exudate, suggesting th a t the colonization o f the central vein by parasites m ay som ehow affect the onset o f m yocarditis in chronic chagasic patients;
e. the inflam m atory aggression against the parasiti zed m yocardiocytes is significantly m ore frequent and m arked than th a t against the parasitised adrenal leiom yocytes. T his fact, w hich m ay be related to the action o f corticoids, turning the central vein into an im portant reservoir W here T . c r u z i succeeds in surviving the antitrypanosom al

im unidade antitripanosom a, que se desenvolve no chagásieo crônico, a qual parece destruir 6 mio- cardiócito e suas am astigotas antes que estas com pletem o seu ciclo intracelular;
f. é muito provável que m esm o o exsudato inflam a
tório focal no interior do qual não se encontra parasitas seja desencadeado p ela existência pré via de m iocardiócito parasitado, cuja agressão inflam atória o destruiu. P o r outro lado, o exsudato menos denso e m ais difuso que, às vezes, se obser v a no m iocárdio, pode representar auto-agressão im unitária.
Pode-se adm itir a seguinte', seqüência p ara explicar a m iocardite chagásica crônica: com o desen volvimento da im unidade o parasitism o tecidual e a parasitem ia d a fase aguda decrescem . A p ó s esta, por ser a veia central d a supra-renal um local privilegiado,
os ninhos de T . c r u z i se perpetuariam com m aior
freqüência, liberando parasitas ou seus constituintes na circulação. E stes atingem o m iocárdio despertando resposta im une com reação inflam atória focal resul tando em destruição de m iocélulas p arasitadas e tam bém das vizinhas, em decorrência de adsorção de antígenos do T . c r u z i à sua superfície. A auto-agressão produziria m iocardite difusa sem relação topográfica com o p arasita e poderia ser devido à antigenicidade cruzada^ ou à liberação de antígenos das miocélulas destruídas na etap a anterior1.

O n the other hand, the less dense and m ore diffuse exudate som etim es observed in the m yo cardium m ay represent im m une autoagression.
T he following sequence m ay be proposed to explain chronic chagasic m yocarditis: w ith the deve lopm ent o f im m unity, tissue parasitism an d the p ara sitem ia o f the acute phase decrease. A fter this T . c r u z i nests m ay persist m ore frequenthy in the w all o f the central vein, a priviledged site, releasing p arasites or their constituents into the circulation. These ento m yocardium and induce an im m une response w ith a focal inflam m atory reaction th a t results in the des truction o f parasitized m yocells as well as surrounding
m yocells because o f the adsorption o f T . c r u z i antigens
on their surface. T h e autoaggression m ay produce diffuse m yocarditis with no topographical relationship to the parasite. T his m ay be due to cross antigenicity^ or to release o f antigens from the m yocells destroyed in the preceeding stag e1.
