The recent report on “Achilles tendon xanthoma” is very interesting.1 Fernandesetal. concludedthat imaging
stud-ieswereusefulfordiagnosisofAchillestendonxanthoma.1
In fact, the Achilles tendon xanthoma is rare. The differ-ential diagnosis should include gouty nodule at Achilles tendon. An important concern is the cost-effectiveness of MRIimaging use. Itshould be specificallyselected in case ofunderlyinghyperlipidemia.Besides,itisreportedthatthe diagnostic property of the MRI is not good. Dussault and Kaplanconcludedthat“xanthomaappearanceinMRIisnot pathognomonic”.2
