N o p erío d o d e setem bro a no v em bro d e 1994 fo ram tratado s 21 p acientes co m leishm anio se m u c o sa ativ a , p re d o m in a n te m e n te ad u lto s lav rad o res d o sexo m asculino , co m sulfato de am ino sid ine, l 6mg/ kg/ dia p ela via intramuscular p o r 20 d ias. Tre z e p ac ie n te s nu n c a tinham receb id o tratam ento esp ecífico co ntra a d o ença m uco sa e o ito tinham sido tratad o s sem sucesso c o m G lu c an tim e ® . O d iag n ó stic o b ase o u -se in ic ialm e n te em c rité rio s e p id e m io ló g ic o s, clínico s e no s resultad o s da intrad em o rreaçâo d e M o ntenegro e a im uno flu o rescência ind ireta para antico rp o s sérico s anti-leishm ânia e d urante o aco m p anham ento no s resultad o s d o s estud o s p arasito ló g ico s. Sessenta e sete p o r cento d os p acientes tiv eram d iag nó stico p arasito ló g ico co nfirm ad o send o a ino cu lação d o m aterial d e bió p sia d as lesõ es em ham sters o m éto d o mais sensív el. D urante o tratamento o s p acientes fo ram av aliad o s cad a d ez d ias e d urante o p erío d o d e aco m p anham ento d e um ano , cad a três m eses (tem p o m éd io d e aco m p anham ento d e 12,6 m eses). A ad esão ao tratam ento fo i d e 100% e ho uv e irregularid ad es secund árias à falta d e p esso al treinad o p ara ap licar a injeção e à d or no lo cal da ap licação . O efeito s co laterais fo ram d o r no lo cal da injeção ( 86% ), p ro teinúria lev e (24% ), elev ação d o nível sérico d e creatinina ( 5% ) e p erd a aud itiv a subclínica em um d o s d o is p acientes q ue realizaram aud io metria. O bserv o u- se cura clínica em 48% d o s p acientes e a p ercentag em acum ulad a d e recid iva fo i d e 29% ( 4 / 1 4 p a c i e n t e s ) d u r a n te o p e r ío d o d e aco m p anham ento d e um ano . N ão fo i p o ssív el e s ta b e l e c e r d i f e r e n ç a s s i g n i f i c a ti v a s n o co m p o rtam ento d o s p acientes em relação co m o u so p rév io d e Glucantim e® .

Fro m S e p t e m b e r to n o v e m b e r 1994, 21 p atients w ith activ e m uco sal leishm aniasis w ere treated w ith am ino sid ine sulp hate l 6mg/ kg/ day by intram uscular injectio n fo r 20 days. They w ere p rin c ip ally ad u lt m ale ag ric u ltu ral w o rk e rs. Thirteen p atients had no t receiv ed sp ecific treatm ent and eig ht had failed to resp o nd to Glucantim e® therapy. D iagno sis w as b ased o n clinical and ep id em io lo g ical o b serv atio ns, a search fo r the parasite, leishm anin skin sensitiv ity and ind irect flu o rescent antibo d y sero lo g ical te s ts . Six ty s e v e n p e r c e n t o f p a tie n ts h ad Leishmania parasites iso lated fro m ino culated h a m s te r s o r v i s u a l i z e d in i m p r i n ts o r histo p atho lo g ical sectio ns. D uring treatm ent patients w ere seen ev eiy ten d ays and ev ery three m o nths afterw ard s fo r o ne year (m ean fo llo w -up 12,6 m o nths). All p atients co m p leted treatment, any irregularities b eing d ue to ab sence o f a drug ap p licato r and p ain at the injectio n site. Sid e effects w ere p ain at the injectio n site (86 %), s lig h t p r o te in u r ia ( 2 4 % ) , e le v a te d se ru m creatinine ( 5%) and subclinical hearing lo ss in o ne o f tw o p atients w ho did aud io m etric tests. Clinical cure w as achiev ed in 48% and the accum ulated relap se rate w as 29% (4/ 14). D uring the year o f fo llo w -up no d ifference in resp o nse co u ld be d etected related to p rev io us use o f Glucantim e®.
