An82-year-oldmalepresentedwithdysphagiatosolidsand
liquids,weightloss,odynophagiaandleftcervicalpainfortwo
months.Feverornightsweatswerenotreported.An
otorhino-laryngologicalexaminationrevealedpermeablenostrils,free
cavum,erythemaofthearytenoidmucosa,andasuspicionof
amalignantlesionintheleftarytenoepiglotticfold.No
lym-phadenopathyofsignificantsizewaspalpable.Biopsyshowed
neoplasticcellswithpositiveimmunohistochemicalpattern
forLCA,Bcl-2,Bcl-6,Epstein–Barrvirus(EBV),CD5,CD30,CD10,
cyclinD1andMUM1,allowingthediagnosisofdiffuselarge
B-celllymphomaofgerminalcenterorigin.Moderatelyhigh
Ki-67labeling(60%)indicatedamalignantnatureofthetumor.

Fluorine-18 fluorodeoxyglucose positron emission
tomography-computed tomography (18F-FDG-PET/CT)
revealed a hypermetabolic lesion resulting in left
trans-mural thickeningofthe larynx withslightdisplacementof
themidline(Figure1).
Primarylaryngeallymphomasareanextremelyrareentity
representing less than 1% of all extranodal lymphomas
withreportedmaleand left-sidedpredominance,and
loca-tion mainly supraglottic.1,2 A definite diagnosis can only
be obtained by immunohistochemistry, as the radiological
findings are similar to primary squamous cell laryngeal
cancers.3
