An otherwise healthy 7 4 -year-old Brazilian man, employed as
a rural worker in the sugar cane plantations, was referred to the hospital with a raised skin lesion on the left cervical area, left
shoulder, and ipsilateral dorsal thoracic region. The disease started
two years before admission with nodules which evolved with bright
tumefactions, formation of multiple sinuses, fistulous tracts that communicated with each other and ulcerated areas of the skin.
The progressive proliferation of granulation and scar tissue led to
enlargement and disfigurement of the affected part ( Figures A and
B) . Sinus tracts drained whitish grain-filled pus. After the initial discomfort he noticed the spread of the skin lesion and an increase
in local pain during the previous 6 months. The patient was treated
with skin creams and intravenous antibiotics, without improvement.
The material draining from the fistulous tracts was collected ( Figure A) and the grains were separated for culture and direct examination
for bacteria. A filamentous Gram positive coccus-like agent was
observed after staining ( Figure C – yellow arrow) . A modified
Ziehl-Neelsen stain ( Kinyoun stain) identified No c a rdia sp ( Figure C –
black arrow) . White-yellowish granular bacterial colonies grew in
the c ulture medium used ( Sabouraud’s agar, blood agar and
c hoc olate agar) , thus c onfirming the diagnosis of No c a rd i a
b ra si li e n si s. The patient was treated with a c ombination of
trimethoprim-sulfamethoxazole ( 1 6 0 /8 0 0 mg) twice daily for 1 2
months. There was c omplete rec overy from the infec tion and
disappearance of the corresponding symptoms, but a residual dark
scar remained in the affected area.
O pac iente, de 7 4 anos de idade, trabalhador em região canavieira de Minas Gerais ( Lajinha) , relatava a presença de lesões
cutâneas elevadas em região cervical, ombro e dorso esquerdo de
iníc io , havia do is ano s. Os nó dulo s e vo luír am par a le sõ e s
tumefeitas, de superfíc ie brilhante, c om fístulas múltiplas de desembocadura carnosa e lesões ulceradas que drenavam material sero-purulento e pequenos grãos brancacentos. A proliferação de tec ido granuloso e a presenç a de c ic atrizes provoc aram tumoraç ão e deformaç ão da área afetada ( Figuras A e B) . O desconforto inicial deu lugar à extensa lesão cutânea que evoluiu com dor forte nos últimos seis meses. Fez uso de cremes para a pele e antibióticos sistêmicos, sem melhora. Parte da secreção que drenava das fístulas foi coletada ( Figura A) , separando-se os grãos para exame bacteriológico e cultura. Ao Gram, identificaram-se filamentos cocóides Gram positivos ( Figura C - identificaram-seta amarela) . realizadas em ágar Sabouraud, ágar sangue e ágar c hoc olate c r e sc e r am c o lô nias gr anulo sas de c o r b r anc o -amar e ladas confirmando o diagnóstico de No c a rdi a b ra si li e n si s. O paciente foi tratado com a associação sulfametoxazol-trimetoprima ( 8 0 0 / 1 6 0 mg) duas vezes ao dia, durante 1 2 meses. Houve melhora c o m ple ta do q ua dr o in fe c c io s o e da s in to m a to lo gia correspondente, permanecendo apenas hipercromia residual na região afetada.
