Objective:TheHipFunctionRecoveryScorequestionnaireisaninstrumentthatwas devel-opedforevaluatingthefunctionalrecoveryofpatientsover60yearsofagewhoundergo surgicaltreatmentduetohipfractures.Theobjectiveofthisstudywastomakea thor-oughtransculturaltranslationofthisquestionnaire,withadaptationtotheindividualand culturaltraitsoftheBrazilianpopulation.
Method:Thistranslationmethodconsistedofinitialtranslation,backtranslation,drafting ofaconsensualversionandpretestingwithcommentsinvited.Subsequently,afinalversion wasdrawnupaftermakingthenecessaryadjustments,withoutalteringthesemanticsof thequestionsintheoriginaltext.
Results:Theconsensualversionwasappliedtothirtypatientsovertheageof60yearswho hadundergonesurgicaltreatmentduetohipfractures.However,somedifficultiesin under-standingsomewordsandexpressionswereobserved,andthesewerethenreplacedwith termsthatweremoreeasilyunderstood.Afterthefinalversionhadbeendrawnup,thiswas appliedtothesamepatientsandfullunderstandingwasachievedamongsomeofthem, withoutalteringthesemanticsofthequestionsoftheoriginaltext.
Conclusion: ThetransculturaltranslationoftheHipFunctionRecoveryScorewillhavean immediateimpactonfunctionalevaluationsonpatientsover60yearsofagewhounderwent surgeryduetohipfracture.ItwillsubsequentlybepossibleforotherBrazilianscientific studiestousethisquestionnaire,whichhasbeenstandardizedandadaptedtoBrazilian culture,inordertomakecomparisonsbetweenresults,therebyenrichingBrazilianscientific production.

Método: Estemétodoconsisteemtraduc¸ãoinicial,retrotraduc¸ão,elaborac¸ãodeumaversão deconsensoepré-testecomentadocomposteriorelaborac¸ãodeumaversãofinalapósas alterac¸õesnecessáriassemmodificarasemânticadasperguntasdotextooriginal.
Resultados: Aversãodeconsensofoiaplicadaem30pacientes,acimade60anos,queforam submetidosatratamentocirúrgicodevidoafraturasdoquadril.Foramobservadas, entre-tanto,algumasdificuldadesnoentendimentodealgumaspalavraseexpressões,asquais foramsubstituídasportermosdemaisfácilentendimento.Apósaelaborac¸ãodaversão final,reaplicou-seessaversãoaosmesmospacienteseobteve-seumentendimentototal.
Conclusão: Atraduc¸ãotransculturaldoquestionárioteráimpactoimediatonaavaliac¸ão funcional dospacientescommaisde 60anosoperados devidoa fraturasdoquadrile, posteriormente,outrostrabalhosnacionaispodemusaressequestionáriopadronizadoe adaptadoànossaculturaparacomparac¸ãoderesultadoseenriqueceraproduc¸ãocientífica doBrasil.

Introduction
Hipfractures includetranstrochanteric, femoralhead/neck andacetabulumfractures.Thisgroupoffracturesoccursin allagegroupsandisamajorproblemfortheBrazilianpublic healthsystem.Themostaffectedpopulationisthegeriatric group,astheincidenceofhipfracturesincreaseswithageand doublesevery10yearsafterage50.1
Theincreasinglifeexpectancyofthepopulationinrecent decades,associatedwithamoreactivelifestyleoftheelderly and comorbidities found in this population, such as the reducedleanmass,balance,reflexesandbonemineral den-sityresultsinosteopeniaandosteoporosisandhasledtoan increaseinfracturesinthegeriatricpopulation.2
Mortalityafter hip fractures is close to10% in the first monthandapproximately30%inoneyear.Amongpatients whosurvive,manydevelopareductioninmobilityanddegree ofindependence.3
Most studies have focused primarily on the mortality causedby thesetraumas. However, studies assessing mor-bidity,or lossof functionpost-fracture,are less common.4 Surgical treatment of hip fractures is considered the gold standard,asit reducessome complicationsrelated to pro-longedimmobilization,suchasthromboemboliceventsand pressureulcers.5
Duetotheincreasingincidenceofhipfractures,and, con-sequently,a higher number ofsurgical procedures forthis typeoftrauma,itbecomesnecessarytoevaluatethedegree of independence and quality of life of patients after they haveundergonesurgery.Onewaytoevaluatethefunctional
results aftersurgical treatment ofhip fractures is through questionnaires,aimingtofollowtheimprovementornotof dailyfunctionsinpatients’life.
A study applied a questionnaireto 537 elderly patients withhipfracturewiththreeparameters,self-care, transfers andlocomotion,tomeasurethedegreeofpatients’ indepen-denceafterthesurgicalrepair.6Subsequently,anotherstudy involved 154 fractures of the proximal third of the femur treatedsurgicallyandusedtheHipFunctionRecoveryScore questionnaire(AppendixA)createdbyZuckermanetal.7–9It consistsof11questionsrelatedtotheactivitiesofdaily liv-ing: four relatedto independenceinbasic activities, sixto instrumentalactivitiesand onetomobility.10 However,this questionnairewasusedinitsoriginalversion,intheEnglish language,afteritsliteraltranslation.10
WeknowtheimportanceofthetranslationintoPortuguese ofquestionnairesoriginallywritten inEnglishand alsothe adaptationofthesequestionnairestoindividualandcultural characteristicsofthelocalpopulationtomakethem appro-priate to the culturalcontext of the target population.11,12 Therefore, a standardized translation of English language questionnairesintoPortugueseisfundamentalforanaccurate assessmentofthe informationextracted fromthese trans-lated questionnaires, which allowsa better comparisonof scientificstudiescoveringdifferentsurgicaltechniquesand different rehabilitation programs in patients that suffered fractures involvingthe hipandsubmittedtosurgical treat-ments.

Material

methods
The present is a cohort, qualitative, observational study, considering that no interventions were performed while patientswerefollowedbythehospitaltraumateam.Thus,we provideonlyanevaluationofthequestionnairesandassess thepossibledifficultiesinunderstandingbythose participat-ingintheresearch.
Themethodoftranslationandculturaladaptationofthe Hip Function Recovery Score questionnaire (Table 1) into BrazilianPortugueseusedthecriteriadescribedbyGuillemin etal.13 and Heruti etal.14 and reviewedbyBeatonetal.,15 whichconsistsinastandardizedsetofinstructionsfor cul-turaladaptationofqualityoflifetools,whichbrieflyinclude four steps: initial translation, back translation, consensus translationandpretest.
Thisstandardizationaimstoachieveaculturaladaptation ofexpressionsso thereis no lossofsemantics duringthe translationprocess.16Thismethodoftranslationconsistsof aninitialtranslation,followedbyabacktranslationandthen anassessmentoftheversionsfollowedbythedevelopmentof aconsensusversionandcommentedpretest,followedbythe creationofthefinalversion.
Initially, the Hip Functional Recovery Score question-naire, in its original version in English, was translated into Portuguese by two certified, independent and bilin-gual translators, whose native language isPortuguese and whoare fluentinEnglish(Table1).Subsequently,both ver-sionswerecomparedandaconsensusversionwascreated. Thisversion was then back-translatedinto Englishby two bilingualtranslators, whosenative language isEnglishand who arefluentinPortugueseand resideinBrazil (Table2).
Thetranslatorsresponsiblefortheback-translation hadno knowledge of the original version ofthe questionnaire in English.
Bothobtainedtranslatedversionswereassessedbyapanel consistingoftranslators,medicalresearchersandateacherof Portuguese,sothat,bycomparingitwiththeoriginaltext, dis-crepanciescouldbecorrectedandaconsensusversioncreated (Table2).
Theconsensusversionwasusedaspre-testandappliedto 30patientsandcaregiversofpatientsthatsufferedhip frac-turesolderthan60years,whounderwentsurgicaltreatment ina referralhospitalin traumatology,toassess the under-standingandacceptabilityofthetoolandtoestimatepossible necessarychangesduetodifficultiesfoundbyresearchersand patientswhenthequestionnairewasapplied.The question-nairewasnotappliedtopatientsorcaregiverswhodidnot have areasonabledegreeoforientationand/or satisfactory cognitioninresponsetothequestionnaires,sothattherewas nointerferenceontheresultsregardingtheunderstandingof thequestions.
All patients invited to participate in the study were instructed onthe study principles andsignedan informed consentform.Thepresentstudywasassessedandapproved byanInstitutionalReviewBoardandisinaccordancewith Resolution466/12.
Afteraconsensusversionwasapplied,meetingswereheld with a hip surgeon, also a member of the team involved withthe research,toassess thedifficultiesencounteredby patientsandthoseapplyingtheconsensusversion.Basedon thesuggestions,thefinalPortugueseversionoftheHip Func-tionRecoveryScorequestionnaire(AppendixA)wascreated, includingsomeexplanationsinbracketsforthoseexpressions considereddifficulttounderstand.

Results
Agreatsimilaritywasobservedbetweenthetwotranslators’ versions(T1andT2).Whenthetermswerenotidentical,we chosethetranslationwethoughtwaseasiertounderstandby theassessedindividuals.Table1showstheoriginalversion togetherwiththetwotranslationsmadebycertified transla-tors.
Amongthedifferencesfoundinthetranslations,wechose the version oftranslator 1for the item ‘bath’,as we real-izedthatthebathtubisnotpresentinthedailylivesofmost ofthepopulation. Wechosethe translationof“capacity to shower”.Fortheitem‘feeding’,wechoosetranslation2,aswe considerediteasiertounderstandtheexpression“tohandle food”instead of“food manipulation”,inaddition to choos-ingthe term“enteralfeeding”insteadof“feedingtube”,as itbest characterizestheuse ofthetoolthataids nutrition. Wealsorealizedthatthereweredifferencesinthetranslation ofthewordbedpanandobtainedastranslationsthewords “aparador”and“comadre”.Weselected“aparador”,asinitially translatedbytranslator1.Anothertopicthatgenerated dis-cussionwasthetranslationoftheword“bending”intheitem “housework”,shownas“flexion”(T1)and“bow”(T2).Inthis casewechosetheexpressionusedbytranslator1.Another importantdisparityoccurredinthecomponent“mobility”,for whichwechosetranslation2,asitreferstowalkingoutdoors, doesnotusetheexpressionwalkingintheexternal environ-ment.
Afterdraftingtheconsensusversion,weappliedthis ver-sionto30patientsinordertoassesscomprehension,semantic equivalence,possibleculturaldiscrepanciesandanalyzedthe conceptoftheproposedtranslation.Fromthepointofview ofunderstanding thequestionnaire,therewasnodifficulty inunderstandingthetranslatedwords.However,therewas difficultyregardingsomehabitsoftheBrazilianpopulation. Analyzingitemsthatreferto“doinglaundry”,“housework” and“foodpreparation”,weobservedthatelderlymenarenot
used todoing domesticchoresand havedifficulty answer-ing these items. In the item “buying food”, we expanded thepossibilitiesofshoppinginstorestoincludeshoppingat thesupermarket,bakeryandgrocerystorebecauseculturally, womeninBrazil domoreshopping insupermarkets,while mendotheusualeverydayshoppinginbakeriesandgrocery stores(e.g.,tobuybread).
In the item “domestic work”, we chose toput between parenthesesthewords“lifting”and“bending”whenreferring tothewordselevationandflexion,respectively,withtheuse ofsuchwordstoincreaseunderstandingbytherespondents. Afterthesuggestedmodificationsweremadeinagreement with the authors during the application of the consen-susversion,the finalversion oftheHipFunctionRecovery Score questionnaire was drawn up. This version can be seen inAppendix A and was applied to the 30 previously assessed patients. At the reapplication, full understanding wasachievedbythepatientswithrespecttothesemantics.
Discussion
Orthopedistsusuallyevaluatetheirsurgicalresultsbasedon parameterssuchaspositioningofthesynthesismaterial,bone healing rate, percentage ofinfection and range ofmotion. However, particularly in the elderly, a satisfactory surgical procedure,fromthetechnicalpointofview,doesnot neces-sarilyresultingoodfunctionalresults.Basedonthatfact,we aimedtobringtotheBrazilianmedicalcommunitythe trans-cultural standardized translation of a globally well-known functionalassessmentquestionnaire,whichhadseveral cita-tions in national medical literature, previously used in its literaltranslationversion,butwithouttheconcerntoadapt it totheculturalrealityofourcountry,whichpreventsthe comparisonbetweennationalarticles.
cognitiveabilityand lowfunctionalcapacityatthe timeof hospitaldischarge.17Factorssuchasfemalegender,absence ofdiabetesmellitus,capacityofindependentambulationand notlivingalonebeforethe fracturecanbefactorsofbetter prognosis.18
Associatedcomorbiditieshavebeenrecordedasprecursors ofmortalityafterhipfractures.Theeffectofcomorbidityon mortalityhasbeenmeasuredbothbythenumberandthetype ofcoexistingdiseases.Patientswithmorecoexistingdiseases haveahigherchanceofdying.19
Inthistypestudy,wedecidedtoalterthestructureofthe originaltoolaslittleaspossible,bynotincludingor exclud-ingitemsofthequestionnaire,soasnottopromotemajor changesofthepsychometricpropertiesandallowthe com-parisonofversions.
We sought to ensure homogeneity between the ver-sions,changingonlyminordifferencesinculturalhabitsof life, which are relevant for the local population. Certified translators who participated in the work were inter-preters/translatorsfrom different health areas, inorder to minimizetechnicaltermsthatcouldhinderthe understand-ingbythegeneralpopulation.
Inthepre-test,thereweresomedifficultiesin understand-ingsometerms,whichafterfurtherdiscussionandcreation ofthefinalversion,showedeasyreproducibilityand under-standingamongpatientsafterthenewapplication.
Conclusion
Theprocessoftranslationandculturaladaptationof question-nairescreatedinaforeignlanguagemustbecarriedoutvery carefully,aswasdonewiththeHipFractureRecoveryScore questionnaire.Weobtainedanexcellentunderstandingbythe respondentsofthequestionsafterthepreparationofthefinal Brazilianversion.Subsequent scientificmanuscriptswillbe abletousethisstandardizedquestionnaire,whichcanbeused tocomparetheresultsintheassessmentofseveraltreatments andrehabilitationprogramsofelderlypatientssubmittedto surgeriesduetohipfracturesandimprovethenational scien-tificproduction.
